LBC Project HSE Plan, Ewekoro INTRODUCTION This plan covers the requirements of the accident prevention rules and safety program to be applied to the construction work for the LBC Project th at will be performed by PW and its subcontractors under the supervision of Lafar ge. Its objective is to promote Health, Safety & Environment of person working w ith PW and to prescribe certain rules, procedures and safe practices in order to create a working environment free of condition and factors that might contribut e to an accident or injury /Illness. This HSE plan will provide guideline for sa fe execution of the project. Subcontractors shall abide by all safety rules and other regulations imposed at the site by the Laws of the country and the provisi ons of applicable laws, rules and regulations, including rules and procedures as applicable from Lafarge. LBC Project HSE Plan, Ewekoro COMPANY HSE POLICY It is the policy of PW NIGERIA LIMITED to carry out all its work in ways that pr ovide healthy and safe working practices and avoid risks of injury to anyone or damage to property and environment as a result of the activity for which the com pany is responsible. The overall responsibility for Health, Safety & Environment rests at the highest level, however, all individual employees have important re sponsibility towards HSE and must participate in carrying out the policy. Approp riate training, supervision and information are provided to encourage and enable workers to carry out their duties safely. Regular Inspection and monitoring of the activities is undertaken and accidents or incidents are investigated promptl y so that appropriate action is taken to prevent from reoccurrence. Unsafe work practices by an individual can often lead to accidents even in safe working envi ronment. It is therefore our goal to eliminate both unsafe working condition and unsafe work practices. To this end, it is the policy of our company to: Comply with all regulations governing Health, Safety & Environment at the job site. To take expedient actions to correct or isolate unsafe conditions or work practices . To promote Health, Safety & Environment awareness. Hold each employee accounta ble for their individual responsibility for Health, Safety & Environment Every r easonable effort will be made to assure that employees can accomplish the safe c ompletion of assigned tasks. LBC Project HSE Plan, Ewekoro A 1 SAFETY MANAGEMENT Purpose The purpose of this document is to manage and cont rol potential exposures and risks associated with fatalities, injuries and incid ents arising from the work performed by PW on behalf of Lafarge. PW and its subcontractor employees shall work under the same HS & E expectations as those of L afarge employees. 2. Responsibility PW is responsible for: Developing and implem enting project specific HS& E systems that ensure compliance and adherence to th e requirements described in the contract document, including any modifications t hat may happen during contract execution as well as all the laws of the country. Ensuring that their employees and all sub-contractors are competent and adequat ely trained to perform the required tasks. Ensuring that their employees and the ir sub-contractors comply with the requirements of this document. Leading by exa mple and ensuring that they and their sub-contractors are applying safe work pra ctices at all times. Ensuring, before any work is started, that all personnel ar e adequately trained with respect to all the H & S aspects of the tasks they are to perform and are given appropriate site induction training. PW Site Manager m ust ensure that their employees understand Lafarges rules and Standards and relat ed Advisories and are fully familiar with the work site and the peculiarities of the work. This requirement also pertains to any employee beginning work on the project after the initial training and start up phase. P.W. shall also provide r efresher training in all related topics at a minimum of 6-months. PW is responsi ble to ensure their sub-contractors comply with Lafarge HSE requirements and are properly trained in all tasks they plan to employ on the site. SITE AND SAFETY ORGANISATION B.

LBC Project HSE Plan, Ewekoro PW shall designate a qualified and certified Safety Manager, Health & Safety Off icers, and will require its subcontractors to do the same. PW Health and Safety Manager shall be proficient in both oral and written communication in the contra ct language. PW Safety Manager will report directly to the Managing Director or his representative and will be the main interface with the Lafarge Safety Manage r / Officer regarding safety matters. He will also have a direct relationship wi th the top (Senior) management. PW Project Manager will be responsible for organ izing, managing, coordinating and controlling all of his employees and his sub-c ontractor employees safety related activities. The Safety Manager, or his designa ted deputy, shall be permanently on site, during all working hours. The site mus t not be left unattended at any time. PW shall provide both technical and safety supervision at site during all work activities. Workers must not be authorized or permitted to execute site works and activities without continuous and compete nt site supervision. PW personnel, empowered by the management to authorize any necessary work, shall always be available for any meetings at Lafarges site offic e when called upon by the Lafarge Site Manager or designee. PW Site Manager or h is designee shall always be permanently on site when work is in progress. PW wil l not be allowed to start or continue any work without competent and adequate su pervision of the work force. P.W. Organization chart structure (To be included): 2. Roles and Responsibilities

PW Project Manager: Is accountable for the Health and Safety of his e d his subcontractors employees. Provide leadership, guidance and support to the H & S team to ensure the H & S plan is implemented and followed. Define and appro ve the H & S organization and provide the required resources and support Evaluat e the H & S performance Ensure proper communication between all parties Provide support when necessary to address HSE gaps and deficiencies Address HSE Gaps and deficiencies and ensure they are corrected and addressed Attend and participate in the weekly safety walk-around & Safety meetings Review & manage corrective a ction plans Communicate lessons learned, SERs and Good Practices Stop and correct at risk unsafe behaviors / acts Page 6 of 55

Attend a nd participate in monthly H & S gate meeting Proactively promote Safety Visible Felt Leadership Ensure his work does not adversely impact the work of Lafarge or other Contractors working for Lafarge. PW Site Safety Manager Advice the Project Manager on evelop and implement the Contractors H & S management plan. Identify and immediat ely correct all Health and Safety deficiencies related to their scope of work. C omplete all related H & S induction training / re-training and orientation for w orkers Develop corrective action plans. Develop a site-specific Emergency Action Plan before the start of any work. Complete all their accident and near-miss in vestigations and provide reports in a timely manner. Record, correlate & update all H & S statistics and provide weekly and monthly H & S reports. Prepare and r eview H & S Risk Assessments, Method Statements and Permits to Work. Complete we ekly H & S briefings with their employees. Complete Toolbox talks on relevant to pics each morning before the work starts. Attend and participate in the weekly s afety walk around and safety meetings. Complete daily site inspections of all ac tive work. Develop, review and implement all corrective action plans in a timely manner. Communicate Lessons learned, SERs and Good Practices from Lafarge and fr om previous work as approved by Lafarge. Stop and correct at risk unsafe behavio rs and acts immediately. Train H & S team members to ensure the proper level of competence and to delegate roles and responsibilities to the Site management tea m. Develop H & S awareness campaigns and activities routinely for all their empl oyees. Prepare a Standard Operating Procedure (S.O.P) for communicating all inci dents to the Site manager and Lafarge. Prepare and update the required H & S sta tistics boards showing daily, weekly and monthly information as required by Lafa rge. 3. Supervision ratios It is a contractual obligation to have a ratio of: 4. 1 Complete Safety Officer or Supervisor per 40 workers, working on the site. 1 Technical supervisor (Civil, Mechanical / Electrical, Automation) per 40 workers engaged in technical activitie s on site. Minimum Competent Peron(s) Requirements: Page 7 of 55

LBC Project HSE Plan, Ewekoro PW shall provide and only employ clearly certified persons with dedicated compet ency, experienced managers, engineers, technical assistants, foremen and supervi sors and works with appropriate training based on the scope of work to be perfor med on the project, for (but not limited to) the following minimum list of criti cal activities: Excavator/Trenching/Shoring/Piling/Formworks/Rebar adders/Netting Confined Space Fall Protection (Working at Heights) Rigging Opera ting a Crane (All crane operators must have and approved license, accepted and a pproved in country of the project) Mobile Equipment Electrical installation or t rouble-shooting. Steel erectors. MEWP operators Welders Safety Supervisors PW shall ensure that his employees have the required credentials and experience to manage the scope of the works, following a reliable certification process whe re an independent certification body exists that meets Lafarges requirements or t hrough a Lafarge supplied certification process, if such bodies do not exist. PW shall provide the CVs and certifications for all L0, L1, L2 and L3 site manageme nt positions. (Site Manager / Deputy Site Manager, Heads of Department (HODs)s / S enior Engineers, Engineers & Supervisors. Lafarge reserves the right to carry ou t skills tests on site for any of PW L4 workers and can refuse access to the sit e should the employee fail these tests. Lafarge reserves the right to remove any body from site they believe is not acting in a safe and professional manner. 5. Management of Change (Safety Planning) When field safety changes are necessary, or unsafe conditions or actions occur, PW shall stop the related work, review all applicable procedures, and Lafarge wi ll require, at a minimum, the completion of a task specific risk assessment. If this assessment determines a change needs to be made, the PWs Safety written proc edures must also be changed and communicated to the site team. 6. PW Health and Safety Committee The Safety Manager together with representation from their entire subcontractors will form the Safety Committee. This committee shall be responsible for preparin g, implementing and enforcing all required site health and safety rules and requ irements. Page 8 of 55

LBC Project HSE Plan, Ewekoro Lafarge will be notified and invited to attend the safety committee meetings. A copy of the minutes will be provided to Lafarge from all meetings to confirm the Safety Committee is operating and functioning in accordance with their expectat ions. 7. Safety meeting PWs Project Manager and the Safety Manager shall attend a regular weekly safety m eeting with Lafarges site management team at an agreed time and day at least once a week while any work is in-progress. Official copies of all safety meetings ar e to be signed by both Lafarge and PW and retained in the project file. 8. Gate Meetings Lafarge and PW shall arrange and conduct a monthly Gate Meeting which s hall be attended by Lafarges project related employees and all PW and its sub con tractors employees on site when the meeting is schedule. C 1 SAFETY PLAN Safety Manual PW shall prepare a detailed safety manual, specific for the job or work to be un dertaken. Any person who refuses or fails to conform to these rules will not be allowed to work on the site or within PWs organization on site. 2 Induction and S afety training

Lafarge has the right to assess the capabilities and experience of PW in their l ocal environment and may take more active role in ensuring that it provides the necessary training to its employees. Safety Camp PW will establish an on-site sa fety camp with at minimum the following items: Safety communication not s H & S Statistics boards Emergency Procedures & Emergency Contact Lists Posters Safety banners Safety posters Lafarge and PW safety communications A mannequin wearing samples of PPE and other required safety equipment Samples of all PPE an d safety equipment to be used at the site First Aid Boxes in adequate number and proper locations based on number of workers at site Eye Wash stations and porta ble bottles Page 9 of 55

