Script Alabama Quality Award Conference December 8, 2011

Alabama Quality Award Logo is on the Screen Dr. Barry Mason will welcome the audience to the University and introduce you as the MC. Note to Charlie: As you begin your part on the program you can interject comments, etc. to set the tone for the morning if you want to. I think that at some point we are going to have to acknowledge the fact that due to the uncertainty of the economy at this time attendance at the conference this year has really been impacted but we can all still learn from the best practices that will be shared by the 2011 Alabama Quality Award recipients that will be on the agenda today. Your slides will be immediately after the AQA logo Charlie Blass begins here: Good Morning Welcome to the annual Alabama Quality Award presentation.

If you want to add anything here about yourself that may not have been covered in introducing you, you may do so. Before we begin our program this morning I would like to take care of a few housekeeping chores. Restrooms are located in the lobby area to the right and left of the registration table. Lunch will be served at noon in the Governors Room of the Hotel Capstone located adjacent to the Conference Center. Our guest speaker today will be We will have a morning break at approximately 9:40 a.m. In order to facilitate the panel discussion at the conclusion of the presentations today, you will find QUESTION CARDS on the table in front of you. You may write your questions on these cards, and we will take them up at the conclusion of the morning presentations right before lunch and then again after the Team Showcase presentations that will follow lunch. I would like to remind the presenters to please try and abide by the time limit that you were given for making your presentations so that we will be able to keep everything on schedule. There is a time keeper located at the table on the front row to the left of the podium that will signal you as you are nearing the end of your allotted time. You will find an evaluation sheet in the inside front pocket of your conference folder; we will ask you to complete this immediately

following the panel discussion at the end of the day. These forms are used to help us as we look for ways to continuously improve the conference. AQA logo is on the screen. To begin our program this morning I would like to thank each of you that have participated in the process this year. Whether your participation was as an applicant or if you volunteered your time as an examiner, you are to be commended for your dedication to performance excellence. As we bring the 2011 cycle to a close we are already making plans for the 2012 award cycle.

Alabama Quality Award Logo is put back on the screen. This is the eleventh year the Award program has had three levels at which an organization could apply depending on where they are in

their quality journey. Also, for organizations that have won the Alabama Quality Award and are not eligible to reapply for three years, there is the Award of Excellence for Continuous Quality Improvement. Logo on Screen will be Alabama Quality Award. With Level 1 Recipients First, we would like to recognize our Level 1 Commitment to Excellence Award recipients. This award category is for those organizations that have made a serious commitment to use and implement performance excellence concepts and principles. We have seven recipients at this level this year. Bring up Canterbury Health Care Facility logo. In the Healthcare category our first recipient is, Canterbury Health Care Facility from Phenix City. Canterbury is a skilled long-term and short-term care nursing facility offering 137 dually certified skilled nursing beds. Presenting for Canterbury today is their administrator, Mary Wahlert and Laura James, the director of nursing. At the end of Canterburys presentation the AQA logo on screen. Bring up the Highlands Medical Center logo. Our other Level One recipient in the healthcare category, Highlands Medical Center in Scottsboro, is not able to be with us this morning but we do want to recognize them for their commitment to the process. Bring up the Sigmatech logo. Our next Level One recipient, Sigmatech, Inc., is from the smallbusiness category. Based in Huntsville, Sigmatech provides technical, management, training and e-learning services to aerospace, defense, and federal clients. Presenting today for Sigmatech is their CEO, Joe Bergantz. At the end of Sigmatechs presentation the AQA logo on screen. Thank you, Joe. Bring up the Standard Roofing logo. Our next Level One recipient is also from the small-business category. Standard Roofing Company, headquartered in Montgomery, is a service contractor specializing in repairs, replacement and maintenance of roofing systems for industrial, institutional and commercial owners. Presenting today for Standard Roofing is their President & CEO, Pete Taylor.

At the end of Standard Roofings presentation the AQA logo on screen. Thank you, Pete. We will now take a short Break (at this point in the program it should be 9:40 if we are on schedule if we are a little ahead of schedule just ask everyone to come back at whatever time would give them a 20-minute break. If by some strange thing we are actually behind then we would need to shorten the break to make sure we get started back no later than 10 a.m. AQA logo stays up on the screen during the break. When we return from the break bring up the URS logo. Welcome back we will continue with our presentations beginning with our final three Level One recipients, starting with URS. URS serves at the U.S. Army Aviation Center of Excellence at Fort Ruckerthe largest helicopter training base in the world and the primary training site for Army Aviation. Presenting for URS today is Chuck Gant, Program Manager of URS. At the end of URSs presentation the AQA logo on screen. Thank you, Chuck.

