Lets take Julia for example. This year, she has to leave her parents residence because she will be attending a university located in another region. She filled out her application for financial assistance and reported earnings of $4 000. Her parental contribution has been established at $5 068.

A Simplified Approach

The earlier, the better!


Aide financire aux tudes receives many applications at the beginning of the academic year. That results in processing delays that you can avoid by filling out the application as soon as it is available. An application complete with all the required documents that is sent in May or June could enable you to access your financial assistance as soon as your studies begin. Thats why the earlier you act, the better. an e-mail address, Aide financire aux tudes will send you a message informing you where to find your Financial Assistance Statement on the Web site. If you did not provide an e-mail address, you will receive your statement by mail. If you are filing your first application for financial assistance, you must pick up your Guarantee Certificate from the financial assistance office of your educational institution. This document will be given to you at the beginning of your first study period. You should submit your certificate to your financial institution as soon as possible so that the financial assistance awarded to you may be deposited directly into your bank account. If this is not the first time you file an application for financial assistance, you are not required to pick up a new Guarantee Certificate, since certificates are issued only once, at the beginning of your studies. Once the amount of assistance to which you are entitled has been assessed, the installments will be deposited directly into your account. However, if you interrupt your studies for a period exceeding six months, you will require a new certificate that you can obtain from the financial assistance office of your educational institution. In addition, you must fill out a Confirmation of Financial Resources form in September and January in order to update or confirm the income you reported in your application for financial assistance. If you fail to do so, payment of your assistance will be suspended. The Confirmation of Financial Resources form is available on Web site of Aide financire aux tudes under On-Line Access to Your File!

How to Apply for Financial Assistance


You can apply for financial assistance by filling out the print form provided in this Guide or available through the financial assistance office of your educational institution. You can also fill out the on-line application form, which is available on the Web site of Aide financire aux tudes at www.afe.gouv.qc.ca. Over 95% of our clients use our on-line services. Its a fast, easy and secure way of applying for financial assistance. If this is not your first application for financial assistance, you can use the personalized form located on Aide financire aux tudess Web site under On-Line Access to Your File! In order to facilitate the application procedure, Aide financire aux tudes has already filled out part of the form using the information in your file. For an estimate of the financial assistance that could be awarded to you under the Loans and Bursaries Program, you can use the assessment simulator, which is available on our Web site. The assessment results are available within minutes. Please note, however, that the result is an approximation and should not be considered official.

How to Proceed Once Your Form Is Sent


The financial assistance to which you may be entitled will be assessed upon receipt of your form and required documents. A Financial Assistance Statement will be generated, informing you of the total amount of financial assistance (loan and bursary, if any) to which you are entitled. Your Application for Financial Assistance

Under certain circumstances, you could qualify for the Loans and Bursaries Program even though you are studying part-time. To find out if you may be deemed a full-time student, consult page 7 of the Guide.

What You Need to Know Before You Begin


Eligibility requirements
You qualify for the Loans and Bursaries Program provided you meet the following requirements: You are a Canadian citizen, or a permanent resident, refugee or protected person within the meaning of the Immigration and Refugee Protection Act. You are residing in Qubec or are deemed to be residing in Qubec at the time of your application, in accordance with the situations in the Criteria for Qubec resident status section on page 5 of the Guide. You have been admitted to an educational institution recog nized by the Ministre de lducation, du Loisir et du Sport for the purposes of awarding financial assistance and you are pursuing full-time studies in a recognized program. or you are deemed to be pursuing full-time studies in a recognized program. You have not exceeded the maximum number of months of study for which financial assistance may be granted for your level of education, type of degree or program. You have not reached the debt limit established for your level of education, type of degree or program. You do not have sufficient financial resources to pursue your studies. Note that people who are serving a prison sentence are not eligible for the Loans and Bursaries Program. 2. Make sure you provide all the required documents indicated by the circled numbers on the forms. These numbers refer to specific documents listed on pages 14 to 20 of the Guide. 3. Have your father, mother, sponsor or spouse fill out the appropriate declaration form(s). Pages 21 to 22 of the Guide are intended for your parents or sponsor. Pages 23 and 24 concern your spouse. 4. Gather all the documents you need to send: Signed forms Schedule A, if applicable Supporting documents (Your permanent code must appear on each document.)

Important!
Do not fax forms or documents required to process your application. You have up to 60 days following the end of your last month of recognized studies of the award year in question to submit your application for financial assistance.

Financial assistance
Amount to which you are entitled Once your application for financial assistance has been processed, Aide financire aux tudes will send you a Financial Assistance Statement. The statement will tell you, among other things, the amount of financial assistance to which you are entitled and the breakdown of your installments. If, when completing your application for financial assistance, you indicated that you wish to receive your correspondence by e-mail, you will receive an e-mail asking you to check your Financial Assistance Statement on Web site of Aide financire aux tudes at www.afe.gouv.qc.ca under On-Line Access to Your File!