The safety camp will be equipped with facilities to enable effective communicati on as follows: Computer Desktop / Laptop DVD / CD / Video player Projector Scre n Speakers

Safety training PW shall be responsible for conducting all necessary training in cluding, WAH, PPE, Confined space, Mobile Equipment, Scaffolding erection and us e, safe excavation, LOTOTO, Safe electrical practices, proper use of lifting equ ipment and other related issues. Training will take the form of induction traini ng for each employee new on site, specific training and tool box meeting (on the job training) PWs Safety Manager shall provide and conduct all induction safety training. No access to the site is allowed without first attending and completin g the training Specifics of the plant industrial hazards Alarm, site ev personnel counting, resumption of work Site safety rules and procedures Local l aws, rules and regulations Contractual stipulations Near miss and accidents repo rting Fire protection Housekeeping Rewards and disciplinary action policy Other topics as required by Lafarge Site Manager The induction training must be given to PWs subcontractors, suppliers, employees and workers, as well as to visitors and any related third party, prior to visiti ng site. Attendance to this training is mandatory before going on-site and shall be documented in a special logbook with the name of the person, the date of the training and the signature of the trainee. PWs Safety Manager shall keep this li st. Lafarges Safety Manager shall be authorized to control it and to get copies a t any time during working hours. PW shall ensure that any other specific trainin g is provided if and when required by Lafarge. This may include, but is not limi ted to, erecting and dismantling scaffolds, works at height, work in confined sp aces, LOTOTO, excavations, etc. Page 10 of 55

LBC Project HSE Plan, Ewekoro 4 Enforcement of the Safety Management Plan Should PW fail to enforce any of the provisions of the Safety Management Plan, the Site Safety Manual or of the Safe ty Rules and Practices, then Lafarges Site Manager, upon recommendation of Lafarg es Safety Manager, shall ask PW in writing, (or verbally followed by written conf irmation in an emergency case), to take appropriate corrective actions. In such an emergency and if PW fails to act, the Lafarges Site Manager reserves the right to take those actions including the stoppage of any related tasks. All costs, r isks, delays and consequences directly related to PWs failure to comply with Lafa rge instructions will be borne by their management unless notice by Lafarge is g iven in writing. 5 Rewards and Sanctions: PW shall inform all employees that repeated violations of the laws, safety rules and regulations will result in disciplinary action, which can include immediate termination. Any fines, fees, etc., levied against Lafarge by any authority or third party due to violations of the safety requirements by PW or one of its sub contractors, suppliers or visitors must be reimbursed to Lafarge by PW. Lafarge may subtract such amounts from any installment to be paid to PW. The Lafarge saf ety Manager / Officer, in agreement with PW safety Manager, will set up an incen tive scheme and a fine system in order to control violations. Fine System Follow ing a safety violation by a worker, Lafarge will request to have the Id badge of a worker, the badge will then be punched from 1 3 times depending on the severi ty of the infringement. All workers will then be sent for re-induction to the sa fety training to the safety training camp following a safety violation / infring ement. When the worker accumulates 3 punches to his ID Badge he will be dismisse d and banned from site permanently. When a worker places himself or others in im mediate danger to life and health, his ID Badge will be removed and he will be i mmediately dismissed from site. Lafarge reserves the right to demand the dismiss al of any personnel who does not respect the site requirements. All costs, risks , delays and consequences directly related to PWs failures will be borne by PW. F or disciplinary reasons, Lafarge may, but not unreasonably, instruct PW to remov e any of its employees from the site . In such a case PW will replace the employ ee in the shortest possible time and will not claim for compensation related to this action. Page 11 of 55

LBC Project HSE Plan, Ewekoro Stop Work order (SWO) Following a very serious incident or event at the site, La farge site manager has the right to issue a stop work order / prohibition notice to PW to cease part or all site work activities and close the site. PW must com ply with and adhere to Lafarges instruction when issued. 6 Employee identificatio n Badges

All personnel working on the site shall have an Identification Badge that will b e issued following the successful completion of the safety induction training. T he ID badge will contain employee details; First Name Family Name Date of Bir Employee Work title Date issued Blood Group A recent picture of the employee will be included on the badge The badge will be laminated to ensure it is waterproof. 7 Monitoring, Inspections, Audits & Contr ols PW shall implement an inspection and audit system to measure the implementation of its health & safety programs. Daily/Weekly/Monthly Safety Inspections and Aud its PW shall be responsible for ensuring that daily, weekly and monthly inspecti ons are completed, documented and provided to Lafarge within 1 day of the inspec tion. PW shall also be responsible for addressing and resolving all defects, pro blems and issues highlighted in the inspections in a timely manner based on the risks and priorities determined. Lafarge will be invited to attend any and all i nspections by PW. Compliance PW shall meet the requirements of the relevant laws and regulations and all the requirements of the contract document. The intent o f these instructions is for PW to take a direct role in the monitoring and conti nuous improvement of its own H & S performance. PW shall ensure that its employe es continuously apply safe work practices while working for Lafarge. Page 12 of 55

LBC Project HSE Plan, Ewekoro Daily Inspections PW Site manager and the Site Safety manager will conduct a dai ly inspection of the critical areas and equipment on the site and document their findings. PW shall also conduct a weekly detailed site safety inspection and re view the site safety conditions with the Lafarge site management team, the inspe ction will be attended by: PW Site Manager ( or by his designate deputy when abs ent from site) PW Safety Manager / designated deputy when absent from site PW te chnical Engineers (Civil, Mechanical & Electrical) Lafarge Site Manager / design ated deputy when absent from site Lafarge Safety Manager / designated deputy whe n absent from site Lafarge technical Engineers (Civil, mechanical & Electrical) A corrective action plan will be defined by PW following the site inspection tha t includes the following information: Item No: Location: Description of p Details of the corrective actions: Risk Level: Time to correct Responsible pers on Date completed: Additional comments A corrective action plan will be issued and communicated to all parties concerne d within 1 working day following the site inspection. Lafarge HSE Audits Lafarge will conduct its own regular H & S inspections and au dit processes to evaluate the contractors performance at their discretion. When P W or its personnel are invited to these inspections, it is mandatory for them to attend. Lafarge reserves the right to audit the safety of the site at any time, without prior notice, and potentially with the support of external resources. Page 13 of 55

LBC Project HSE Plan, Ewekoro These inspections will encompass all construction operations, office, shops, war ehouses, PW living areas and any other related facility including any and all ad ministrative documents. Request for documents and information Lafarges Safety Manager is also entitled to request any documents required to certify the conformity of the equipment, tool s and materials used by PW. PW shall also comply with this request ASAP. Followup of action plans PW shall be required to provide regular updates in relation t o the safety corrective action plans developed, within the agreed time durations . If the corrective actions are not completed in a timely manner, Lafarge has th e right to engage any and all means to make and implement the corrective actions and deduct the associated costs from PWs invoice. 8 Reporting

Compliance by PW with the Lafarge Group Governance Standard for Reporting & Inve stigation is mandatory at all sites. HSE Statistics Lafarge will set up a report ing system to measure PWs Safety performance. Each accident or near miss accident must be reported in accordance with the Lafarge Governance Standard for Reporti ng and Investigation. Details and information will be recorded for the following Fatalities Lost time injuries Medical injuries First Aid in occurrences: Near miss Serious Near Miss Unsafe Act Unsafe condition Vehicle Accidents (FI) ( LTI) (MI) (FA) (NM) (SNM) (UA) (UC) (VA) Page 14 of 55

LBC Project HSE Plan, Ewekoro Property Damage (PD) Total man-Hours worked in the month; Average number of e oyees: Lost Time Injury Frequency Rate: (LTIF) Total Injury Frequency Rate: (TIF R) Severity Rate: (SR) Lost Work Days: (LWD) Weekly / Monthly HSE Statistics Reports Man-hours and statistical information sh all be calculated weekly and monthly. PW shall provide Lafarge with weekly HSE s tatistics and information every Monday or on the next working day following a ho liday. Monthly HSE statistics will be provided within 2 working days of the end of each month, together with the monthly summary of all LTIs, Fatalities, Lost Ti me incidents, First Aid Treatments, near misses, Unsafe acts, Unsafe conditions and other requirements described above... Site statistics boards PW shall provid e, at least 2 HSE statistics boards of adequate size, at its project office and its project site entrance. PW shall update the site HSE statistics board daily, weekly and monthly. The statistics board will record the following information: Fatal injuries (FI) Lost time incidents (LTI) Lost work days (LWD) Total man-hou rs worked Man-hours worked since last LTI Total project man-hours worked to-date Number of days since the start of the project Incident and Root Cause Analysis Reports Fatal accidents, major injuries, deteri oration in health condition or damages to Lafarge or third party equipment and p roperty must be reported immediately, in writing, to Lafarges Site Manager. Any o ther accident must also be reported on the day it happened to Lafarges Site Manag er that includes a written report issued to Lafarges Site Manager within one work ing day of the accident. LBC Project HSE Plan, Ewekoro PW shall utilize the Lafarge forms and templates unless PWs forms and templates a re approved by Lafarge, in writing, before use. Incident and root cause analysis (RCA) reports shall be completed for: Fatalities Lost Time incidence Serious Ne ar Misses, which could have resulted in an FI or LTI PW shall perform a (RCA) Root Cause Analysis to understand the real causes of th e accident and to take appropriate measures. Lafarges Site Manager / Officer will train PW, if required, for such analysis. All things must be provided n writing to the Lafarge Site Manager. Accident Report Book All medical injuries and refe rrals to hospitals or clinics, either on or off site, should be reported to Lafa rge on a daily basis. First aid treatments must also be reported , but on a week ly basis in writing. First Aid Report Book A first aid book must be maintained b y the site medical personnel, doctor, nurse or any first aid providers. PW shall maintain a logbook of all injuries and keep it available for consultation by La farges Site Manager. Light duties In the event of an injury with work restriction s, PW is responsible for providing light duty work in conjunction and compliance with the doctors recommendations. All light duty will be tracked and reported on by PW to Lafarge on a weekly basis. PW shall identify in their Safety Manual ho w they plan to comply with this requirement. The plan must include a detailed li sting of light duty jobs that will be made available at the project site. 9 Hous ekeeping Housekeeping is a critical component of site safety. Housekeeping will be strictly enforced by PW and Lafarge. Note: A clean and organized site is norm ally a safe site. PW shall be fully responsible to keep the Site and the Works f ree from waste materials or rubbish resulting from the Works or caused by PWs emp loyees, subcontractors or suppliers. PW also shall keep the site free from stand ing water and take all precautions to ensure safe operations during bad weather conditions such as heavy rains and freezing conditions.