Bring up the UWA logo. Our next Level One recipient is from the education category. The University West Alabama College of Business supports the Universitys mission of providing opportunities for quality education by providing educational experiences in specialized fields of business and technology. Presenting today for the UWA College of Business is their Dean, Dr. Ken Tucker. At the end of UWAs presentation the AQA logo on screen. Thank you, Dr. Tucker. Bring up the LBWCC logo. Our final Level One recipient is also from the education category. Lurleen B. Wallace Community College is a multi-campus, comprehensive community college offering three associates degree options and long- or short-term program certificates. LBWCC is part of the Alabama Community College System. Presenting today for Lurleen B. Wallace Community College is their President, Dr. Herbert Riedel. At the end of LBWCCs presentation the AQA logo on screen. Thank you, Riedel.

We need a slide to come up now with Level Two: Progress Toward Excellence Award. The Level Two: Progress Toward Excellence Award recognizes organizations that have demonstrated sustained commitment to and application of performance excellence principles and practices. They have made progress in building sound and notable systematic processes that have improved results in key portions of the organization. We have three Level Two recipients this year. Our first recipient is the U.S. Army Cruise Missile Defense Systems Project Office. Bring up CMDSs logo. CMDS is located on Redstone Arsenal and is the Armys principal manager of weaponry and systems to counter the fixed and rotary wing, Unmanned Aerial Systems and cruise missile threat. Presenting for CMDS this morning is David Roden, Director of the CMDS Logistics Directorate. At the end of CMDSs presentation the AQA logo on screen. Thank you, David. Bring up the PIF logo.

Our next Level Two recipient is also from the nonprofit category. The U.S. Armys Prototype Integration Facility, located on and nearby Redstone Arsenal, is a government-owned, governmentoperated one-of-a-kind facility and concept. Presenting today for the Prototype Integration Facility is Larry Daniel. At the end of PIFs presentation the AQA logo on screen. Thank you Larry. We will now take a brief 10-minute break (at this point in the program it should be 11:05 a.m. if we are on schedule. If by some strange thing we are actually behind then we would need to shorten the break to make sure we get started back no later than 11:15 a.m. Welcome back. We will conclude our morning schedule with our final Level Two recipient and our Level Three recipient. Bring up the JAMS logo. Our final Level Two recipient is from the nonprofit category. The Joint Attack Munition Systems Project Office is a subordinate unit of Program Executive Office Missiles & Space. JAMS manages weapon systems that have been used in both Operation Iraqi Freedom and Operation Enduring Freedom. Representing JAMS today is Rachelle Lapperre, Lean Six Sigma Deployment Coordinator.

At the end of JAMSs presentation the AQA logo on screen. Thank you, Rachelle. Need a slide that says Level Three: Alabama Excellence Award. The highest level of recognition is the Level Three: Alabama Excellence Award. This highest level of recognition is presented to an organization that has demonstrated through their practices and achievements sustained performance excellence. This year we will recognize only one Level Three recipient, coming from the manufacturing category. Bring up L.B. Fosters logo. L.B. Foster Coated Products applies corrosion protection and coating services to carbon steel line pipe for use primarily in the oil and gas industry. They are located in Birmingham. Presenting for L.B. Foster today is Patrick Petrino, Plant Manager. At the end of L.B. Fosters Presentation the AQA logo will come up on the screen.

The Alabama Quality Award logo remains on the screen. This concludes our morning presentations. PLEASE LEAVE YOUR QUESTION CARDS ON THE TABLE AND WE WILL PICK THEM UP. We will now adjourn to the Governors Room of the Hotel Capstone for lunch. The Governors Room is located straight through the Hotel Lobby on the left. [Any information regarding lunch speaker?]Following lunch we will reconvene at 1:45 p.m. in this room for the Team Showcase presentations and the question and answer discussion. Adjourn to the Hotel Capstone for the luncheon. There will not be a head table but there will be a table reserved near the podium for the speaker, you, Dr. Mason, etc. Mike Bailey will introduce the speaker. When we return to the Auditorium following lunch for the team showcase Jim Percy will serve as the MC for this segment and introduce the teams and talk about the process since he served as one of the judges.