How to proceed
To find out if you qualify for financial assistance and the amount to which you may be entitled, you must proceed as follows: 1. Fill out all sections of the 2011-2012 Application for Financial Assistance and Schedule A, if applicable. For help in doing so, consult pages 4 to 13 of the Guide.

Payment of your financial assistance


If you are filing an application for financial assistance for the first time If you recently filed your first application for financial assistance and it was approved, you must go to the financial assistance office of your educational institution several days before your first study period begins. The personnel there will provide you with an envelope that contains, among other things, your Guarantee Certificate. You must submit that envelope to your financial institution as soon as possible so that it can send your banking information to Aide financire aux tudes. Subsequently, the financial assistance to which you are entitled will be deposited directly into your bank account. If you are not filing an application for financial assistance for the first time Once you have sent your application, you will not have any other procedures to follow. If your application is approved, your assistance will be deposited directly into your bank account on the dates indicated in your Financial Assistance Statement. However, if you interrupted your studies for more than six months since you last filed an application, you must follow the procedures for filing an application for the first time.

Changes occurring during the year


Your financial resources In September and January, you must confirm or update the income you reported in your application for financial assis tance by filling out a Confirmation of Financial Resources form. You must fill out this form even if you have no changes to report with regard to your income. If you fail to do so, payment of the financial assistance to which you are entitled will be suspended. Please note that the amount of assistance awarded to you for the entire year will be reassessed at the end of the school year on the basis of information received from Revenu Qubec.

Your circumstances
Over the course of the year, you must inform Aide financire aux tudes of any change with regard to the information you provided in your application. You can change your mailing address in the section Your information of your on-line student file. To access your file, go to our Web site and click on On-Line Access to Your File. To report any other changes, you must fill out a Declaration of Change form, which is also available in your on-line student file under Full-time forms. You can also obtain the form from the financial assistance office of your educational institution.

Personal information is confidential


The Act respecting access to documents held by public bodies and the protection of personal information requires public bodies to collect only the personal information they need to perform their duties or administer the programs under their jurisdiction. Such information and the relevant supporting documents are essential for the administration of the Act respecting financial assistance for education expenses and the related Regulation. The information received will only be used for the purposes of awarding student financial assistance and, when necessary, for recovering financial assistance overpayments or government-guaranteed loans, in cases where recipients fail to honour their repayment terms and conditions. Who has access to your personal information? Only the authorized Aide financire aux tudes staff and the personnel responsible for financial assistance at educational institutions may have access to the information in such files. Which organizations are called upon to verify personal information? In its efforts to verify the accuracy of the information provided or to recover outstanding amounts, in accordance with the Act, Aide financire aux tudes may contact the following bodies: The Ministre de lducation, du Loisir et du Sport: to verify information concerning students school records The Ministre de lEmploi et de la Solidarit sociale: to identify persons who benefit under the Social Assistance Program or the Social Solidarity Program and grant them extensions for the repayment of a bursary overpayment or a loan granted under the Loans and Bursaries Program Revenu Qubec: to verify information provided, such as a persons address, civil status, employers, incomes, or to recover from income tax refunds any amount owed by students who have not honoured their repayment agreement A credit-reporting agency: to trace the place of residence of certain persons who cannot be located and check certain items of information Educational institutions: in order to confirm educational information Financial institutions: to update the files of persons who have benefited under a program offered by Aide financire aux tudes and ensure a follow-up on their loan balance Rgie de lassurance maladie: to determine the place of residence of persons who cannot be located Debt collection agencies outside Qubec: to obtain debt repayment from a debtor or conclude a debt repayment agreement

Your Form

Important!
Some educational institutions assign each student a number they call a permanent code or student number. This code or number should not be confused with the permanent code assigned by the Ministre de lducation, du Loisir et du Sport. If you dont have a permanent code If you were born in Canada, Aide financire aux tudes will have a permanent code assigned to you if you send an original birth certificate (or a copy that is certified true by a commissioner for oaths) that indicates the last and first names of your father and mother, as specified in the left-hand column of this page. If you were not born in Canada, Aide financire aux tudes can have a permanent code assigned to you if you send, in addition to the documents specified in the left-hand column of this page, a legible document, such as your birth certificate, indicating your place of birth (city and country) and the last and first names of both your parents, even if they are deceased. Only original documents or copies certified as true before a commissioner of oaths will be accepted.

Section 1: Students Personal Information


A and B. Identification and citizenship
Last name and first name at birth as given on your birth certificate
If this is your first application for financial assistance and you are a Canadian citizen by birth If you were born in Qubec and you have a permanent code, you are not required to provide your birth certificate. If you were born in Qubec or another Canadian province but do not have a permanent code, you must provide an original birth certificate (or a copy that is certified true by a commissioner for oaths). It must give the last and first names of your father and mother. An original copy of an act of birth issued by the Directeur de ltat civil is preferable, since it contains all the required information. Certificates issued before January 1, 1994, by an authority other than Directeur de ltat civil have legal standing and will also be accepted. If you are not Canadian by birth If you are a naturalized Canadian citizen, you must provide the documents listed in item 2 of the table on page 15. If you are a permanent resident, you must provide the documents listed in item 3 of the table on page 15. If you are a refugee or a protected person, you must provide the documents listed in item 4 of the table on page 15. If you do not have a permanent code, copies of those documents must be certified true by a commissioner for oaths.