LBC Project HSE Plan, Ewekoro Ground and surface run-off shall be controlled and channeled onto drains and dit ched, designed and provided by PW, Lafarge maintains the right to require PW to modify their design if the system is deemed inadequate. Pedestrian walkways, acc ess routes (entrances and exits) working platforms and working areas must be kep t clean and clear at all times in order to maintain safe working conditions. Wor k areas shall be maintained in a clean, neat and orderly manner to minimize any potential trip and fall hazards. The site must be clean daily by PW and the wast e disposed of in appropriate containers and locations. The containers will be em ptied as soon as they are full. Scrap materials must be removed from all areas, floors on a daily basis. Worn out defective or non-functioning equipment shall b e removed from the site and properly discarded. Fire extinguishers, fire hydrant s, fire alarms and electrical panels must always be kept clear of any obstructio ns. Any spillage of hazardous material must be immediately cleaned-up and report ed to Lafarge. PW must dedicate permanent project resources, including personnel , tools and equipment, to properly manage all site housekeeping. The locations o f storage areas identified for waste and scrap materials must be approved by Laf arge. All construction and scrap materials must be stacked and stored safely in an organized manner throughout the project site and in the Lafarge approved mate rial staging and storage areas. PW is responsible for completing daily and weekl y housekeeping activities to maintain the site in a clean and safe condition. Al l waste materials and garbage must be stored in appropriate places before being disposed of at licensed and approved dump sites. Bins / containers shall be prov ided and labeled according to the types of waste as follows: Bio-degradab d waste Plastics, bottles, containers, cups etc. Paper, cardboard Wood, scrap ca sings, formwork materials Steel, scrap rebars and steel, scaffolding tubes Oil S olvents Other items as required by Lafarge The following types of industrial waste also be properly managed: Asbestos (Asbe stos sheeting / insulating materials exist in old plants Chromium bricks Waste o il Other items as required by Lafarge The following scrap materials must be cont rolled, stored and disposed of on a regular basis: Wood / form work and false wo rk materials Scrap Rebar Page 17 of 55

LBC Project HSE Plan, Ewekoro Damaged scaffolding materials Demolished equipment and concrete debris Protruding nails will not be left on planks and must be hammered down or removed as soon as the planks are dismantled. PW shall be responsible for removing any graffiti promptly. PW will not throw or drop material from upper levels to lower levels or the ground. Water spray trucks must be provided to ensure that the si te roads are constantly dampened to control potential levels. No burning of any rubbish or material is permitted on the site without written permission from Laf arge. All excavated materials, waste, rubbish, etc., must be disposed according to local laws and regulations. Within one month after the Provisional Acceptance of job completion, PW shall dismantle its entire temporary facilities and utili ties, including all temporary concrete slabs, and perform a final site cleaning and rough leveling. All areas used by PW, its subcontractors, and suppliers shal l be left free of rubbish , timber, scrap metal, equipment, supplies, materials or any other construction related debris. In the case of non-compliance, Lafarge will have the site cleaned at PWs expense. When the housekeeping has not been co mpleted to the required satisfactory standard or is falling behind, Lafarge rese rves the right to stop the work activities in the area until the housekeeping is sues have been resolved. If more than one party is involved, they must agree and clean the site together at their own expense. In the case of non-compliance, La farge is entitled to have the site cleaned at PWs expense. The distribution of co sts will be decided by Lafarge and the relevant amounts deducted from PWs next in voice. D SAFETY PROCEDURES 1 1.1 Site practices and conditions Site Guarding / Security PW shall provide a full time security guard service for its work site and all it s facilities including, but not limited to, offices, tools, materials, storage a reas, vehicles and equipment. All personnel and vehicles access to the site will be through controlled gates. PW shall provide a full time security watch and se rvice at these selected gates. Page 18 of 55

LBC Project HSE Plan, Ewekoro PW shall be responsible for receiving and registering all the incoming and outgo ing equipment, materials and tools related to its work. PW shall make the necess ary arrangements to allow quick and effective access to any emergency and firefi ghting crews and vehicles. 1.2 Fencing Before starting any permanent work, PW will install a temporary fence around its work site according to the agreed upon site installation layout. The fence must be erected within 1 month of PW mobilizing to site. The fencing material must b e approved by Lafarge and noted on the site installation layout. The fence will define the boundaries in which the site safety rules are enforced. All construct ion areas and offices must be segregated with separate fencing. 1.3 Site access & circulation plan Permanent access: PW and Lafarge vehicles will be provided with a permanent site entry permit. Temporary access: Visitors will be provided color-coded badges th at must be displayed at all times in their vehicles. Circulation plan All person nel entering the site, including drivers, must follow all site safety rules incl uding the wearing of required PPE. PW shall provide all required PPE at the gate s to ensure the implementation of this rule. PW is responsible for building and maintaining all necessary site roads and pedestrian walkways as described on the site circulation plan. PW shall ensure that the vehicles leaving the site do no t pollute the plant and public roads system. An installation, if necessary of an appropriate wheels washing station at allocation approved by Lafarge, and will take care of any waste according to existing regulations. All vehicles entering or existing the site may be subject to a search by Lafarge. The parking location s and areas must be defined and marked by PW in accordance with the site install ation layout and must include the following items: Bicycles Motor cycles Buses ars Trucks Mobile Equipment Page 19 of 55

LBC Project HSE Plan, Ewekoro 1.4 Driving Policy and Pedestrian Protection Seat belts must be worn at all tim by any driver or operator and any passengers of any vehicle or mobile equipment . It is forbidden for the driver of a vehicle or any mobile equipment to use a m obile phone while in motion. A maximum speed limit of 20km/h will be enforced fo r all vehicles on the site. Adequate traffic signs, speed bumps and other simila r requirements will be provided PW to ensure a safe traffic management system. P edestrians and mobile traffic shall be segregated by defined and protected walkw ays. All traffic for the operating plants must be segregated from project. 1.5 Site Lighting PW will install temporary lighting for all roads, pathways and working areas. The lighting levels must comply with all country and local requi rements and meet or exceed the following illuminating levels. Access and evacuat ion roads Walkways, general, platforms, accesses, stairs at all time and permane ntly 20 Lux 20 Lux External work areas, including concreting work, brickwork, rebars works and pipi ng 50 Lux Inside Works, compressor rooms and electrical substations 1.6 Health a nd Hygiene 200 Lux PW shall provide portable drinking water, sanitary toilet and hand washing facil ities inside the work place. PW will test the proposed supply for contaminates a nd provide Lafarge with the results before using the water on the site. PW is re sponsible for the cleanliness of all areas under its control PW shall provide ap propriate rest areas for consuming food and taking breaks outside the work areas and ensure that provisions are made for hand washing and will include a portabl e water supply. 1.6.1 Health Procedure PW NIGERIA LTD Health policy is to promote the overall health of the entire work force and to ensure that the companys activities do not adversely affect any thir d parties. Objectives To ensure that all persons engaged in our operations are p hysically and emotionally fit for the jobs they are engaged for. To provide medi cal facilities capable of responding to and arresting emergency situations. To m aintain retainer clinics and ensure that they offer the highest possible medical attention to employees in need. To ensure that all employees have annual medica l examination. Page 20 of 55

First Aid Facilities First aid boxes shall be adequately provided at site. These shall be equipped with relevant drugs and facilities. The first aid posts shall be manned by qualified Nurse. The first a id facilities shall be used for administering initial treatment to the patient/i njured. Any patient requiring further treatment shall be referred to the site cl inic. Site Clinic The site clinic located within the site shall be fully equipped with essential drugs. A qualified Doctor shall be in charge of the site clinic. Drug s shall be requested when they are below reorder level. Any illness/accident tha t could not be handled by the site clinic shall be referred to the company retai ner clinic. Medical cases that could not be handled by the site clinic shall be referred to the retainer clinic. Arrangement shall also be made with the retain er clinic to train a considerable number of the workforce in first aid Managemen t. Drug/Alcohol It shall be a termination offence for any employee of PW NIGERIA LT D to be under the influence of alcohol or in possession of or under the influenc e of any non-prescription drug, illegal or controlled substance while working on the project. This rule shall apply on a 24-hours basis. Supervisors shall be em powered to conduct periodic checks to ensure compliance. Smoking Smoking shall b e strictly prohibited in every part of a Restricted Area. Medical Evacuation Procedure Page 21 of 55

The casualty s hall be given first aid attention at the site clinic and later referred to the r etainer clinic for proper medical attention. Depending on the severity of the in jury, the injured person will be sent to the retainer hospital: SOREMEKUN MEMORIAL HOPITAL LTD. No.127, Lagos Motor Road, Vesper Bus-Stop, Ifo-O gun State. Doctor I/C: Dr. Ibikunle Soremekun. 1.7 Temporary Site Facilities & Camp

Site Facilities: PW, at its own expense, shall arrange for the supply, to all it s employees, subcontractors and suppliers of at least the minimum following faci lities; Temporary and fully equipped offices, canteen, first aid stations and dical facilities. Temporary covered rest areas easily accessible by the workers. Temporary sanitary toilet units properly distributes on site and show on the si te plan. PW shall arrange for regular cleaning and service at its cost. Sanitary washing facilities on site for the workers. Temporary drainage, sewer, setting basins and sanitary pits, approved by Lafarge prior to construction. Temporary p ower generation and distribution system, including fuel storage with retainage c apability. All distribution panels must be able to be de-energized and locked-ou t without opening the panel door. PW shall be required to submit a plan of its t emporary power distribution with sufficient detail to allow Lafarge to review an d approve the plan it is implemented. Any changes to the system must be followed -up with a revision by PW and pre-approved by Lafarge. The camp and facilities shall be constructed following safe standards and practi ces as required by the ILO (International Labour Organization) the country and l ocal codes and Lafarge. Lafarge has the right to audit and inspect PW housing an d camp areas at any time, to ensure that all issues are being properly managed. 1.8 Handling and warehousing PW shall prepare the equipment storage and lay-down areas as follows: Providing Lafarge with a detailed lay-down plan that anticipates app phases of the project . Backfilling, leveling, compacting and stabilizing all storage areas. Storing e quipment properly on level prepared ground. Provide lighting throughout the stor age areas (See Lux requirements above) Provide security posts in the storage are as Provide temporary drainage to ensure the areas remain accessible at all times Clearly mark and define vehicle access roads, storage areas, pedestrian walkway s Materials must be neatly stacked in a safe manner Supports, wood or other mate rials will be placed under all equipment. No equipment can be set directly on th e ground without proper drainage or support to ensure the equipment is not expos ed to mud and other related impacts. Dewatering pumps will be provided to ensure the areas do not become submerged or flooded should the drainage become blocked . Special precautions must be taken when loading and unloading materials with re spect to crane safety, ground condition, reach or angle of crane boom, boom exte nsion, load centre of gravity, load spreading of crane feet / tracks. All lifts must be managed by a competent lifting and rigging person, prequalified by PW an d approved by Lafarge. Underage Motor Boys are not allowed on the site at any time . PW must supply a plan to Lafarge as to how they will manage this issue before any material is received on-site. 1.9 Ammunitions, Drugs, Alcohol, Weapons, Expl osives Page 23 of 55

