Serve as Care Coordinator, Discharge Planner, and Geriatric Resource Nurse at 75
0-bed teaching and research medical center. Support 25-bed inpatient department.
Perform discharge risk screening and design discharge plans from admission to f
inal discharge, to reduce patient length of stay. Oversee all patient education
relating to discharge planning. Additionally support KCI wound therapy V.A.C. un

or appropriateness of admission using InterQual criteria. Coordinated all hospit
al admissions, admissions from ER, direct admission, acute-to-acute transfer, PA
CU, and other areas. Placed nursing float pool staff to maximize efficiency. Ens
ured all certifications and competencies up-to-date for home health care technic
ians used by hospital for 1:1 sitting.