Social Insurance Number


If you dont have a Social Insurance Number Go to the Service Canada Centre in your area to apply for a Social Insurance Number (SIN). You will receive your number the same day and your card in the days that follow. To find out which documents are required to apply for an SIN, see the related section on Service Canadas Web site (www.servicecanada.gc.ca) or dial 1-800-808-6352 and select option 3. If you apply by mail, you will receive your SIN within 3 to 4 weeks. In that case, dont wait to receive your SIN before applying. You can inform us of your SIN as soon as you receive it.

Permanent code assigned by the Ministre de lducation, du Loisir et du Sport


Example of a permanent code: TREP11596701 Your permanent code appears on your secondary school or college transcript and on all correspondence sent by Aide financire aux tudes. You must write it on all documents that you send to Aide financire aux tudes. If you fail to do so or if you provide an incorrect code, your application will not be processed.

C. Mailing address
Full address and telephone number
Remember to enter your postal code and telephone number. If you change mailing address over the course of the year, you must quickly inform Aide financire aux tudes. You can change your mailing address in the section Your information of your on-line student file. To access your file, go to our Web site and click on On-Line Access to Your File.

E-mail address
You may choose to receive your correspondence by e-mail. To do so, just enter your e-mail address in the space provided for that purpose on the application form. Its a fast and easy way to stay informed of the status of your file! In most cases, instead of receiving an Aide financire aux tudes notice in the mail, you will receive an e-mail message asking you to consult your file on our Web site. If you choose this option, it is important that you notify Aide financire aux tudes of any change in your e-mail address.

B. Studying outside Qubec


You must complete subsection B if you are or were pursuing studies outside Qubec, but were not enrolled in a Qubec institution.* See the table below for the applicable criterion and enter the number in the space provided on the form. Studying outside Qubec 11. You are studying outside Qubec, you live in Qubec and you are in one of the ten situations described in the preceding table. In addition, you travel morning and evening to and from your educational institution. 12. You are studying and living outside Qubec, and your parents (or your sponsor) reside in Qubec. Before your departure, you were in one of the situations listed in the preceding table. 13. You are studying and living outside Qubec. Your parents (or your sponsor) no longer reside in Qubec, but left the province less than five years ago and resided in Qubec before their departure. Before your departure, you were in one of the situations listed in the preceding table. 14. You are studying and living outside Qubec. You have not interrupted your full-time studies for more than 12 consecutive months since the date of your departure, before which you were in one of the situations listed in the preceding table. 15. You are studying and living outside Qubec. Your spouse has not worked outside Qubec for more than 12 consecutive months since the date of your departure, before which you were in one of the situations listed in the preceding table. Prior to leaving to study outside Qubec, remember to sign a power of attorney that will enable a member of your family or a friend to remit your Guarantee Certificate to your financial institution.

Section 2: Residence in Qubec


A. Criteria for Qubec resident status
Even if you answer NO to the first 3 questions of this section, Aide financire aux tudes may still consider you a Qubec resident. Read criteria 4 to 10 in the following table. If you meet one of those criteria, enter the number corresponding to that criterion under question 4 of the form. If you answered 10, remember to indicate the criterion that applies to your spouse. Criteria for Qubec resident status 1. You were born in Qubec. 2. One of your parents or your sponsor resides in Qubec. 3. You hold a Certificat de slection du Qubec (Qubec selection certificate). 4. Both your parents (or your sponsor) are deceased and one of them resided in Qubec at the time of his or her death. 5. You still have your residence in Qubec, although your parents (or your sponsor) now live elsewhere. 6. Qubec is the last place where you resided for 12 consecutive months without pursuing full-time studies. 7. You were adopted by a person who resided in Qubec at the time of your adoption. 8. You have been residing in Qubec for at least three months and have not resided in another Canadian province for more than three months. 9. You have been in one of the following situations for three consecutive years within the last five years: 2, 5, 6 or 8. 10. One of the previously listed criteria (1 to 9) applies to your spouse.

* If you are pursuing studies outside Qubec under an exchange program administered by a Qubec institution, you are not required to fill out this section.

Sections 3A and 3B: Educational Information


Which section should you fill out?
You must fill out section 3A if you are in one of the following situations: You are pursuing studies in a regular path college or university program, e.g. your school year is divided into three terms called study periods in the Guide and the formsstarting and ending on fixed dates. You are enrolled in a TlUniversit or Cgep@distance program. You must fill out section 3B if you are in one of the following situations: You are pursuing studies in a college continuing education program leading to an Attestation of College Studies (ACS). You are pursuing studies in an intensive or accelerated program leading to a Diploma of College Studies (DCS). You are pursuing studies in a secondary school vocational training program leading to a Diploma of Vocational Studies (DVS) or an Attestation of Vocational Specialization (AVS).