LBC Project HSE Plan, Ewekoro Any ammunition, explosives or weapons of any type are not authorized on site. An y ammunition, explosive, weapons found on the site shall be immediately reported to the Lafarge Site Manager. In the case where ammunition or explosives are fou nd, the area or areas affected shall be cordoned off to prevent access by any pe rson and all personnel shall leave the areas immediately. The consumption, posse ssion or sales of alcoholic beverages, drugs or any banned or illegal substances is prohibited on the site. Any person under the influence of alcohol or banned substances or the carrying of weapons of any kind in, on or around the Site is s trictly forbidden. The abuse of alcohol and controlled substances on the project site is interpreted as a serious safety threat to all personnel involved. The D rug and Alcohol Policy applies to all disciplines and all levels of the workforc e. PW and its subcontractors will be required to proactively participate in the programme on the Lafarge project. The following is the policy that will be enfor ced on the project site regarding the abuse of alcohol and controlled substances : Possession, use, distribution or sale of alcoholic beverages, narcotics and no n-prescribed controlled substance is prohibited from the project site Persons un der the influence of any of the above substances are not allowed to enter the si te and will be asked to leave the project site if found to be in violation of th e policy. Individuals may be tested when involved in an accident or anytime as r equested by Lafarge. Violation of this Policy is a condition for dismissal and p ossible prosecution under country and local laws. Lafarge expects PW to enforce an alcohol and drug free work place. The Site security shall have the right at a ny time to search any vehicles entering the site and refuse admission to anyone found in possession of the aforementioned items or if any person is found to be under the influence of alcohol or drugs. Any person found to be under the influe nce or in possession of alcohol or drugs, or in possession of weapons while on t he site, shall be dismissed from the site immediately. The cost of replacing any offending person or persons under the control of the PW shall be covered by its management. 1.10 Working hours PW shall follow all country and local laws for d ay, night, weekend and holiday work. A full day off per week is mandatory for al l workers, regardless of the country or local requirements. Lafarge personnel wearing light red badges are not allowed to give comments, recommendations, instructions or orders to PW unless it conce rns a dangerous situation where people are in immediate or imminent danger to li fe and health which must be addressed without delay. 1.12 Personnel Transport PW shall provide proper safety means of transport for his personnel equipped wit h individual seat belts. No personnel with the transported on open pick-up truck s, flat bed trucks, high walled trucks or similar improper modes of transport. P W shall follow the Lafarge Mobile Equipment Advisory attached to this document. All employees must be dropped off at the site entrance or in the specified, dedi cated parking area to ensure they are properly screened prior to entering the si te. 1.13 Employee Safety Handbook PW shall provide his workers with an employee safety handbook explaining the hea lth and safety rules and requirements of the work site. PW shall submit a draft for approval by Lafarge prior to site mobilization. 1.14 2. As a principle, a Permit to Work will be associated to each specific and clearly identified highrisk task. A Permit to Work will be a written and formal documen t, which authorizes certain people to carry out a specific work at a certain tim e and place, and defines the precautions to be taken before allowing work to sta r. WAH Permits to Work must follow the required Lafarge format detailed in the W AH Standard attached to this document. As a consequence, any Permit to Work will have to include a related method statement and a risk assessment. No PTW will b e validated without them. Submission to the Lafarges representative of task requi ring Permits to Work are required one week ahead of the schedule time to executi on. Failure to comply with this requirement can delay the work and all impacts a re at the PWs expenses. The Permit to Work will be issued by PW and will need to be countersigned by the Lafarge representative, for the task to be authorized. P Ws supervisor responsible for the execution of the task, must check that the work is implemented in compliance with the conditions of the Permit to Work and that these conditions have not changed. Any changes must be captured on a new Permit to Work, approved by Lafarge, and communicated all personnel involved with the task before work is allowed to continue. Lafarge have the right and will routine ly check the Permit to Work. If conditions detailed in the permit are not being followed, Lafarge has the right to stop the related task. A copy of the permit/p rocedure, risk assessment and method statement must always be posted in a weathe rproof folder (provide by PW) at location of the work in order to allow Lafarge and PWs team to check it. Duration of a Permit to work: The maximum duration of a permit-to-work is one week unless authorized by Lafarge in writing. Page 26 of 55

LBC Project HSE Plan, Ewekoro At minimum, a carbonized triplicate system (3 copies of the permits to work) or similar Lafarge approved system should be utilized. 2.2 Method Statement A method statement details the way a work task or process is to be completed. Th e method statement should also outline the hazards involved, and include a step by step guide as to how to do the job safety, leading to the risk assessment pro cess for each task. The method statement must also detail which control measures will need to be introduced to ensure the safety of anyone who is affected by th e task or process. PW must produce method statements for all major construction activities. These method statements are mandatory for any works that have a high risk rating and for all activities presenting a potential risk for safety of pe rsonnel as listed in Chapter 2.1. Lafarge reserves the right to request a method statement for any task they believe requires this level of review. The method s tatement shall be reviewed and approved by all concerned parties within the PWs o rganization and signed by its Safety Manager. It will then be submitted to the L afarge Site Manager for comments and approval. Related work must not be started before the method statement is approved by Lafarge. PW is also responsible to check that actual operations follow the method stateme nt and to inform Lafarge in case of any changes. When there is a need for a devi ation from the agreed to method, the work must be stopped and PW and Lafarge mus t meet to review and approve the changes. Afterwards PW may recommence the work provided all safety requirements and associated documentation is in place. Once approved, the method statement must be strictly implemented and followed; any de viation must be discussed and approved by Lafarge before any changes can be made . During the course of the work, if further activities arise and are identified as having HSE risks, method statements must also be developed for these activiti es before the work continues. Safety work method statements must be attached to or incorporated into the associated site Health and Safety plan and be kept up t o date for review by Lafarge at any time. All parties involved in the work must also be notified of any changes before the change can be implemented. The actual format will be provided by PW but must contain the following elements , at a minimum: Page 27 of 55

Arrangements for changing and or deviati ng from the method statement. The potential hazards and risk associated with the work activities. All precautions taken to protect health and safety Identification of applicable sections of HSE legislation, regulations, codes, standards and procedures appli cable to the work. The names and qualifications of those who will supervise the work, Inspect and approve the work area conditions, work methods, protective mea sures, plant equipment and tools. A description of what training is required for personnel involved with the work The names of those who have been or will be tr ained for the work activities described in the safety work method statement and the names and qualifications of those responsible for training them. Identificat ion of the resources, both plant and equipment, that is mostly likely to be used on the work site, such as ladders, scaffolds, grinders, electrical leads, weldi ng machines, fire extinguishers, tools and materials, including safety precautio ns and any restrictions. Details of any work permits required to complete the wo rk and where they will be posted. Details of the inspection and maintenance chec ks which will be needed for any equipment listed. All requirements for Personal Protective Equipment and confined space testing. Safety checks and related clear ances at specific stages. Final clearance that they activity is completed to the written requirements. 2.3 Risk Assessment PW shall complete risk assessments for tasks presenting a potential risk for the safety of personnel as listed in chapter 2.1 and any other task as requested by Lafarge. All risk assessments must be reviewed with PWs personnel prior to under taking the specific task. Risk Assessments must be carried out to satisfy the re quirements of legislation but above all to ensure the Health & Safety of all per sonnel. Risk assessments must be carried out by a person w ho is experienced, trained and competent to do so. Competence can be expressed a s a combination of Knowledge, Awareness, training and experience and the person must have the authority to make the necessary changes or to stop the work if a v ariance is discovered. Expert assistance should be consulted when the required e xpertise is not available on the site. Risk Assessments must be reviewed routine ly, depending on risk, or if something changes (new workers are involved, a chan ge in the process, etc). PW shall prov ide personal protective equipment for all personnel (including visitors) in acco rdance with the project-Specific Health & Safety Plan. PW shall post signs throu ghout the site identifying the PPE requirements for each area / and zone. Clothing Requirements: Long Pants and shirts covering the shoulder with a minimu m 10 cm sleeve. Hearing Conservation: Personal hearing protection shall be worn by ALL PW employees, visitors, and vendors when the noise exposure exceeds 80 de cibels over an eight hour weighted average (TWA). In no case shall the personnel be exposed to more than 105 dB (A). PW shall post sign warnings of any potentia l high noise levels due to their work activities. PW shall document in their Pro ject-specific Health & Safety Plan all anticipated high noise levels and areas t hat will be generated due to their work activities. All safety personnel must be easily visible and distinguishable from other personnel by using a different co lored helmet, high visibility vest etc. All inexperienced new hires, with less t han 1 years site work experience, must also be easily visible and distinguishable by wearing a different colored helmet in order to help monitor and supervise th em while they are working on site. Respiratory Protection: At a minimum, PW shal l ensure their policy on respiratory protection addresses all regulatory require ments at the Lafarge Standard for PPE. 3.2 Working at Height (WAH) Page 32 of 55

