Admission

Pre-Admission Except for emergency cases, admission to the hospital is by doctors' referrals during your outpatient consultation. For your convenience, our staff at your Specialist Outpatient Clinic/Centre will arrange for your admission, appointment date, financial counselling and advise on your estimated cost of hospitalization and mode of payment as well as all other necessary admission preparations and information. Surgery Cases If you are scheduled for a surgery or procedure, you may need to undergo pre-admission investigations to ensure your fitness for surgery. This includes obtaining your clinical history and conducting basic investigations such as blood tests or x-rays. Please inform your doctor if you have any pre-existing conditions such as high blood pressure, diabetes, asthma or drug allergies. Admission On the day of your admission, please register at : Class Wards Registration Location Main Building 1, Patient Service Centre 4, 'B1', 'B2' and 'C' Class wards 5 or 6 Deluxe Suites and 'A' Class Kent Ridge Wing 2, Patient Service Centre wards 3 Our Patient Service personnel will contact you on the eve or on the morning of your admission should there be a need to offer you an alternative choice of accommodation or to re-schedule your admission due to bed availability. We would appreciate if you can provide us the most appropriate contact number to contact you. You may also wish to confirm your bed availability status by calling the respective Patient Service Centre at the telephone number listed in your Admission Form. Should you need to cancel or postpone your admission appointment, kindly contact the staff at the respective Specialist Outpatient Clinic/Centre by calling the telephone number listed in your Admission Form. Documents to Bring Please bring along the following original documents for your admission registration :

Admission and Financial Counselling Form Permanent Resident documents/NRIC/Birth Certificate (for patient under 15 years old) Valid notification letter from work permit department (for temporary work permit holders) Passport/Employment Pass/Work Permit/Department Pass (for non-Singapore citizens) Insurance card/Employer's Guarantee Letter (if you are settling your bills through insurance or employer's coverage)

Medical Benefits documents, eg. Civil Service Card (CSC), Hospitalisation Identity Card (HIC) or Blood Donor Card.

We request that you do not bring jewellery or other valuables or excess cash when you are admitted to the hospital. We will not be responsible for any loss of valuables or cash kept in the ward.