Need a slide that says Alabama Quality Award Team Showcase. Good afternoon and welcome to the Team Showcase presentations. I am Jim Percy and I have the pleasure of introducing the teams to you this afternoon. This is the 15th year that the Alabama Quality Award has sponsored the team showcase. More than 130 teams have participated in this outstanding process. Each team must have completed an improvement project within the last two years. There are three types of teams: Natural process team Special project teams Kaizen teams which implement Lean principles Teams may represent a variety of organizations: Small business Service Manufacturing Education Health Care Non-profit The three award levels are:

Gold Silver Bronze Some reasons for participating in the award process are: To recognize your employees for their continuous improvement contributions To showcase and gain recognition for your organizations continuous improvement process To learn from other teams Our Bronze Level presentation is from Springhill Memorial Hospital in Mobile. Their special projects team tackled newborn blood sampling, with the goal of having less than 5 percent of their results declared unsatisfactory. Presenting for the team today is Beth Edwards. Springhill logo should be on screen. At the end of the presentation the AQA logo will be on the screen. Thank you, Beth. Our next presentation is from a Silver Level Recipient. Springhill Memorial Hospitals Medication Error Committee addressed the improvement of factors contributing to medication errors, with the goal of decreasing and eliminating those errors. Presenting for the team will be Anthony Beck and Sherri Stroud.

Our next presentation is also a Silver Level Recipient from Springhill Memorial Hospital. Their Hospital 30-Day Readmissions Process Team looked at those hospital readmissions that are considered potentially preventable. Presenting today will be Sharon Barnicle. Springhill logo will be on screen. After presentation ends the AQA logo will come up on the screen. Thank you Sharon. Our next presentation is also a Silver Level Recipient from Springhill Memorial Hospital. Their NIMS Reduction Teams target was to decrease nosocomial infection markers (NIMs),

which would in turn help eliminate healthcare-acquired infections. Presenting will be Beth Beck, Director of Infection Prevention. Springhill logo will be on screen. After presentation ends the AQA logo will come up on the screen. Our next Silver Level Recipient is from Huntsville Hospital. Their Antimicrobial Management Team worked on developing, implementing and evaluating methods to optimize the utilization of antimicrobials. Presenting will be Mickala Thompson, a clinical pharmacy specialist. Huntsville Hospital logo will be on screen. After presentation ends the AQA logo will come up on the screen. AQA Logo stays on screen during the break. At this time we will have a fifteen minute break. Welcome back.

Our next Silver Level presentation is from Springhill Memorial Hospital. Their Postoperative Blood Sugar Management Team worked to design and implement glucose management strategies in postoperative patients. Presenting for the team will be Ann Eubanks. Springhill logo should be on screen. After the presentation the AQA logo will come back on the screen. Thank you Ann. Huntsville Hospital is our next Silver Level Recipient. Their Development and Improvement of a Pain Medication Safety Initiative in a Community Hospital Team looked at reducing excessive initial dosing of intravenous HYDROmorphone through implementation of a dose interchange process. Presenting for them today is Pam Schindler, pharmacy IT specialist. Huntsville Hospital logo should come up on screen. At the end of their presentation the AQA logo will be on the screen. Our final silver-level presentation is from Lockheed Martin. Their AGMS Robotic Coating Team analyzed their production line through value stream mapping and indentifies and implements a robotic system. Presenting for them is Fname Lname. Lockheed Martin logo should be on screen. At the end of presentation the AQA logo will appear on the screen. Our final team showcase presentation will be by the only 2011 gold level recipient, Huntsville Hospital. Huntsville Hospital logo should be on the screen. Their Pharmacist Prospective Review of Medication Orders Team evaluated the feasibility of pharmacist review of medication orders, and how best to provide a process to accomplish this task without disrupting patient care. Presenting for them today is Pamela Schindler, pharmacy IT specialist. At the end of presentation the AQA logo will appear on the screen. Thank you, Pam. This will complete our team show case presentations.

It should now be 4:15 p.m. if we are on schedule. Encourage participants to place any questions they might have of the team showcase presenters on the cards and pass them to the ends of the tables and someone will come by to pick them up. We will take a short 5-minute break to allow time for a representative from each of the participating organizations from the morning presentations and one member of each of the teams to take their place at the designated places at the tables in preparation for the Q&A panel discussion to be facilitated by Dr. David Miller. At the conclusion of the panel discussion Dr. Miller will remind everyone of the reception which begins at 5:30 at the Capstone Hotel followed immediately by the banquet at 6:30. We need you to emcee the banquet. Once we get everyone in the banquet room you will welcome everyone to the award ceremony and then call on _____________ for the invocation. After we have finished with our meal. You will introduce the guest speaker STILL FINALIZING THIS. Once __________ completes his speech he will stay on the stage and be joined by Dr. Miller and we will begin the presentation of the awards.

We will follow the order of presentation of awards as they are listed in the program. You will just call each company up and allow them to say something if they would like after they are presented the awards. Once all of the awards are handed out Dr. Mason thinks that I should be the one that you call on to make the closing remarks.
