    Clara County Department of Correction ATTN: Carmen 885 North San Pedro Avenue San Jose, California 95110 To Carmen, Santa Clara County Department of Corrections: Per the request of the Office of the Independent Police Auditor, I am hereby requesting a copy of my inmate medical records. I was incarcerated from December 13th, 2008, to until August 3rd, 2011. My PFN is DWF967. The particular portion of the medical records requested is notes pertaining to the evaluation conducted by Dr. Cazmo J. Lukrich, M.D., in March 2009, which describe the injuries sustained to my rib and shoulder.