General information regarding sections 3A and 3B


Reduction of the student contribution with regard to part-time studies
You may be entitled to a contribution reduction if you pursued part-time studies during the 4-month period preceding the start of your full-time studies in 2011-2012. If you pursued part-time studies in more than one educational institution or program, you must attach a separate sheet of paper to your form with the name and code of each educational institution and the number of course hours (college) or credits (university) in which you were enrolled. Note that if you are pursuing part-time studies, you could be deemed to be pursuing full-time studies and thus qualify under the Loans and Bursaries Program. To find out if you qualify, read the following criteria on full-time studies and studies deemed full-time.

Full-time studies and studies deemed full-time


To qualify for the Loans and Bursaries Program, you must be a full-time student or be deemed a full-time student. You must therefore be in one of the following situations. You are a full-time student if the educational institution you are attending in Qubec con siders you as such. If you have any questions regarding this matter, consult the personnel of the financial assistance office of your educational institution. the educational institution you are attending outside Qubec considers you as such. To find out which documents you need to provide in that situation, contact us or go to our Web site and click on During Your Studies, then on Studies Outside Qubec.

You are deemed a full-time student if you are studying parttime, you are enrolled in at least 20 course hours per month and you meet one of the following four criteria: You are a single parent and you are living with your child who is under age 12 as of September 30, 2011 or you are living with your child who is under age 21 as of September 30, 2011, and your child is a student with a major functional disability or mental disorder.* You have a spouse and you are living with your child who is under age 6 as of September 30, 2011 or you are living with your child who is under age 21 as of September 30, 2011, and your child is a student with a major functional disability or mental disorder.* You are at least 20 weeks pregnant. You have a major functional disability. You are participating in the Russir program of the Ministre de lEmploi et de la Solidarit sociale If you and your spouse are students, only one of you can be deemed to be a full-time student during the award year in question.

Full-time practicum
Regular path college or university program Aide financire aux tudes defines a practicum as a period of practical training in the workplace. You must indicate that you are doing a full-time practicum if you meet one of the following criteria: The practicum is compulsory and its duration corresponds to a study period (e.g. the fall of 2011, the winter of 2012, the summer of 2012). Generally, it lasts between 12 and 16 weeks. In addition, it is declared a full-time practicum by your educational institution and provides credits. If you are pursuing college studies, the practicum is part of a work-study program (WSP). If you are pursuing university studies, the practicum is part of a cooperative program. If you are enrolled in a regular path program and your practicum meets none of the previously listed requirements, check the Full-time studies box. Secondary school vocational training program or college continuing education program You may do a practicum as part of your secondary school vocational training program or your college continuing education program. If the practicum is compulsory and provides credits, be sure to include the duration of your practicum within that of your program. For more information, consult the personnel of the financial assistance office of your educational institution.

Name and code of educational institution and program


The names and codes of educational institutions and programs are listed on pages 29 to 42 of the Guide.

Year of the program


If you are enrolled in one of the following programs, please specify which year of the program you reached or will reach in 2011-2012. If in doubt, contact the financial assistance office of your educational institution.

Details Concerning Section 3A Educational Information Regular Path College or University Program
University education Masters level
Program with or without thesis A thesis is a comprehensive written paper on a given problem. Its author must demonstrate that he or she can make a contribution to a specific field of study. If you must write a thesis at the end of your masters degree program, check one of the boxes under Program with thesis. If, on the other hand, you must submit essays or complete directed studies, check one of the boxes under Program without thesis.

Details Concerning Section 3B Educational Information Secondary School Vocational Training Program or College Continuing Education Program
Start date and end date of your program
Enter the start date and end date of your program, making sure to specify the year. For example, if your program begins on August 13, 2011, and ends on October 5, 2012, you must specify those dates, even if they are not in the same award year. Depending on your circumstances, you must file one or two applications for financial assistance: If your program starts before June 16, 2011, you must fill out one application for financial assistance for 2011-2012 and another one for 2010-2011 to ensure that your financial needs for the summer of 2011 (i.e. from May 1 to August 31, 2011) are taken into account. If your program starts after June 16, 2011, you must fill out only one application for financial assistance for 2011-2012. Your financial needs for the summer of 2011 (i.e. from May 1 to August 31, 2011) will be taken into account. If your program ends before November 1, 2012, you must fill out only one application for financial assistance for 2011-2012. Your financial needs for the fall of 2012 (i.e. from September 1 to December 31, 2012) will be taken into account. If your program ends after October 31, 2012, you must fill out one application for financial assistance for 20112012 and another one for 2012-2013 to ensure that your financial needs for the fall of 2012 (i.e. from September 1 to December 31, 2012) are taken into account. University education Masters and doctorate levels


Writing or deposit of thesis or dissertation Check the Deposit of thesis or Deposit of dissertation box if you meet the following conditions: You finished writing your thesis or dissertation during the previous study period. You will be depositing your thesis or dissertation during the study period in question. This is the only step left for you to complete your program. You have to enroll for the study period in question in order to be able to deposit your thesis or dissertation and you will be considered a full-time student by your educational institution. You will have to pay tuition fees or administrative fees to enroll.