LBC Project HSE Plan, Ewekoro Compliance with the Lafarge Group H & S Standard Working at Height (WAH) is mand atory at all sites. Risks related with WAH must be systematically analyzed durin g review of the method statements and risk assessments performed for the related WAH construction jobs. Special attention must be paid to the following conditio ns: Edge protection: PW shall install the final edge protection (handrails, guar drails and toe-boards etc) of permanent platform, roofs, walkways, stairs and ac cesses before any related work is allowed to start. For non-permanent platforms, edge protection must also be provided using scaffolding tubes or other suitable methods capable of resisting 100 kg of lateral force. Floor openings and holes: Holes are not allowed on floors, platforms, roofs or any walkway accesses. Hole s must be protected either by edge protection (see above) or covered and secured by steel plate of adequate strength and reinforcement and welded or bolted into position. A systematic and all-encompassing review of edge and hole protection systems must be performed weekly and detailed, in writing, and submitted to the Lafarge site team. Working on structural steel buildings and structures Work at Height permits must be obtained for all structural steel erection works unless t he areas are fully isolated with handrail, proper access provided and all holes or potential fall risks properly mitigated. Access to the work areas shall be by fixed ladder, scaffolding, Mobile Equipment Work Platform or by a properly desi gned and tested man-basket operated by a competent crane operator trained in the lifting of personnel. Safety nets shall be properly designed and utilized where possible. Beam walking is strictly forbidden except with a properly designed li feline, self retracting lanyards or similar device as approved by Lafarge. Ladde r/Stairways: PWs Competent Person shall inspect all ladders weekly. The use of la dders with broken or missing rungs or steps, broken or split rails, or other fau lty or defective construction is prohibited. When ladders with such defects are discovered, they shall be removed from services and destroyed. Job-made wooden l adders are not allowed on any Lafarge project. Stepladders shall only be used in an open (full extended) position The top cap and top step of the stepladder sha ll not be used as a step Ladders shall be placed on a substantial base of a 4-1 pitch, have clear access at top and bottom, extend a minimum of 1 meter above th e landing and be secured against movement while in use. LBC Project HSE Plan, Ewekoro Ladders used in any location where they could be displaced by traffic shall be s ecured to prevent displacement and shall be barricaded. The area around the top and bottom of a ladder shall be kept clear of any debris or material. When ascen ding or descending a ladder, all personnel must face the ladder and maintain a m inimum of three points of contact. An employee shall not carry an object that co ld cause the employee to lose balance and fall. Hoists shall be provided next to ladders where employees must carry materials between levels. When working from ladders, fall protection equipment shall be utilized if working near the buildin g perimeter or around open shafts or other related hazards. Ladders, stairs, or ramps will be provided where a change in elevation is equal to 19 or greater. Lad ders are required when ascending on or descending from truck beds or trailers. T he 3-point-contract rule must be observed and utilized when working on ladders o r travelling up or down stairs. If working off ladders, the workers must use a t ool belt. Hoists or similar devices should be used to raise toolboxes and tools to working platforms to ensure the 3-point-contract rule is followed. 3.3 Energy Isolation (LOTOTO) Compliance with the Lafarge Group H & S Advisory LOTOTO is mandatory at all site s. Only qualified persons shall be permitted to work on or near energized or iso lated electrical systems or components. PW, when working on or around energized electrical equipment and related electrical hazards, will be properly trained an d must complete a risk assessment. PW, when performing electrical work, must be qualified accordingly to all applicable county laws and regulations to perform a ny electrical work. Electrical equipment supplied by PW shall provide full emplo yee protection and shall minimize exposures to any live electrical components. A ll electrical equipment installation, repair, and maintenance shall follow the a ppropriate IEC standards as provided by Lafarge. PW shall use the required PPE w hen working on electrical equipment. 3.4 Confined space entry (CSE) Compliance with the Lafarge Group Advisory for CSE is mandatory at all sites. Page 34 of 55

PW shall identify any and such risks and clearly identify them wit h proper safe distances clearly posted. For any work required near these utiliti es, PW shall consult Lafarge and submit alternative action plans if requested to do so. Whenever PW performs excavation work, it is their responsibility to cont act the appropriate local locating services and comply with all country and loca l requirements. A Cat Scanner will be provided and utilized by PW to identify any underground services. The required actions must be taken to ensure the undergrou nd services are isolated before excavation commences. A task specific risk asses sment shall be completed for all works activities that my impact the following u tilities. Electric Power Cables Plant or external utility lines (Steam, Fuel, Ga s, Water pipes, Sewage, Lubrication, Hydraulics etc.) Other lines (Communication cables, Telephone lines, Fire Alarm Systems, Control Systems, IT Systems) Inlan d transport and site 4.2 PW shall use every reasonable means to prevent any of the access ways or bridges to the Site and/or Quarry from being damaged by any traffic from PW, his subcon tractors or his suppliers. PW shall select appropriate routes and appropriate ve hicles. He shall plan his logistics to minimize the moves to and from the Site a nd/or the Quarry in order to avoid any damage to roads, pathways, bridges or bui ldings. PW shall, at his cost, obtain all necessary permits from the proper coun try, state and/or local authorities as required for the purpose of transporting the Supplies and any Materials and other equipment to the Site and/or Quarry. La farge, at PWs request shall reasonably assist in its developing procedures. PW sh all be responsible for all site warehousing and handling systems. Packaging mate rials must not be left on site and shall be removed routinely at its expense. St orage areas shall be organized in such a way as to facilitate the safe retrieval of equipment, parts, tools, fixtures, accessories and materials. Trucks should be selected based on the suitability to complete the required tasks. At any time , trucks must not be overloaded. Page 36 of 55

Proper lifti ng and rigging of equipment must be utilized when offloading the trucks or other transport equipment in line with typical safe practices with respect to cranes, slings, shackles, taglines, signalling, riggers, crane lifting capacities, pick s and reach calculations. E 1 EQUIPMENT Mobile equipment Compliance with the Lafarge Group Advisory for mobile equipment is mandatory at all sites.

All equipment brought on the site must meet all applicable laws, standards and r egulations of the country the project is executed in. All mobile equipment must be inspected and approved by Lafarge prior to use on site. The associated docume ntation should be handled over to Lafarge for review and assessment and equipmen t found to be defective in way, will not be permitted to enter the site. All veh icles or equipment must be fit for use and the following points must be checked: o Clean and undamaged mirrors. o All lights must function, including high & low beam headlights, turn-indicator lights and brake lights. o Functioning and audi ble reverse sirens, back-up alarms, movement alarm. This includes PW equipment a nd vendor trucks with obstructed vision in the rear. Equipment not having an ope rate-able back up alarm will be removed from service or the site. o Earth moving or other equipment designated by Lafarge, must be equipped with rollover safety cages. o Tires with adequate thread depth and suitable for the condition they w ill be used in. o If applicable, roadworthiness inspection certificates in line with all country and local laws and practices. o Functioning and audible horn. o Clean and undamaged windows. o All equipment must be free from leaks, (fuel, hy draulic oil, lubrication oil, break oil or coolant). Where required by Lafarge, the operator must be certified and trained, holding a valid license and must be medically fit. The operator must not use or answer a mobile phone while any equipment or vehicle is in operation. Banks-men and Signa l-men must be provided for guiding mobile equipment as required by the job or La farge. Page 37 of 55

LBC Project HSE Plan, Ewekoro 2 Any mobile equipment must be loaded and unloaded from any transporter truck foll owing safe work practices and procedures under full supervision by PW and equipp ed with proper ramps. All mobile equipment operators shall follow al posted spee d limits, road traffic signs and safety instructions. Passengers are forbidden f rom riding in the bed of pick-up trucks. This rule also applies to personal vehi cles entering or exiting the plant property. Seat belts shall be worn at all tim es when operating any mobile equipment. Employees shall not ride in cabs of equi pment unless there are 2 seats and the equipment is designed for that purpose an d has 2 seat-belts. Personnel shall not be hoisted by forklifts at any time. Compressed Gas Cylinders Gas Cylinders can only be transported on site using the following methods;

Cylinder Trolleys Cylinder cages / Lifting baskets Note : The design of these methods must be approved by Lafarge. Cylinder Storage All gas Cylinders must be tied and secured at all times. The following requirem ents must also be followed for gas Cylinder Storage. 3 They must be stored more than 10 meter of any open flame. They must be stored aw ay from any heat source warmer than 50C. They must be stored vertically or in an approved basket/carrier when in use. They must not be stored near an electrical source nor where they could come in contact with exposed electrical wiring. All empty cylinders shall be stored separately from full cylinders with their valve s close and shall be segregated by bottle type. Any oxygen and Acetylene gas cyl inders should be separated by a distance of 5 meters when stored in bulk stores. Acetylene cylinders must be secured before they are transported or moved. Cylin ders must be pulled by the cylinder valve. Gas Cutting / Welding Personnel performing welding and cutting tasks shall qualified and trained regar ding the hazards associated with compressed gas cylinders and the appropriate em ergency response procedures. In specific cases, Lafarge may require a specific p ermit to work in potentially hazardous areas. Proper PPE shall be worn at all ti mes, including the following items:

LBC Project HSE Plan, Ewekoro Welding apron Other items requested by Lafarge for specific tasks Screens shall be provided by PW and placed around any welding or cutting areas a nd the following requirements 4 Systems must conform with all relevant country and local regulations. The gas re gulators must be in proper working condition, with functioning inlet and outlet pressure gauges. The regulators should be tightened using properly sized cylinde r spanners. Adjustable spanners or pliers are not to be used on these systems. G as hoses (pipes) must be of suitable designed materials, fit for the purpose. Wh en gas hoses are worn, cracked or damaged, they must be replaced. All hoses must be inspected regularly prior to each use. Flash back arresters must be fitted a t both the gas cylinder regulators for each gas and at inlet to the cutting torc h for both gases. The gas hoses must be secured with proper fasteners, secured a nd tightened with the locking screws and soap tested for leaks. Plastic soap sol ution bottles should be provided at each system for detecting regulator or conne ction point gas leaks. Any leaks must be corrected before the system can be used . Connecting multiple systems to single tanks is prohibited. Hoses must be prope rly run on overhead brackets or similar methods to prevent tangling of the hoses or causing trip hazards. All hoses and systems must be protected from any weldi ng sparks and molten metal. Proper portable fire extinguishers shall be supplied at each welding or cutting work areas. A fire watch shall be implemented to che ck the site at 30 minutes and then 2 hours after welding. Electric Welding Machines

