CONSULTATIONS AND ADVISORY SERVICES Provided Consultations with patients on the wards, at Fitzsimmons Army Medical C enter, Behavioral Health Clinic, Army Community Services, and various hospital a nd clinics, to determine if the patient have the appropriate after care needed f or recovery of their illness injury. A statistical success rate of 99% of the pa tient's recovery unless they are terminal. Determining biweekly follow ups, or a s needed as stated in the treatment plan. Conduct Assessments of each individua l patient's case to make the necessary referral as needed. Advise the staff of the patient's care and needs to support their treatment plan. Provide numbers to make necessary provisions and accommodations for the patients in need of such a ccessories and equipment for example; wheel chairs, walkers or home modification s to avoid further injury to the patient. CLINICAL SKILLS Listening with your eyes and ears, by paraphrasing, reflection of feelings, show empathy, observing body language of the patient and eye contact to get a throug h mental status. In group therapy make sure that everyone participates, to dete rmine how the patient thought process is and interaction with others at all time s, and if they have benefit from the group exercises. The impact of Resiliency Training o n wellbeing stress management training, will increase the happiness of the patie nt from an excellent class from the provider after the training. STATISTICAL DATA It is extremely imperative to gather statistical data, to promote alternative w ays to view occurrences. Also prepare reports to reflect the results after train ing. Conduct a thorough research of methodologies the training and how effective the training was and report the results to the unit Commander and your supervis or at all times. Provide a quiz of how care providers thought process was prior to the training and results after the training. This will assist with informat ion on how much of an impact training is. Build a rapport to market public rela tions to target population in need of the training, such as flyers, questioners in medical treatment facilities. ACCOMPLISHMENTS I am a certified Swim instructor and water aerobics instructor assisting with patient recovery and learning a new tasks and a successful road to recovery and happiness. Received an award in Iraq for teaching 160 Marines how to swim properly and also decreasing their physical training run time by a minute and a half. l also refe rred individuals to the Battle Fatigue Counseling Center in Qwest Iraq for acute or chronic counseling preventing 3 suicides that were planned in 2008. Senior Instructor served as an Instructor for the Total Army Instructor's Cours e consisting of a two week, 10-12 students a class. A total of 80 hours per mont h. Final examination consisted of a total of two classes developed and taught b y each student with a checklist to determine a pass or fail grade. Small Group f acilitation Course was 1 week 40 hours total of 10-15 students twice a month for a total of 80 hours a month. Which was a of 160 hours of instructing a month f or three years. Senior Community Counselor supervisor implemented programs for the soldiers. Col lected mental status evaluations for all active duty military personnel for 10 c ontingent countries in Egypt 2001-2002 . I interviewed soldiers and their family members when necessary. Provided 2 week Drug and Alcohol education classes mon thly for a total of 6 hours a week, 24 hours a month. Supportive Group counseli ng 2 hours 2 days a week for a total of 8 hours a month. Individual counseling services provided daily and follow up supportive counseling 3 days a week, or a s needed basis. Stress Management provide to the units quarterly or (2) hour st

ress debriefing for units when a death of a soldier occurred. Supervised (2) sol diers and a civilian for staffing the cases. Rehabilitation Counselor assisted patients with in-patient group therapy 3 times a week for a total of 6 hours a week 24 hours a month, for methadone patients and individual sessions, of 80 pat ients a month for follow-up and walk-in's. Taught rehabilitation methods to the veterans, in treatments daily at 1300- 1400hrs totaling 5 hours weekly and 20 hours monthly. Observe the taking the methadone, examining under their tongue t o ensure that the medication is swallowed to prevent distribution among their pe ers. Conducted random urinalysis for the patients on an as needed basis to ensur e that the remain clean for their treatment plan. Provides screening, testing, and psycho diagnostic evaluation for children and adolescents with a variety of problems that can affects their daily functions. Conducted initial screening an d evaluation of children with all levels of abuse, to include sexual, neglect, a nd chronic behavioral problems, uncooperative, providing support counseling for 12 residence with (2) counselors per ward providing education services, on 12 ho urs shift's such as facilitating Tuesday and Wednesday at 1300 hrs to 1400hrs we ekly for a total of 2 hours a week 8 hours a month, Resiliency Education Classes for the tragedy they have experienced, and to chang e their way of thinking giving the children Hope. Supervisor for 5 soldiers and provided them with quarterly counseling. Facilitated Drug and Alcohol awareness classes Tuesdays and Thursday's for a total of two hours a session, totaling 4 h ours a week for the inmates. Also co- facilitated the sex offenders group Monda y's and Wednesday 2 hours sessions for a total of 8 hours a week. charge of reco rds documentation and management of mental health referral. Collected and record ed social and psychological data daily 5 days a week for a total of 160 hrs mont hly. Provided crisis intervention and individual counseling on a as needed basis . Served as an Education Services counselor providing, planning, creating and im plementing a comprehensive training program for the inmate population at Mannhie m Germany Confinement facility. Developed a Drug and Alcohol/Resiliency/Stress M anagement classes 3 hours per class for a total of 27 hours a week with a succes s rate of 95% of the inmate population. Water Aerobics and Aquatic Therapy/Swim Instructor in Iraq, for 63 wounded soldiers for a total of 6 months of 12 months spearheading the beginning of the program. I counseled in conjunction with wat er therapy, counseled them to live a productive life with the capabilities they have. Conducted water therapy daily from 0800 am to 11:00 am Monday and Wednesday's to eliminate stress on joints in conjunction with the stress management program, a nd to regain mobility and strength to the muscles. Also too lift morale for the soldiers referred to us by teaching to have fun aga in to rebuild confidence and esprit de corps in their life.
