Overdenture Abutments The Overdenture abutment offers a simple solution for stabilizing an overdenture with implants. An O-Ring embedded in the overdenture acrylic engages the undercut of the Overdenture abutment. Alternatively, an insert of polysiloxane material within the overdenture acrylic engages the undercut of the overdenture abutment. Proper preparation with either arrangement will provide resilience in the connection.

UMA System UMA System Abutments The UMA (Universal Modification Abutment) system is used for screw retained restorations where it is desired to have the points of attachment superior to the soft tissue. Where there is insufficient clearance to use a UMA or when a sub-gingival separation is desired, a UCLA abutment can be used to achieve separation of the restoration at the implant level. The UMA is a transmucosal collar that should be secured to the implant with 30 Ncm of torque. The UMA screw used to secure the restoration to the abutment is packaged with each UMA plastic coping. A 0.05 hex driver is used to secure the screw which should only be tightened to 20 Ncm. Standard UMA System Abutment he upper diameter for all UMA System T Abutments is 3.85 mm.

Clinical Steps
1 Expose the implant by removing the flared healing abutment with a 0.05 hex driver and rinse with an air-water syringe. Note: If water purity is not confirmed, sterile saline or sterile water is recommended.
46
2 Engage the hex portion of the flared transfer coping onto the hex portion of the implant using a 0.05 hex driver and tighten with finger pressure (left graphic). Note: It is important to take a periapical X-ray to verify the fit between the transfer coping and the implant (right graphic). Note: A long transfer coping screw is available for an open tray technique.
3 Block-out the transfer coping hex opening with a cotton pellet or soft wax to facilitate access after the impression. Inject impression material around the transfer coping (left graphic). Generously fill the customized impression tray with a heavy body impression material and seat the impression tray to take the impression.
4 After taking the impression remove the transfer coping and attach it to the implant analog. Place the transfer coping back into the impression, aligning the flat surface of the transfer coping with the flat surface in the impression (right graphic). Send the impression to the laboratory and include the impression of the opposing arch and a proper bite registration. Note: After the impression has been taken, replace the healing abutment or use a temporary crown fabricated chairside or supplied by the laboratory. Temporary crown: Shorten the temporary abutment to appropriate occlusal scheme. Attach the hexed temporary titanium abutment onto the implant and torque to 20 N/cm. Attach the crown form or shape the resin to the temporary abutment using self-curing resin. Allow for setting, remove and trim acrylic access.

5 Stone Master Model: Soft tissue material is injected around the transfer coping before pouring the stone master model. If requested by the clinician the laboratory can create a temporary crown using a temporary abutment.
6 Remove the transfer coping and place the flared hexed titanium abutment onto the implant analog in the stone model. The titanium abutment can be shortened and modified in preparation for the cemented crown. The titanium abutment margins may be prepared subgingival. To facilitate the process, the titanium abutment can be removed and modified as desired by attaching it to an extra implant analog (left graphic). Note: Thickness on the titanium abutment to protect the hex access hole. 7 Wax a coping over the prepared titanium abutment. Cast the coping in precious or semiprecious ceramic alloy. Finish the coping and seat passively onto the abutment. 47
8 Apply porcelain or other veneer material and return the crown to the clinician. A mark can be placed at the buccal surface of the titanium abutment to facilitate the orientation for the dentist.
Clinical Steps
9 Remove the healing abutment or temporary crown. Insert the titanium abutment with a 0.05 hex driver tip and tighten with finger pressure. Take an X-ray. Torque to 30 N/cm. Retighten in 10 minutes. Try-in the crown, evaluate occlusion and esthetics. Block-out the screw access hole with a cotton pellet and cement the final single unit onto the titanium abutment. Equilibrate occlusion as necessary.
Patient Follow-Up: After receiving the crown, the patient is given oral hygiene instructions. Recall appointments are recommended.

5 Stone Master Model: Soft tissue material is injected around the transfer copings before pouring the stone master model. If requested by the clinician the laboratory can create a non-hexed temporary bridge using temporary abutments.
6 Remove the transfer copings and place the straight hexed abutments onto the implant analogs. The titanium abutments can be shortened and modified in preparation for the cemented bridge. To facilitate the process, the titanium abutments may be removed and modified as desired by attaching them to extra implant analogs (left graphic). Note: Care should be taken in the selection of abutments as each implant diameter uses a matching abutment with the same diameter. It is important to leave enough height and thickness on the abutments to ensure hex engagement. 7 Wax the copings over the prepared abutments. Sprue, invest, cast and finish the framework for the ceramics. Finish the framework and seat over the abutments. An intraoral try-in is recommended for passive fit verification before applying ceramics. A resin jig may be fabricated to aid the clinician with the orientation of the prepared abutments or alternatively by marking the buccal area of the abutments. 51
8 Apply porcelain or other veneer material to the bridge framework. The final bridge is sent back to the clinician.