5 All welding machines must be earthed and protected by an adequately sized, singl e or 3 phase Earth Leaking Circuit breaker (ELCB) OR Ground Fault Current Interr upter (GFCI). All welding machines shall be grounded. All electrical welding mac hines must be located in a waterproof location. All electrical machines must hav e 2 welding leads, an earth lead and an electrode lead. The earth should be conn ected as close as possible to the point of welding. This distance must not excee d 10 meters at any time. The cables must be specified correctly and have the cor rect type of insulation and the correct cross sectional area for the required cu rrent carrying capacity to avoid overheating. All connections must be properly c rimped and the machine must have an approved electrical label fixed on it. All w elding rods shall be removed from the electrode holder when not in use. LBC Project HSE Plan, Ewekoro All equipment shall be in good condition, properly maintained and have all requi red certificates required by country and local laws and regulations. All equipme nt must be provided with documentation of an annual inspection by an independent third party, and the manufactures operations manual. The annual inspection certi ficate and the above described documentation shall be submitted to Lafarge prior to the crane arriving on the site. All crane activities shall be conducted by a trained and certified competent person. Back up, movement and slewing horn shal l be in proper working condition and must be regularly inspected. Trained signal men must be present the full time the cranes are in operation. PW shall provide a Lafarge approved communication system before any crane would be used on site. The area around the crane and under the path of a crane load must be roped-off with white and red streamers or other approved methods. Crane outriggers must re st on firm ground, adequate for the load. If the supporting ground for the crane is soft, the lift shall not be made without properly applied crane mats or stee l plates. No lift shall be made if the crane is not on level ground. Crane cable s and slings must conform to all country and local regulations and must be inspe cted routinely as required by Lafarge. All cranes must be fitted with operationa l anti-two blocking safety systems. All cranes must be fitted and equipped with functioning load indicators. All cranes must be fitted and equipped with levelli ng systems. All cranes must be equipped with proper pads to distribute the stabi lizer loads. For heavy or critical lifts, no lift will be attempted when the win d speed is over 30 KPH and care must be taken where wind speeds over 15KPH are p resent. All loads must be supervised by only by competent certified riggers. Cra ne operators and riggers must undergo and pass a medical examination to confirm they are medically fit for the tasks. Eye tests must be completed for any crane operator and rigger. All riggers and Tower crane operators must also be checked for Vertigo, High Blood pressure, and other ailments that could affect their wor k.PW shall submit a list of the tests he plans to perform on crane operators and riggers to Lafarge for review and approval before the start of any lifting oper ations. The areas below operating cranes, including lift and swing radius areas will be clearly defined and marked. Any drop zones must be established and cordo ned off prior to any lift. Any personnel not related to the lift activity must b e removed from these areas during the lift. Crane activities will be suspended i n the event of poor weather conditions. Cranes shall be evaluated for aircraft f light pattern issues and identified, as needed, with flagging and/or beacon. In the event of the use of a crane PW submit a documented crane plan before utilizi ng any crane on site. The crane plan must be reviewed and approved during the pr eConstruction Safety Review. A daily documented safety inspection shall be compl eted and maintained for each crane and made available for review by Lafarge when requested. No lifting shall be made that exceeds 90 percent of the manufactures rated capacity of the crane, as indicated on the cranes capacity (load) chart(s). Crane load charts specific to the crane must be available inside the crane oper ators cab or the crane must be immediately taken out of service. Page 40 of 55

LBC Project HSE Plan, Ewekoro A communications system acceptable to Lafarge must be developed for multiple cra ne operations. When two cranes are working in the same area, a procedure must al so be submitted explaining the method of coordination to be used between the cra nes to ensure any possibility of a collision is prevented. Mobile cranes are onl y to be used with outriggers fully extended and tyres off the ground unless manu factures recommendations allow other means. If the full range of motion of the li ft is not visible to the operator, a signal man or radio communicator must be pr ovided. All loads shall have a tag line attached to the load when possible. Must be designed and engineered by competent person with a professional licence spec ific for that activity. Must be installed by competent personnel. Must be inspec ted and approved for use by a person in accordance with requirements and frequen cy stipulated by the local laws and regulations. Must be certified as fit for th e use intended by a competent person. Must be load tested after full assembly. M ust meet all specified rigging and safety requirements. Must be periodically ins pected in line with the cranes maintenance plan and any related local or country regulations. Minimum inspection periods cannot exceed one month. Slings, Rigging and Lifting Equipment:

Tower Cranes 6

Qualified and properly certified riggers will only be allowed to rig material or equipment to be lifted by a crane. All slings, rigging and lifting equipment wi ll be inspected in accordance with British standard, OSHA 29 CFR 1926.552 or equ ivalent standard as approved by Lafarge. All wire rope slings must be factory ma nufactured and certified by a competent authority. No handmade or hand woven sli ngs shall be utilized on any Lafarge site. All slings must be tagged to indicate their lifting capacity. All slings, rigging and lifting equipment shall be insp ected by a qualified person (as the term qualified person is defined by OSHA), dai ly before each use. A colour coded tagging system must be used for the inspectio ns and tracking of the slings. All slings, lifting tackles and shackles shall be inspected every 3 months. All Slings must be tagged and colour coded to indicat e that they have passed inspection and are permitted for use. If any slings are frayed or have red strings showing that they were overstressed, they will be tak en out of service. If the slings show signs of any heat related stress, they wil l be taken out of service. Wire rope rigging will be checked for broken wires or strands, bird caging, flat spots, burn marks, or melting. If any of these condi tions are found, they will be taken out of service. All rigging equipment shall have the safe lifting capacity displayed by means of a tag or other marking. Lif ting equipment such as shackles, manufactured spreader bars, chain falls or come a-longs will be inspected for any type of distortion, cracking of the metal and any flat or worn spots. If any of these are found the equipment will be removed from service. Page 41 of 55

LBC Project HSE Plan, Ewekoro PW shall be required to submit to Lafarge professionally engineered drawings of any field fabricated spreader bars or lifting devices. These devices must be des igned or approved by a Professional Engineer and must be inspected and certified prior to use. (Including welding inspection, load testing and load calculation) . PW is required to ensure that any lifting lugs, pick points, slings and liftin g devices are in good condition and robust enough for the intended load. All sli ngs or cables must be used in a way to ensure the handling of the load will occu r without any risk potential spillage. A signal-man must be on hand when moving loads with slings or cables and the whole area must be roped off. All loads must be lifted vertically if possible. If a skewed lift cannot be avoided, there sha ll be a presence of a PW supervisor managing it and taking all possible special precautions. Taglines must be provided and utilized where possible to guide and control loads. Sling must be protected against any sharp edges to avoid damage. Synthetic Slings should be used to lift steel or other equipment if the paint co uld be damaged. PW shall be able to provide copies of certification for any lift ing and rigging equipment. All clips and shackles must be installed properly in accordance with best practices. The riggers must be capable of properly assessin g the loads, determine the proper type of slings, shackles and related tackle re quired. They must also be able to determine the centre of gravity of the load, a ssess the weight of the load, the lifting capacity of the sling and accessories depending upon the type of lifting method utilized. All winches must be anchored in position, properly. Any pulleys or sheaves must be secured safely. All lifti ng wire rope must be regularly inspected and be replaced if the strands of the w ire rope are found to be damaged. All inspections must be completed by competent , certified persons.

Lifting & Rigging Plans Lifting and rigging plans and the associated calculation s are related information that shall be provided by PW with the related method s tatement and risk assessments in advance of any related work for Lafarge review, comment and approval. Critical Lift Plans: Any lift exceeding 75 percent of the cranes rated capacity or multiple lifts shall be considered as critical lift. A pre-lift meeting shall be held in the field with the crew involved before makin g the lift. Any lift above 5 tons requires a lifting permit. All multiple tandem crane lifts require a lifting permit. 7 Man Baskets The use of a man basket on a crane is prohibited without prior Lafarge review an d authorization. Page 42 of 55

LBC Project HSE Plan, Ewekoro All cranes and man basket activities must be in accordance and fully comply with British Standard, OSHA 29 CFR 1926.550 requirements or a Lafarge approved equiv alent standard.

All man baskets shall be purchased from a reputable supplier designed and approv ed by a Professional Engineer (PE). Man baskets shall not be manufactured at sit e. All welds must be inspected by an approved expert with the proper inspection tools and equipment. All man baskets must be certified and be provided with a un ique serial number. All man baskets must be load tested prior to the first use o n site. All man baskets must be inspected prior to each use. The slings used wit h the man basket must be inspected prior to each use. The basket must be lifted by 4 individual slings and shackles, at a minimum. A proper locking mechanism sh all be provided for locking the access door. Any shackles must be lifting wired so that they cannot accidentally open. A separate sling must be provided for eac h occupant to attach their full body safety harnesses to and shall be attached d irectly to the crane lifting hook. The crane must have proper locking devices to ensure the sling hooks cannot slip from the crane hook. A non-skid floor Full t oe-boards Full handrails A Steel frame with 4 structural elements connected to a hanging ring having a safety factor as required by local regulations or greater than 10, if there is no governing country or local rules. The number of personn el will be limited to 3. All personnel in the man basket must wear a full body h arness clipped on the crane hook. All slings utilized for lifting man baskets mu st not be utilized for any other lifting purpose or activity. Mobile Elevated Working Platforms (MEWPs) All man baskets must be built with the following:

8 MEWPs are to be operated only by personnel who have been trained and qualified wi th current certification on the model that they will be operating. Personal fall protection shall be worn at all times and must be connected to the approved att achment point while working in a MEWP. Any scissor-lifts shall not be used witho ut t safety closure in place across entrance. Hi-jacks or similar devices shall not be used without the outriggers fully extended. All operating and safety manu als, and other related inspection must be located in a weatherproof box on the e quipment. All related equipment shall be inspected prior to use and documentatio n of annual inspection certification shall be provided to Lafarge prior to mobil ization on site. Page 43 of 55

