Clinical Rotation Checklist

Rotation: OR/ Surgery Daily Grade:

The supervisor for this unit is Marry Carroll an R.N. at supervises the OR and PACU. 2. Who is the contact person(s) for this unit. Include full name, professional initials and title? The contact person for this unit is Lisa Clissman an R.N. who is the board runner for the OR. 3. What is the procedure if you are going to miss a clinical day? If a clinical day is to be missed due to sickness or an emergency, call Lisa Clissman the contact person of the OR and call Mrs. Sittre on her cell to report that you are not able to come class that day. Then a clinical paper must be written and turned in by the next class meeting. If a clinical day is missed due to a tardy, call Lisa Clissman and Mrs. Sittre to inform them of your absence of that day. Then go to the library and write the clinical paper there and turn in at the end of clinical class. 4. Give the names and numbers of the persons you MUST call. Lisa Clissman- tgb

5. Give the phone number to call within your hospital for emergencies.

6. Describe the different color codes for your hospital. The color codes for Baptist hospital are as followed. Code red is for a fire in the hospital, code blue is used when a patient/ person has stopped breathing, their heart has stopped beating or both. Code yellow is used when there is an uncontrolled individual or a threatening situation to the facility. Code green is used for when there is an uncontrolled patient. ( When patient goes wild)

7. Specifically state the location of fire extinguishers in this area. In each wing (OR hallway) there are 2 fire extinguisher at each end; one at the end of each wing ( hallway) near the fire exit and one towards the middle of the hallway. In the Surgery waiting room there is one fire extinguisher. Near the elevator of A wing floor 6, there is a fire extinguisher across the hall and to right next to a fire exit. In the hallway to OR 16-20 ( C Wing) there is one fire extinguisher near the elevators of C wing. ( When the C wing elevators are on the left, then the fire extinguisher is located to the right near an emergency stairwell.) Also there is a fire extinguisher right at the front office of the OR. 8. Where it the nearest exit in case of a fire? Indicate stairwell and hospital exit. In case of a fire, there are 6 stairwells located in the OR. The first one is located at the of the surgery waiting room, the second one right across the hall from A wing elevators, and the third one is located at the end of B wing (Hallway for OR 4,5,6,10,11,12). The fourth one is located at the end of D wing (Hallway of OR 1,2,3,4,8,9), the fifth one is located across the hall from the elevators of C wing (

Hallway going to Cath. Lab and OR 16-20), and finally the sixth one is located at the end of hallway of OR 16-20. 9. Give on example of how body mechanics is used in your area to prevent back/body injury. One example of how body mechanics is used in the OR to prevent back/ body injury is by maintaining a broad base to lift and getting help when transferring patients from bed to surgery table. 10. Where is the MSDS notebook in your area? The MSDS notebook is located on the computer in the OR.
