ABOUT LOGBOOK
A logbook is an official record book used to keep track about activities performed by the students throughout their Clinical Placement. Logbook consist List Of Activities assigned for them to fulfill. It is designed for coordinating learning with LOCAL PRECEPTOR . Students are required to attend to activities listed and signed by Local Preceptor. Fulfilling activities in the Logbook is of a mandatory for the following Clinical Placement: YEAR 2 SEMESTER 1 (Physical 1) 2 weeks Focus for this clinical placement is to develop competency in OCCUPATIONAL THERAPY ASSESSMENT YEAR 2 SEMESTER 2 (Psychosocial 1) 2 weeks Focus for this clinical placement is to develop competency in OCCUPATIONAL THERAPY ASSESSMENT YEAR 3 SEMESTER 1 (Physical 2) 8 weeks Focus for this clinical placement is to develop competency in OCCUPATIONAL THERAPY APPLICATION TO PHYSICAL CONDITIONS YEAR 3 SEMESTER 2 (Psychosocial 2) 8 weeks Focus for this clinical placement is to develop competency in OCCUPATIONAL THERAPY APPLICATION TO PSYCHIATRIC CONDITIONS YEAR 3 SEMESTER 2 (Pediatrics) 8 weeks Focus for this clinical placement is to develop competency in OCCUPATIONAL THERAPY APPLICATION TO PEDIATRIC CONDITIONS A completion of Logbook is a pre-requisite to qualify students for final examination. Learning Objectives for every Clinical Placement are also incorporated in this Logbook. Students are required to attend to learning objectives listed, prepare documentation and compile for marking.
This Logbook has a marking sheet to compile results of all activities students required to perform throughout their Clinical Placement.
Goals
To produce graduates who are competent in delivering safe and effective occupational therapy services.
To promote a balance of personality development including physical, intellectual, emotional, social, spiritual and esthetical.
To develop acceptable professional attitudes including caring, accountability, dynamism, creativity, innovative, and teamwork.
To instill a good self concept and appreciating the concept of lifelong learning

GUIDELINES
FOR STUDENTS AND LOCAL PRECEPTORS
STUDENTS
1. Each student must meet the objectives of the clinical placement. Students must a. complete all task that are applicable in this logbook. Completion of Logbook is a mandatory to
Preceptors. f. assessment forms (standardized / non-standardized) being used should be put into your clinical file for references.
g. receive clinical performance appraisal by Local Preceptors using Clinical Performance Report
Form (Appendix B). 2. Student is expected at all times to conduct himself/herself in a seemly manner that is unlikely to cause offence to members of the general public. In particular while attending a hospital or
community health clinic, each student must be dressed in a manner, acceptable to the clinical staff and patients, maintain clinical confidence absolutely, and at all times be courteous to every patient they meet. 3. Attendance at teaching sessions. a. b. Each student is required to attend all the teaching sessions appropriate for the part of the course in which he/she is enrolled. Each student who, for any reason, finds himself/herself unable to attend any such particle class or examination or other assessment procedure should, wherever possible, obtain the prior approval of his/her Teacher/Local Preceptors or, where this is not possible, provide an explanation as soon thereafter as possible. c. Each student must obey all the Laws of Malaysia and to conform to the expected norms of good conduct and behavior during transport to and from and during the course of clinical teaching sessions in hospital, clinics and health centers. d. Each student are expected to treat the buildings, library books, apparatus and other facilities provided by hospital, clinics and health centers with care and respect. e. Any student who damages or breaks the properties will be required to pay for its repair or replacement. Smoking of tobacco is prohibited in the premises of the hospitals, clinics and health centers. f. g. Students attending the teaching sessions in hospitals, clinics and health centers should at all times be in proper attire and wear the college name tag. Students are subjected to all rules and regulations of KSKB and disciplinary action will be taken if these rules are not adhered to. clinical teaching session, or any

Students shall abide by all the rules, regulation and procedures of hospitals and health centers. i. Students shall handle all patient records in strict confidence and shall not divulge any information concerning patient care to unauthorized personnel.

Students shall follow the prescribed rules for infection control in the wards and clinics.
LOCAL PRECEPTORS
1. Local Preceptors must ensure students i.
Attend task listed in Logbook. Local preceptors are to sign in the box given for each task / activity Complete Logbook is a mandatory to qualify students to sit for Final Examination.

Perform task listed in Competency Assessment Manual and to be mark and graded by Local
Preceptors.

Receive clinical performance appraisal by Local Preceptors using Clinical Performance Report Form
(Appendix B).
2. The local preceptors will monitor, guide, advice, observe and assess students continually.
3. The local preceptors will allow students to participate in the organizational activity (official
duties), this may include patient assessment and other departmental duties 4. Local preceptors have a duty to inform the College or Head Program of Occupational Therapy, concerned should any accidents or mishap occurred.
5. Kindly ensure that you mark the Clinical Performance Report at the end of the Clinical Placement
and fill up the profile/comments of students performance at the end of the Clinical Placement.
CLINICAL OBJECTIVES
YEAR 2 SEMESTER 1
(Physical I)
1. Carry out structured interview with two clients. Document the occupational profile. (D) 2. Conduct assessments as stipulated in clinical competency assessment booklet. 3. Design and fabricate one static splint. Reflect on the fabrication of the splint and suggest
improvements. (D)
4.
Conduct and interpret the findings of an assessment and reassessment on the same patient. (D)
5. Carry out two interventions on two different clients, under the following headings: (D) a. Aims of the intervention
At the end of the placement, the student must submit written documentation (D) on the above activities and to be certified by the local preceptor. Upon return, each student must hand over the file to the Head Unit of Clinical Practice, Occupational Therapy Program for assessment.