LBC Project HSE Plan, Ewekoro Aerial-lifts, articulating and telescoping boom lifts, scissors lifts, mast clim bers, spider, and similar equipment shall not be operated when wind speeds excee d the manufacturers operating instructions. Aerial man lifts shall not be used to lift any suspended materials. MEWPs shall not be used on unsuitable or inclined ground exceeding the design capabilities of the specific device. All outriggers or stabilizers must be set and adjusted before MEWP is elevated from the fully l owered position. Personnel utilizing MEWPs must be supervised at all times they a re operating the lift. Personnel must never climb on the handrails of the MEWPs b asket. PW shall supply Lafarge with a list of all types of MEWPs they plan to use on the site before the equipment is believed to allow Lafarge to accept or reje ct the equipment. In case the case of expedited equipment is provided which was not anticipated on the list, the equipment must be inspected by Lafarge before u se on the site. 9 * Alimak Lifts / Gondolas / Bosuns Chair Theses devices must be designed, specified and engineered by computer persons with professional certif ication from the country they equipment will be used in. The hand-tools must be inspected every 3 months and utilize a colo r coding system to describe their condition. All electrical tools must be equipp ed with industrial waterproof plugs and sockets which can meet IP55. All safety devices and switches must be functioning and be in place for the tool to be cert ified fit for use. 12 Dewatering pumps Diesel dewatering pumps must be utilized in all situations where practical. If electric sump pumps are utilized, the temp orary power distribution system must be designed for the environment, with each distribution circuit equipped with an Earth Leakage Circuit Breaker (ELCB) or Gr ound Fault Current Interrupter (GFCI) protective devices and be reviewed, inspec ted, tested and pre-approved by Lafarge 13 Generators All generators must be ear thed with proper neutral and earth bars. F 1 CONSTRUCTION STANDARDS Piling: The Contractor must provide an acceptable standard practice to Lafarge before an y related work can be performed on site. Lafarge reserves the right to challenge and or reject any standard practices they determine is not safe. 2 Excavations All excavations must be engineered, managed, supervised and controlled by compet ent persons. A method statement and related risk assessments shall be prepared f or any excavation deeper than 1.0 meter. Before starting any excavation, PW must plan against: Any potential collapse of the sidewalls. Any material falling ins ide the excavation. Any potential for people or vehicles falling into the excava tion. Any potential for undermining adjacent structures. Any potential of hittin g any underground services. Page 45 of 55

LBC Project HSE Plan, Ewekoro PW shall ensure that all necessary supporting, draining and access equipment is on site before starting the work. They must also ensure the work has proper esca pe plans and systems. No one is allowed to work alone in an excavation deeper th an 1.0 meters. Care must be taken to keep traffic away from any excavations. Equ ipment operators and truck drivers must not operate closer than the recommended minimum clearance distances from overhead or underground electrical wires as det ermined by the local utilities. If work is required near these utilities, the Co ntractor must consult with Lafarge and provide pre-approved written alternative action plans. All excavations must be substantial and adequately barricaded in o rder to protect pedestrian vehicular traffic from entering the work areas. All b arricades must withstand an axial force of 100 KG at a minimum. All barricades m ust be 1.1 meters high with both upper and mid rails. Mid-rails must be at 0,55 meters from the ground. All temporary barriers must be secured and tied to preve nt the barriers from falling over or being blown away during high winds. All bar riers must be placed at a safe distance to ensure personnel or equipment cannot enter the excavated area. The position of the barricade should also be placed at a safe distance, taking into account the soil conditions and the excavations sl oping and benching. All excavations must be sloped or benched per OSHA or equiva lent standards, as approved by Lafarge, and shored or safeguarded through the us e of a trench box or other engineered earth retention device(s) when excavation reaches 1.8 m or greater in depth. Protection against a possible cave-in at a de pth of less than 1.8 m may be required if the Competent Person determines that s oil or other conditions warrant such protection. Soil classification: Definition s of soil classifications and approved inspection methods are outlined in 29 CFR 1926.652, Appendix A to Subpart P. All soil and rock deposits shall be classifi ed in accordance with 29 CFR 1926.652, Appendix A to Subpart P Sloping and Bench ing. If the Contractor is unfamiliar with this standard, they can request a copy from Lafarge. Soil types maximum allowable slopes; A B 3/4:1 53degrees 1:1 45de grees C 1.5:1 34degrees All soil classifications increase 1 level if heavy machinery i s operated within 100 meters of the excavation {i.e., A becomes a B, B becomes a C). All excavation walls must be checked after heavy rains and during a thaw cy cle before a trench or excavation can be entered. Shoring systems must be inspec ted and checked for integrity to provide access in the excavation. Trench boxes, shoring systems or sheet piles must be utilized in poor soil conditions where t here is a potential for the soil to collapse. PW will develop an action plan to mitigate all adverse weather effects, including monsoons, flash floods or heavy rains and provide adequate drainage facilities, equipment and diesel de-watering pumps. 3 Drilling, Blasting & Explosives Page 46 of 55

LBC Project HSE Plan, Ewekoro Only authorized and qualified persons, pre-approved by Lafarge, will handle, set and detonate any explosives. No open flames, matches, or other incendiary devices will be allowed near or aro und explosive magazines. All explosives will be accounted for at all times and a detailed inventory must be kept by PW for review by Lafarge at any time. All ex plosives must be kept in a locked, dry, and cool area until such time they are t o be used. All debris from the explosive supplies shall be disposed of in a preapproved area. Other non-related materials cannot be stored in the same storage space as the explosives. Signs with clear warning against the use of radio trans mitters will be located by the PW on any roads or work areas within 250 meters o f the blasting zone. There shall be no blasting or explosive placement activitie s in the event of an electrical storm. Special precautions must be documented an d taken when blasting in any congested work area or when blasting near utilities , both above ground and underground. A blasting warning and an all-clear siren must be provided and utilized by PW. F lagmen or other Lafarge approved signalers must be provided to ensure no personn el enter the blast zone. PW shall propose for every stage of the work the follow ing items: A Blasting Program A location and orientation plan of the area. Any d rilling, loading and blasting patterns, Related methods for scaling and grading of the slopes and waifs. Written Safety procedures pre-approved by Lafarge. Explosive storage PW must provide, in writing, all safe working practices and re lated management plans for the storage, transportation and handling of explosive s in compliance with country or local laws and regulations. 4 Rebar Placement, P lanning & Protection: Prior to any work involving rebar, a sequencing plan will be submitted to Lafarge for review. No one shall be permitted to work above vert ically protruding rebar unless it has been protected to eliminate the hazard of impalement or other related injury. The tops of all exposed rebar must be covere d with a rebar cap, wood board or similar device. 5 Slip-forming PW must provide an acceptable standard practice to Lafarge before any related wo rk can be performed on site. Lafarge reserves the right to challenge and or reje ct any standard practices they determine is not safe. 6 Silo Construction PW must provide an acceptable standard practice to Lafarge before any related wo rk can be performed on site. Lafarge reserves the right to challenge and or reje ct any standard practices they determine is not safe. 9 Scaffolding General: The following options must be considered at the contract negotiation st age and one of the options will be selected to provide scaffolding solutions for the project duration: Independent scaffolding erection company Dedicated scaffo lding team supplied by PW Lafarge reserves the right to provide internal or 3rd party scaffolding resources who will be empowered to supervise and control all s caffolding erection. Scaffolding design and use: At a minimum, PW will be requir ed to follow at minimum the Chinese code {JGJ130-2001) or BS 5973 & EN 12811-1 a nd TG20:08 performance requirements and general design considerations for Scaffo lds. System Scaffolding (Peri, Haki, Layher, Doka, or equivalent d by Lafarge) should be used for all tasks, where possible, Tube & Coupler scaff olding should be used for applications for which it is not possible to utilize s ystem scaffolding, including special applications such as cantilever scaffolding , working platforms and cyclone support structures. Stairs towers must be utiliz ed to provide access for any silos and temporary access to buildings and structu res. Scaffolding ramps are not to be used in these applications. For all working levels, there must be fall protection comprised of handrail, "knee"-ledger and toeboards. On other levels, toe-boards are not necessary. At the inside of the s caffolding i.e. the side facing the building, there must be a three-piece constr uctional fall protection, handrail, "knee -ledger and toe-board, if the distance between scaffolding and the related structure is more than 20 cm. Scaffoldings in or near traffic areas must be equipped with safety devices that protect the s caffold from collision or accidental impact. Reflecting warning signs must be in stalled before the scaffold is used. Scaffolding situated above roads must have signboards defining the clear passage height (if the height is less than 10 m. S caffolding situated at roads or at points of transfer and passage must be equipp ed with protective flooring to prevent tools or debris from falling to the area below. Ground or floor conditions have to be capable of accepting the load to th e scaffold fully loaded. Scaffoldings wheels must be equipped with a locking or blocking system. Any wheels must be blocked when the scaffolding is being used. Riding the scaffolding when one is being moved is forbidden. Page 48 of 55

LBC Project HSE Plan, Ewekoro Scaffold tagging will be required for all scaffolds. The tagging system shall in clude identification for completed safe scaffolds and incomplete scaffolds. All diagonal bracing must be in place and secure. Braces do not take the place of mi d and top rails. The scaffold system must be securely braced against the structu re per the minimum requirements of approved country and local standards. If the scaffolding system is to be enclosed for wind or weather protection, it must be designed by a competent person to withstand the additional loads. Decks: For construction levels (i.e. levels used only for erection/dismantling o f the scaffold} then the width must be at least 60cm. Planks are to be placed ne xt to each other, with less than 3 cm gaps. Work platforms must be fully planked , except during the erection and dismantling process. Two planks or at least a 4 5cm wide work platform will be provided at any time the level is accessed. All p lanking situations shall follow either British standard, OSHA or equivalent Lafa rge approved standards. A thickness of at least 3cm (throughout) is required for all wooden planks Iron hoops at the front of timber planks are preferred to prevent clefts in the decks, No clefts, knotholes or cuts are allowed in any deck members. Compliance must be maintained with the maximum span of wooden decks or planks in relation t o thickness according to proper design and any country or local regulations. Dec ks and planks are to be secured as to prevent them from moving. The minimum widt h of the working platform shall be 60 cm. Sufficient space on the edges must be provided to allow access to the ledge tube at the platform level with a harness hook when there is no other way to clip the harness. This space shall be less th an 20 cm .and the system must be designed to accept this load. All platform boar ds must be designed to support the intended loads. Access: Ladders must be included as early as possible in the erection process an d removed as late as possible in the dismantling process to reduce the risk of p ersonnel climbing on the related structure. Incorporation of protected ladder ba ys is required. Ladders outside the scaffolding framing are not allowed. Ladders must be fixed at an angle between 65 and 75 (see picture below). For scaffoldings of a width of more than 1,0m, the access decks should be mounted in the same di rection, one above the other, in order not to have opening at the bottom of the previous flight of steps. If an access deck arrangement as described above is no t feasible, additional measures must be taken to ensure that the is protected (s ee pictures below). Climbing in or on the scaffold frame is forbidden. Ladders m ust be arranged inside the scaffolding framing. Ladders must not be arranged out side the scaffolding between lifts. Inspection and use: Scaffolding shall be used as long as it has been inspected a nd approved by PW s Safety Officer. Lafarge reserves the right to restrict the u se of any scaffold they believe is Page 49 of 55

LBC Project HSE Plan, Ewekoro unsafe, When the first level has been erected, a warning sign must be attac hat the scaffold is not allowed to be entered, other than by the personal erecti ng it. After the scaffolding has been erected, the scaffolding foreman will plac e an "inspection certificate" at a prominent location near the access, stating t hat scaffolding is released for use. This "inspection certificate" will take the form of a green tag. Payload per area [kg/m2] must be provided by PW for all sc affolds. If the scaffold has more than one entrance or access, multiple tags wil l be needed at each entrance. Once erected, all scaffolding must be inspected at least once a week or if it is modified in any fashion. When the scaffolding is not used anymore or if the use is no longer safe, the inspection certificate mus t be immediately removed and a red tag will be put in place. Until the scaffoldi ng is dismantled completely, there must be a warning sign, that the scaffold is no longer allowed to be entered. Manually Propelled Mobile Scaffolds - All caste rs shall be provided with a positive locking device to prevent the scaffolding f rom rolling. Platforms will be tightly planked for their full width. The floor o r work surface must be free from voids, holes, or obstructions. The height of ro lling scaffolds must not exceed four (4) times the shortest base dimension.

