Patient Relations Liaison
This position enhanced relationships with local hospitals, doctors and other ren
al care providers by enabling patients and families to make informed decisions r
egarding their dialysis provider.
The core duties of this position included the following:
· Developing and implementing DaVita’s Patient Management Program by building relatio
ships and providing educational materials to patients
· Conducting consultations which included answering questions from patients and th
eir family members in accordance with DaVita’s policies
· Planning and coordinating patient registration
· Working with hospital staff and Outpatient Clinics to assist with patient transi
tion from Inpatient to Outpatient Facility to ensure patient satisfaction
· Extensive interaction with patients to ensure their needs were being met
· Preparing detailed reports of weekly placements and education activities
