PRACTICUM GUIDELINES VISION MISSION Vision De La Salle University-Dasmarias is a Filipino Catholic University established and managed by the De La Salle

Brothers and their lay partners in the historic province of Cavite in response to the needs of the Church and the Nation for human and Christian education, particularly the youth at risk. Guided by the Lasallian values of Religio, Mores, and Cultura, the University participates meaningfully in the process of social transformation by forming God-centered, people-oriented, and patriotic persons who serve as responsible and professionally competent stewards of God's creation. PRACTICUM GENERAL OBJECTIVES To enable the students to apply and appreciate the relevance of classroom learning to the actual work setting. To provide students with workplace experience/exposure in their respective fields. To enhance the departments network and linkages vis--vis career placement for its graduates by establishing and maintaining harmonious relationships with institutions. To provide opportunities for future professionals relative to the labor market To continuously improve the OJT/Practicum Program of the departments by gaining inputs from the partner institutions. Exhibit the spirit of faith by interpreting, judging, and evaluating lifes realities in the workplace in light of the Gospel.

Demonstrate the spirit of zeal by translating the acquired knowledge into practice in assisting individuals in the workplace deal with their everyday human experience and problems. Manifest the spirit of communion when engaged in actual understanding and appreciation of persons to promote individual and social change.

DEFINITION OF TERMS Practicumer refers to the student enrolled in the practicum program. University refers to De La Salle University-Dasmarinas (DLSU-D). Institution / Company refers to the organization for possible practicum setting accredited / acknowledge/ recognized by the university. Practicum Supervisor refers to the faculty member from DLSU-D who oversees the practicum activity. On-site Supervisor refers to the representative of the institution/company who supervises the practicumers.

PART I THE PRACTICUMER Section 1 - Admission to the Practicum Program 1.1 An incoming practicumer should finish all academic/nonacademic requirements prerequisite to the practicum program endorsed by the academic adviser and approved by the department chair. An incoming practicumer should secure a waiver duly signed by his/her parents/guardian, which serves as proof that DLSU-D will not be held liable for any untoward incident that may occur during the practicum/internship period.

Section 2 - General Directives 2.1 An admitted practicumer should abide by the rules and regulations set forth by the institution/company where s/he is undergoing internship. The practicum site may be assigned by the practicum supervisor or chosen by the practicumer. In the latter case, s/he has to request for a formal approval from the practicum supervisor.

The practicumer should pay the required practicum fee determined by the department concerned to cover the expenses for this purpose. The practicumer should always observe discipline and right conduct. 2.4.1 2.4.2 2.4.3 2.4.4 2.4.5 2.4.6 S/he should always project the Lasallian image. S/he should not engage in any form of intimate relationship with clients, patients, students, staff and onsite intern supervisor. S/he is not allowed to invite/entertain personal visitors in the practicum site. S/he should avoid incessant talking and loitering in the practicum premises. Cellular phones and other electronic gadgets should be turned off or set on a silent mode when s/he is on duty. The practicumer should not engage in any form of gambling/drinking intoxicating beverages and the like while on duty or off duty within the practicum site premises.

Section 3 - Attendance 3.1 The practicumer should observe punctuality and regular attendance in reporting to his/her assigned duty. S/he should religiously accomplish the attendance sheet noted by the on-site supervisor. The required number of hours of practicum must be completed within the time frame set by the department.

Section 5 - Grading System 5.1 The basis for the computation of grade is as follows:

5.1.1 The practicum supervisor will give the 40% of the grade. 5.1.2 The practicum/training site supervisor will give the 60% of the grade (Practicum Performance Evaluation). Section 6 - Criteria for Best Practicumer 6 6.1 At the end of each semester, the committee on Honors and Awards will select the Best Practicum Student based on the following criteria: 6.1.1 6.1.2 6.1.3 6.1.4 S/he should not have violated any rules and regulations set forth by the University and the institution/company. S/he must have garnered the highest possible practicum grade. S/he must be highly recommended by the on-site supervisor, practicum supervisor and the chair of the Department after deliberation. The committee on Honors and Awards shall be composed of the dean, department chair, practicum supervisor and a faculty representative.

Section 7 - Student Discipline Board 7.1 Composition of the Board 7.1.1 Dean/Chair 7.1.2 Practicum Supervisor 7.1.3 On-site Supervisor 7.2 Jurisdiction The Board shall have the entire jurisdiction over all cases involving the behavior of the practicumers.

PART II THE PRACTICUM SUPERVISOR Section 1 General Provisions 1.1 1.2 The practicum supervisor shall not receive additional remuneration and/or deloading from the institution. The practicum load will serve as his/her teaching load. S/he shall be allowed to reimburse the expenses incurred during his/her practicum visit such as meals, transportation, etc.

S/he should present to the chair the form/certificate of appearance duly signed by the authorized representative of the institution/company on a weekly basis. Professional conduct should be properly observed by the practicum supervisor as stipulated in the faculty manual.

Conducts Practicum Orientation and evaluation(s) for incoming students. Maintains data bank of current sites, students file containing proper documents e.g. contract, evaluation, letters, waiver, etc. Sends letters/tokens of appreciation, if necessary, to forge closer ties with the affiliating institutions/ companies. Plans and schedules the students time in the institution/company. Convenes practicum focus discussion with the institutions organization partners to facilitate constructive feedback for the improvement of the program. Oversees all administrative responsibilities associated with the practicum in terms of preparation of MOA, endorsement letter, evaluation, grades, etc Communicates and regularly visits the institution/company for purposes of administration, assistance, monitoring and evaluation of students performance. Serves, if and when necessary, as a mediator between the participating institution and student. Meets with practicum students during the semester, in a group setting, for the purpose of bringing them together to share their experiences and to offer support. Submits all necessary documents to the department for completion of reports and other evaluations.