Erection and dismantling: Tools: Proper tools must be provided for all workers e ngaged in scaffolding erection including the following: Tape measure > 5 meters Wood saws Pipe saws Scaffolding spanners 1 meter spirit level Proper scaffolding fixtures and fittings must be provided, including: Mudsills B ase plates Screw jacks Scaffolding planks Scaffolding ladders Couplings The "ass embly and application instruction" of the scaffolding and the scaffolding design has to be on site and reviewed by the scaffolding team before any erection acti vities. The Scaffolding design and erection drawings must be provided for all sc affolds > 5 meters in height. The scaffolding team must determine within the int ended assembly area if there are risks existing. Risks could be electrical insta llations, shafts and or tunnels, cranes, conveyor systems, poor load-bearing fou ndations and traffic to name a few. Scaffold components must be inspected for ex cessive wear and or and damage before installation. Damaged or inappropriate ele ments are to be removed from the site immediately. There shall be an employee at every level during manual vertical transport of scaffold elements. During trans port by powered measures such as elevators or winches, the work zone has to be b arricaded. Scaffolding shall be completed progressively with scaffolders install ing a single guardrail on all Page 50 of 55

LBC Project HSE Plan, Ewekoro lifts to provide protection while at work or traversing the works. Scaffolders m ust be secured in a full body harness when installing components outside of the handrail. All erection works must adhere to the following sequence; Visual inspe ction of the scaffolding element that shall be used - all damaged or inappropria te elements must be rejected. Lay down load distributing bases and position thre aded base plates. Put up the first two frames or poles into the threaded base pl ates, connect them by ledger near the base plates, put in the decks and fit the vertical diagonal member. 3. Adjust the threaded base plate according to spirit level of frames and ledgers. 4. Repeat steps 2 to 4 until the required full leng th of the scaffold is reached. 5. Beginning with the first level (approximately 2 to 4m) and then at every level, there must be a handrail, an intermediate addi tional "hand"- and "knee"-ledger, as well as toe-boards. 6. Immediately after th e first scaffolding level has been erected, a warning sign shall be placed near the access ladder stating that the scaffolding is under erection and shall not b e used other than by approved scaffolding erectors. 7. Before entering the next level, the scaffolders must tie themselves off, either to the ledger on the walk ing level or to a steel tube that shall be fixed vertically outside near the acc ess deck by couplings. Never shall the scaffolders work unprotected. 8. Repeat t he proceeding steps till the whole scaffold is erected. 1. 2. For dismantling th e above procedure must be followed in reverse order. Anchors and diagonals have to be mounted continuously during the scaffolding progress During erection, dism antling and alteration, it is not allowed for any workers to stay on the scaffol ding, Inspection & Use: The Scaffolding should be approved and inspected by the Contra ctor s scaffolding expert and determined to be "Safe" & "Fit for use", then the Contractor must request Lafarge to complete a safety review and inspection of th e scaffolding. The scaffolding will then be inspected and approved by the Lafarg e scaffolding expert, or the expert scaffolding company or resource engaged dire ctly by Lafarge. Scaffold Inspection Tagging System: A Scaffolding inspection tagging system must be implemented at site which clearly highlights the status of each scaffolding system. The Scaffolding inspection tagging system should be provided by the cont actor and follow the conventions below. o Green: Safe for use, inspected and sig ned with date and next inspection due date indicated. o Yellow: Fall protection or special use issues described in writing. o Red: Do not use, under erection, m odification or disassembly clearly described in writing on the tag. PW or expert scaffolding company must inform the Lafarge site management team, when and wher e they intend to erect, modify or dismantle a scaffold in advance to ensure the appropriate tags are attached. LBC Project HSE Plan, Ewekoro An access ladder or other approved method is required to allow safe access and m ust be provided for all scaffolding. If the fall distance exceeds 5m, stair towe rs or internal ladder systems must be used. Training, inspection procedures, mai ntenance, and operation of self-propelled mobile scaffolds must comply with the manufacturer s requirements and documentation. For mobile scaffolding towers, ou triggers or stabilizers must be used, if required by the manufacturer All guardr ails must be in place and access gates closed while the unit is in use. Minimum safe distances from energized power lines must be maintained at all times. PW sh all submit to Lafarge a plan for any temporary scaffolding and platforms to be u tilized during the erection of cyclones, other irregular shaped components or us es highlighted by Lafarge as requiring a plan. PW shall provide an acceptable st andard practice to Lafarge before any related work can be performed on site. Laf arge reserves the right to challenge and or reject any standard practices they d etermine is not safe. 11 Equipment Erection PW shall provide an acceptable stand ard practice to Lafarge before any related work can be performed on site. Lafarg e reserves the right to challenge and or reject any standard practices they dete rmine is not safe. 12 Temporary Electrical Power Distribution System PW shall comply with the Energy Isolation and Lockout / Tagout / Tryout (LOTOTO) policy and procedure established by Lafarge for the Project. PW shall review th is policy to ensure it covers all her needs to ensure their workers are properly protected in terms of LOTOTO risks. The design, construction, operation and mai ntenance of the site temporary power supply must comply with all relevant countr y and local regulations. A single line drawing (SLD) and related location map mu st be available at site with details of the cable routings and must be maintaine d as the system expands or contracts. The details must be kept up-todate and be available for inspection by Lafarge. A map must be maintained, indicating all te mporary cable routings for the site and which Contractor is responsible for each section. All sections and components of the temporary power distribution system must be labeled clearly indicating the source of power and the point of isolati on. PW shall ensure that electrical panels have dry wooden platforms, rubber mat s or other nonconductive materials under them to protect employees accessing the panels. All panels must be located in waterproof locations. All panels must be lockable and reach IP55 {be waterproof), and reach IP20 {all live parts must be protected against accidental contact or touch) when the panel doors are open. Th e key must be under the control, at alt times, of a competent person approved by Lafarge. All panels, distribution circuits must be designed to be to isolated a nd locked for all incoming and outgoing power circuits. Page 52 of 55

LBC Project HSE Plan, Ewekoro The temporary power distribution system must be earthed properly in line with th e TNS system. A 5-core cable must be used to distribute the 3 phase, neutral and earth throughout the temporary power distribution system. For single phase powe r, a 3 core cable with Live, Neutral and Earth must be utilized. All cables shal l be suitably designed with adequate wire cross sectional areas for the applicat ion. PW shall ensure that all electrical panels have a disconnection device for each branch circuit, which must be labeled to show what each panel and branch co ntrols. These panels must remain permanently and properly locked. When generator s are utilized for temporary power, earthing and neutral bars must be establishe d. Ail single and 3 phase incoming and outgoing lines must be protected by an Ea rth Leakage Circuit Breakers (ELCB) or Ground Fault Current Interrupters (GFCI) of suitable and adequate size. Lafarge reserves the right to reject any device t hey determine unsuitable for the application, Adequate PPE and safety equipment must be provided to work on temporary power distribution systems. PW shall suppl y a detailed list of all PPE they plan to provide for review prior to mobilizing on the site. No live work is permitted on any temporary power distribution syst em. The system must be isolated, locked out, tagged and verified that it is prop erly isolated. The electrical Contractor shall test ELCB s and GFCI s for the te mporary power distribution system on a monthly basis, A record of this testing m ust be maintained at the project site and available for review by Lafarge. Porta ble ELCB s or GFCI s are to be tested daily. ELCB s or GFCI s must be used with all temporary power systems including all cord connected equipment and power too ls. All plugs and sockets must be of an industrial type, with male & female conn ections and capable of reaching IP 55. All portable electrical tools, and extens ion leads or cords, and devices must be inspected, approved and tagged prior to use and audited and tested following a clearly defined inspection plan and colou r coding system. Lafarge reserves the right to approve or reject this procedure, Portable hand tools must be removed from service when they become defective or if they do not meet minimum safety requirements required by Lafarge. Cables must be run above ground if possible. If this is not possible, they must be protecte d by steel pipes, as when buried under ground at road crossings. Locks & lockout devices must be provided for all circuit breakers. Extension cords shall be ins pected daily prior to each use. If a cord is found to be damaged, it must be rem oved from service immediately. Cords shall be run in an orderly manner as not to cause tripping hazards. PW shall identify all extension cords with a tag or sim ilar method, identifying the its company name. No electrical cords will be fasten ed with staples, hung from nails, suspended by wire, or positioned in doorways, windows, or other openings that may result in pinch points. Where cord sets have the potential to be damaged or could cause an unsafe condition, cords must be s uspended, (e.g.; aerial/scissor lifts, drywall carts, trash carts, main walking path trip hazards, etc.) All temporary power distribution systems, electric mach ines, tools, devices and equipment, handtools etc. must be inspected and tested on a regular basis and a documented record of inspection and testing must be mai ntained. 13 Commissioning A Commissioning Plan, specific to the project, will be developed with PW and Laf arge to ensure proper safety methods are employed in this potentially dangerous period of the project. It will include the following topics; Plant Equipment Ene rgizing Page 53 of 55

An y questions concerning compliance with these standards must be submitted, in wri ting, by PW before it mobilizes on site.