Would you like to obtain financial assistance to purchase computer equipment?


If you qualify under the Loans and Bursaries Program and you have never received financial assistance to purchase computer equipment, you can apply using the Declaration of Change form, which is available on our Web site at www.afe.gouv.qc.ca under the section On-Line Access to Your File! Upon accessing your file, click on the Full-time forms tab. Please consult the section Costs related to the purchase of computer equipment of our Web site for details.

Section 4: Students Situation


The information you provide in this section will enable us to determine which of the following student categories you fall into: Student with a parental or sponsor contribution Student without a parental or sponsor contribution Student with a spousal contribution Aide financire aux tudes considers you a self-supporting student if you fall into one of the last two categories.

B. Major functional disability


If you suffer from a major functional disability, you could benefit from special measures under the Loans and Bursaries Program. A major functional disability is a physical handicap that persistently limits a persons ability to carry out daily activities and limits that persons prospects for study and employment. The following disabilities are recognized: severe hearing impairment, severe visual impairment, motor impairment and organic impairment. The last two disabilities are recognized when they result in significant and persistent limitations. The disabilities recognized are defined in the Medical CertificateMajor Functional Disabilities and Other Recognized Disabilities form, which must be completed by your doctor and attached to your application for financial assistance. If you need special equipment, special services or paratransit services in order to be able to pursue your studies, you may qualify for the Allowance for Special Needs Program.

A. Marital status
De facto spouse
You may be considered a de facto spouse in accordance with one or more statutes. The answers you provide in this subsection and elsewhere in the form will enable us to determine if you are a de facto spouse according to Aide financire aux tudess criteria. If you are cohabiting with a person without being married or in a civil union, and if you were never married or in a civil union, you must indicate that you are single. If you are cohabiting without being married or in a civil union, but were previously married or in a civil union with another person from whom you are now separated, you must indicate that you are either divorced, legally separated or de facto separated, as the case may be. If the person to whom you were married or with whom you were in a civil union is deceased, you must indicate that you are widowed. While the Civil Code of Qubec does not grant de facto spouses legal standing, certain Qubec statutes (e.g. the Taxation Act and the Act respecting financial assistance for education expenses) do recognize de facto spouses. However the criteria for recognition of de facto unions vary from one Act to another.

Aide financire aux tudes will recognize your transportation costs if there is no overland transportation between your home and your educational institution or the organization hosting your practicum, provided they are both located in Qubec. You must send us the Declaration of Change.

C. Self-supporting student (based on family situation)


Biological or adoptive parent
A biological parent is a person who has a child of his or her own (whether the child is alive or deceased). An adoptive parent is a person who has legally adopted a child (whether the child is alive or deceased).

Single parent Civil union spouse


The civil union is a commitment by two persons who consent to live together. The civil union is contracted openly before a competent celebrant, as is marriage, and is witnessed through a civil union act. A single parent is a person who does not have a spouse and who has custody of at least one dependent child (his or her own) living with him or her. A person cannot be a single parent if he or she is not a biological or adoptive parent.

De facto spouse with one or more dependent children


Aide financire aux tudes defines a de facto spouse as a person who is living in a de facto union with a partner (of the opposite sex or the same sex) without being married or in a civil union and who is living with at least one child (either the persons or the partners child).

Legally or de facto separated


De facto separated persons are still married or in a civil union, but they are separated from their spouses, without having obtained a court judgment granting them legal separation. As for legally separated persons, they were once married or in a civil union but have since obtained a court judgment granting them legal separation.

D. Self-supporting student based on studies


Undergraduate degree earned in Qubec
If you obtained an undergraduate degree in Qubec, you must provide the appropriate supporting document (see situation 20, page 18). You must have earned a bachelors degree, not a 1-year certificate or a 2-year diploma.

Are you recognized by Aide financire aux tudes as having a major functional disability and have you been pursuing university studies in or outside Qubec? You need to have earned 45 credits (instead of 90 credits) to be recognized as self-supporting, based on your studies. If that is the case and you meet the criteria above, check box 5 or 6 in section D of the form.

Undergraduate degree earned outside Qubec


If you obtained an undergraduate degree outside Qubec, you must provide the appropriate supporting document (see situation 21, page 18). If you obtained your degree outside Canada, you must provide a copy of the valuation comparative des tudes effectues hors du Qubec (comparative evaluation for studies done outside Qubec), issued by the Ministre de lImmigration et des Communauts culturelles.