Clinical Steps
1 Expose the implants by removing the straight healing abutments with a 0.05 hex driver and rinse with an air-water syringe. Note: If water purity is not confirmed, sterile saline or sterile water is recommended.
56
2 Measure the tissue above the implants to determine the appropriate height of the UMA abutments. The UMA abutments should be slightly subgingival for esthetics. Insert the selected UMA abutments with an UMA abutment driver and torque to 30 N/cm (right graphic). Take an x-ray.
3 Engage the UMA transfer copings onto the UMA abutments and tighten with light finger pressure, using the UMA friction grip driver. Apply impression material around the UMA transfer copings (left graphic). Generously fill the customized impression tray with a heavy body impression material and seat the impression tray to take the impression.
4 After taking the impression remove the UMA transfer copings and attach them to the UMA analogs. Place the UMA transfer copings back into the impression. Send the impression to the laboratory and include the impression of the opposing arch. Note: Place the UMA healing caps onto the UMA abutments with a 0.05 hex driver with finger pressure only.

Laboratory Steps
5 Stone Master Model: The technician pours the stone master model. A base plate and bite rim are fabricated and sent to the clinician for intraoral registration. After the bite registration is returned, the casts are mounted and a denture tooth setup is completed and returned for an intraoral try-in. Once the try-in is returned to the laboratory, a silicon matrix is fabricated to aid in the proper bar positioning.
6 Engage the non-hexed UMA plastic or gold bridge copings to the UMA analogs in the stone master model with a 0.05 hex driver.
7 Use the silicon matrix to determine the bar position between the UMA bridge copings. The bar should be positioned approximately 1mm above the ridge for hygiene purposes.
57
8 Sprue, invest, cast and finish the bar framework. The bar should be cast with hard precious or semiprecious alloy. (Do not use non-precious alloys). Upon completion send the bar framework to the clinician for an intraoral try-in to verify passive fit. After the registration is returned, place the spacer and clip over the bar. Block-out the bar gingivally with plaster and a thin coping of latex material before the polymerization process and finish the acrylic. Note: The final bar and overdenture are returned to the clinician for insertion. Once the cast bar is available from the laboratory, its passive fit should be confirmed through patient try-in and x-ray. 9 Remove the UMA healing abutments and screw the bar into place with a 0.05 hex driver and tighten with finger pressure. Take an X-ray. Torque to 20 N/cm. Retighten in 10 minutes. Place finished prosthesis over bar and equilibrate if necessary.
Patient Follow-Up: After receiving the overdenture, the patient is given oral hygiene instructions. Recall appointments are recommended.

2 Measure the tissue depth above the implants to determine the appropriate height of the O-Ring male abutments to be used. The shoulder of the abutments should be slightly above the tissue (approximately 2mm). Place the O-Ring abutments onto the implants with the abutment driver and torque to 30 N/cm (left graphic). Note: Tighten the O-Ring abutments with finger pressure only if the healing abutments will be re-inserted after the impression stage.
3 Inject a impression material around the O-Ring abutments. Generously fill the customized impression tray with a heavy body impression material and seat the impression tray to take the impression.
4 After taking the impression, insert O-Ring analogs into the impression. The analogs may be secured with wax or adhesive. Send the impression to the laboratory and include the housings and red processing rings for fabrication of the master cast. Note: Keep the white O-Rings for final insertion or send to the laboratory for completion. The existing denture can serve as a temporary restoration.

Laboratory Steps
5 Stone Master Model: The technician pours the stone master model incorporating the O-Ring analogs. A bite rim is fabricated for proper registration and sent back to the clinician. The bite registration is then mounted with the master cast onto the articulator. A base plate bite rim provided by the laboratory is used to determine the proper vertical dimension.
6 Complete the preliminary wax-up on the processing model. An intraoral try-in is recommended for verification of fit and esthetics.
7 Insert the red rubber O-Rings into the metal housings. Place onto the O-Ring analogs. The smaller opening of the housing is positioned occlusally. The housings are blocked out gingivally and occlusally with latex material.
59
8 The acrylic is injected/packed in one step and should be soft before packing, then polymerize the denture. The acrylic is processed and the overdenture is trimmed and finished. Send the overdenture back to the clinician for insertion.
Clinical Steps
9 Insert the white O-Rings if not already in the housing and deliver the final O-Ring overdenture. Equilibrate as necessary. Note: If using an existing denture, the housing in the denture may be cured chairside.
Patient Follow-Up: After receiving the final overdenture, the patient is given oral hygiene instructions. Recall appointments are recommended.