E. Self-supporting student based on time spent in the labour market


Independent for 24 months
If you supported yourself for an accumulated total of 24 months without studying full-time, you could be recognized as a selfsupporting student on the basis of the time you spent on the labour market. You are considered independent in the following situations: Situation 1 While living with your parents, sponsor or elsewhere: You held a paid job or You received employment insurance benefits (including maternity and sickness benefits) or You received income replacement benefits from the Commission de la sant et de la scurit du travail (CSST), the Socit de lassurance automobile du Qubec (SAAQ) or another organization Situation 2 While not living with your parents, you supported yourself in a manner that is not mentioned above. For more information on situations in which you could be considered independent, consult the personnel of the financial assistance office of your educational institution. Examples Situation 1 You held a paid job for nine months, received employment insurance benefits for six months and then held another paid job for six months. Subsequently, you received income replacement benefits from the CSST for three months. At no time during this entire period were you a full-time student. Situation 2 You received social assistance or social solidarity benefits for 24 months while not living with your parents. In this case, you will be required to provide a copy of your lease(s) covering this 24-month period.

Diploma from a Qubec music or drama conservatory


If you obtained a diploma from a Qubec music or drama conservatory, you must provide the appropriate supporting document (see situation 22, page 18). You must hold a Level I graduate diploma in music (Diplme dtudes suprieures I en musique) or an attestation issued upon completion of three years of training, not a certificate of secondary school or college studies.

90 credits in a single university program in Qubec


You have earned 90 credits in a single university program in Qubec if you meet the following two requirements: Minimum of three years of fulltime university studies (at least one period of full-time studies is required to establish a year of full-time studies) Minimum of 90 credits obtained in a single program of study (credits from another program are considered only if they were formally transferred into the program comprising the 90 credits)

90 credits in a single university program outside Qubec


You have earned 90 credits in a single university program if you meet one of the following requirements: You hold a Diploma of College Studies (DCS) and have studied at the university level outside Qubec in a single university program for at least three years. You have not obtained a Diploma of College Studies (DCS) and have studied at the university level outside Qubec in a single university program for at least four years.

Important!
A year is counted for each school year during which you pursued full-time studies at the university level, whether for one, two or three study periods (fall, winter, summer).

Income replacement benefits related to a job you have held are paid by the following organizations: Rgie des rentes du Qubec Socit de lassurance automobile du Qubec Commission de la sant et de la scurit du travail (act of good citizenship, occupational injury, occupational illness, crime victim) Human Resources and Skills Development Canada (HRSDC) Service Canada (benefits from the Canada Pension Plan)

Situations 1 and 2 combined You received social assistance or social solidarity benefits for eight months, then pursued full-time university studies. After one session, you withdrew from your studies and held a paid job for at least 16 months. You must provide the required documents related to your situation in item 25 on page 19.

Living in a halfway house in 2011-2012


People who are serving a prison sentence are not eligible for the Loans and Bursaries Program. Only those who are living or who will be living in a halfway house in 2011-2012 can file an application for financial assistance.

Deteriorated family situation


Check the box corresponding to this situation if your family situation has deteriorated so much that you had to leave home. If this is the case, you must provide either or both of the 2011-2012 Declaration of Father or the 2011-2012 Declaration of Mother forms. If you had to cease communication with one of your parents because of a serious problem (such as alcoholism, incest, violence or a disagreement), you must provide the form of the parent with whom you last resided and still communicate (i.e. the 2011-2012 Declaration of Father form or the 2011-2012 Declaration of Mother form). If in doubt as to which form(s) you need to provide, please consult the personnel of the financial assistance office of your educational institution.

Cessation of studies for seven years


If you have ceased full-time studies for at least seven years, whether consecutive or not, you must provide your transcripts, as specified in situation 26 on page 19. Such periods of cessation of study can only be considered as of the date on which you were no longer legally required to attend school (in Qubec, the legal requirement to attend school applies until age 16).

F. Exceptional family situation


If none of the statements in subsections C, D and E apply to you, and if your situation with respect to your parents, sponsor or spouse is exceptional, we could take this into account when processing your application. In certain cases, the parental, sponsors or spousal contribution will not be required. To confirm such circumstances, you must include the Declaration of Exceptional Family Situation form, duly completed by an authorized person, with your application for financial assistance.

G. Student with a parental contribution


If you need to fill out this subsection, make sure that your father or your mother, or both (i.e. depending on your situation), send Aide financire aux tudes their declaration form.

Parents or sponsor living in a special care home


Check the box corresponding to this situation if both your father and your mother are living in a special care home or if your sponsor is living in a special care home. If your parents are in a special care home, you must send a 2011-2012 Declaration of Father form and a 2011-2012 Declaration of Mother form. If only one of your parents is living in a special care home, you must fill out subsection G instead. If your sponsor is in a special care home, you must send a 2011-2012 Declaration of Father form in the case of a man or a 2011-2012 Declaration of Mother form in the case of a woman.

Permanent resident, naturalized Canadian citizen, refugee or protected person


If you have a sponsor and both your parents reside outside Canada at the beginning of the award year, you must have the 2011-2012 Declaration of Father form or the 2011-2012 Declaration of Mother form filled out by your sponsor. If you do not have a sponsor, check the appropriate box in subsection F.

Important!
The financial assistance to which you may be entitled will not be assessed until Aide financire aux tudes has received the requested declaration form(s).

Parents or sponsor cannot be located


Check the box corresponding to this situation if both your father and your mother cannot be located. If only one of them cannot be located, you must fill out subsection G instead.

Section 5: Other Financial Support Programs


Emploi-Qubec manpower training measure
You are not eligible for the Loans and Bursaries Program if you are participating in a manpower training measure offered by Emploi-Qubec and if you are attending an educational institution under the authority of a school board or a public college. You may, however, qualify for financial assistance under the Loans and Bursaries Program if you are participating in an Emploi-Qubec manpower training measure and you are in one of the following situations: You are enrolled in a program offered at a private secondary school or college. You are enrolled in a program offered at a university. You are enrolled in a selffinanced program offered at a CEGEP. Please note that your educational expenses will be the only expenses recognized. Moreover, if your educational expenses are paid in full by Emploi-Qubec, you will not be awarded any financial assistance.

Income related to an election


You must report the income you earned as: an election official or a candidate representative (provided you were appointed by proxy) This includes income earned on the advanced polling day, the polling day and while participating in related training sessions.

Employment insurance benefits


Employment insurance benefits are paid by Human Resources and Skills Development Canada (HRSDC). Until 1997, these benefits were known as unemployment insurance benefits.

Important!
At the end of the school year, the income you report is systematically checked against the information held on file by Revenu Qubec. Under paragraph (o) of section 69.1 of the Act respecting the Ministre du Revenu, Aide financire aux tudes does not need the persons consent in order to run such checks.

Section 6: Financial Resources


A. Employment income and income considered employment income
You must report the income you have received or expect to receive between January 1 and December 31, 2011. If you cannot provide the exact amount, give as accurate an estimate as possible. On several occasions during the school year, you will have the opportunity to confirm or update the income you reported.

B. Other income
Orphans pension, pension for a disabled persons child, surviving spouses pension, death benefits in the form of pensions and pensions paid to the child of a crime victim
Indicate only the amount that you have received or will receive starting at age 18 since, with few exceptions, the amount awarded to you prior to age 18 is generally paid directly to the person who has responsibility for you. You are therefore not required to report it.

Important!
If you fail to report an amount, you risk having to repay all or part of the financial assistance you received. The installments expected for the current award year or a subsequent year may serve to reimburse the amount owed.

Alimony and child support payments


You must enter the total alimony and child support payments you will receive for yourself and, if applicable, your children. You must report the actual amount you will receive, taxable or not, even if it is not the same as the amount indicated in the court judgment. Do not report child support payments received by either of your parents to provide for your needs. See the list of amounts you must not report on the following page.

Gross employment income


Specify the gross income from full-time or part-time employment, including severance pay, tips and income received during training.

Scholarships or bursaries
You must enter the total amount of any scholarships or bursaries you will receive in 2011 from an association, foundation, firm or government agency, even if they total less than $5000. You must exclude bursaries received under the Loans and Bursaries Program and the Explore Program.

Income replacement benefits


Specify income replacement benefits paid in accordance with the Act respecting industrial accidents and occupational diseases or any similar provincial or federal law, the Automobile Insurance Act or the Crime Victims Compensation Act.

You must however report bursaries awarded by Aide financire aux tudes to elected executive officers of provincial student associations.

C. Employment prior to start of full-time studies


If you were employed by more than one employer or selfemployed with more than one client while living elsewhere than with your parents, indicate the name of one employer or client only and your workplace address.

Do not report the following income:


Amounts paid by the Rgie des rentes du Qubec (RRQ) as part of its child assistance measure (provincial family allowances) Canada Child Tax Benefit Universal Child Care Benefit Social assistance or social solidarity benefits Amounts paid under a registered education savings plan (RESP) Amounts received from the Socit de lassurance automobile du Qubec (SAAQ) or the Commission de la sant et de la scurit du travail (CSST) as compensation, for example, for the loss of a limb or to reimburse certain expenses (physiotherapy, orthopedic devices, etc.) Withdrawals from a pension plan (RRSP, RRSF, RREGOP, etc.) Benefits from a strike fund Income from rental units Lottery winnings

Section 7: Signature
Your application for financial assistance will be processed only if it is signed. If you do not sign your application form, it will be returned to you by mail and the processing of your file will be delayed. The amount of financial assistance awarded to you may be claimed if you make any false statements. Furthermore, if you fail to comply with an audit request within the specified time, your financial assistance for the current year may be cancelled and further assistance denied.

Important!
Each year, Aide financire aux tudes examines files. You should be prepared to provide supporting documents to justify all the information in the application form, Schedule A and supporting or other documents.

It pays to declare your income!


Under the Act respecting financial assistance for education expenses, you must inform us of any change in your circumstances over the course of the year that could influence the amount of assistance awarded to you. Keeping your file up to date with accurate information could help you avoid a major inconvenience, such as the suspension of scheduled installments, the obligation to immediately repay a financial assistance overpayment or the loss of eligibility under the Loans and Bursaries Program and the Loans Program for Part-Time Studies for at least two years.

Do you have to provide a certified copy or a sworn statement?


The following persons are authorized to certify copies as true copies or administer oaths: lawyers, notaries, justices of the peace, mayors, municipal clerks and commissioners of oaths. Some educational institutions and financial institutions appoint members of their staff to act as commissioners of oaths.

You must submit the original of the application for financial assistance, Schedule A and any other Aide financire aux tudes form that you are asked to fill out. As regards other required documents, copies will be accepted in most cases. Please note that you do not have to resubmit documents supplied with a previous application if the information contained in these documents is still up-to-date. All forms mentioned under Required Documents are produced by Aide financire aux tudes. They are available from the student financial assistance office of your educational institution and on our Web site (www.afe.gouv.qc.ca). You must submit this form with your application each year if your spouse, mother, father or sponsor cannot be located. In all other cases, you are not required to submit this form again if you have already submitted it and if your situation has not changed. The 2011-2012 Declaration of Father and 2011-2012 Declaration of Mother forms are required in the following cases: your sponsor or your parents are living in a special care home or your family situation has deteriorated. In the latter case, if you have ceased all communication with one of your parents, you must provide the form of the parent with whom you last resided and still communicate. The 2011-2012 Declaration of Father and 2011-2012 Declaration of Mother forms may be required if you are living in a halfway house. Contact the person in charge of the financial assistance office of your educational institution to find out if you must submit these forms.

What You Need to Know Before You Begin


Who must fill out a declaration?
You must fill out the 2011-2012 Declaration of Father form or the 2011-2012 Declaration of Mother form if you are the parent or sponsor of a student and must contribute to his or her education. A parental or sponsors contribution is required if the student has not selected any of the situations presented in subsections C to F of section 4 of the 2011-2012 Application for Financial Assistance form. Subsection G of section 4 tells the student whether or not you must fill out a form. If, for example, you are no longer living with the students other parent, only the parent with whom the student resides or last resided before beginning his or her studies must complete a declaration form.

The sponsor who has a spouse must send us a duly-completed declaration of change form in order to inform us of his or her marital status. An additional exemption will therefore be subtracted from the sponsors income during the assessment of his or her contribution.

Before sending your declaration


Before sending your declaration form, you must make sure that you have signed it and that you have attached the required supporting documents. You may send your declaration form along with your childs application for financial assistance in the same envelope. Make sure that the required documents are attached to the corresponding forms. Copies of the declaration forms are available on Web site of Aide financire aux tudes or from educational institutions.

Changes occurring during the year


The 2011-2012 Declaration of Father and the 2011-2012 Declaration of Mother forms must be completed on the basis of information that is accurate and up-to-date on the date of signature. You must notify Aide financire aux tudes of any changes that occur during the year with respect to the information you provided in your declaration form. These changes could affect the amount of assistance awarded to the student. You can use the Declaration of Change form to inform Aide financire aux tudes of these changes. Simply go to our Web site and click On-Line Access to Your File. You can also notify us by sending a letter. Remember to specify the students permanent code. Important!
Only one declaration form per parent is required, regardless of the number of dependent children applying for financial assistance.

Which declaration must you fill out?


To ensure that the students application for financial assistance is processed, you must send us a 2011-2012 Declaration of Father form if you are the father or sponsor, or a 2011-2012 Declaration of Mother form if you are the mother or sponsor. You will find these forms in the middle of this Guide.

The sponsor
The sponsor is the person who agreed to provide for the basic needs of the student when he or she obtained status as a permanent resident, refugee or protected person. He or she is not a guardian, but rather a sponsor within the meaning of the Immigration and Refugee Protection Act.

What is your responsibility with regard to your childs student loan debt?
Parents are not liable for their childs student loans. The Act respecting financial assistance for education expenses states that the person applying for financial assistance under the Loans and Bursaries Program is considered to have reached majority, even if he or she is not 18 years of age Should the applicant die while he or she is a full-time student, the government will pay off the balance of the student loans. No amount will be claimed from the succession. However, if the applicant has completed his or her full-time studies at the time of death, his or her legal heirs will be responsible for the applicants student debts, up to the value of the property of the succession, provided they accept the succession.

Dependent child aged 18 or over who is studying full-time


If you have at least one dependent child aged 18 or over who is studying full-time at the elementary or secondary level (general education), you must provide one of the following documents: The Confirmation of Enrollment form, which is available on Web site of Aide financire aux tudes. Proof of enrollment provided by the educational institution

This is the amount entered on line 199 of your 2010 income tax return. However, if your spouse has transferred some retirement income to you (line 123 of your income tax return), you must subtract that income from line 199 and enter the new amount.

Section 2: Dependent Children


In this section, enter the names of the students who are filing an application for financial assistance and your other dependent children. For a child to be considered your dependent, you must be his or her biological or adoptive father or mother. Enter the names of children who are preschoolers, who are single and have no children of their own and, in the case of daughters, who are not at least 20 weeks pregnant. Do not enter the names of children who are age 18 or over who are not pursuing full-time studies. If you are the sponsor of one or more children who have submitted an application for financial assistance (see definition of sponsor on previous page), you must also enter their name.
