Who will you be teaching? (Identify student grade level as well as academic functioning level and specific needs of individual students) I will be teaching 9th and 10th grade health and physical education students. There are 20 students in the class, one of whom is below average reading level for 9th grade. He will need accommodations for this lesson in order to understand the specifics of what he is to do. Who (if anybody) will assist with the presentation of this lesson, and what will their role be? N/A

What is the long range goal(s) that is tied to this lesson? The long range goal of this lesson and unit is for students to be informed enough to make positive decisions towards their overall health and wellness. Students will be able to apply the knowledge they acquire in order to lead a healthy active lifestyle.

What is the specific learning objective(s) for this lesson? Students will list three causes that contribute to an unhealthy lifestyle Students will list three causes that contribute to healthy active lifestyles. Students will show comprehension and applications of this knowledge by creating a 5-10 minute you tube video. Student will evaluate each other using a rubric and listening to the presentations.

What prerequisite skills/knowledge will students need to effectively access and participate in this lesson? Students will need to comprehend what childhood obesity is. What causes this disease, and how to prevent this disease. When within the stages of learning will this lesson be presented? (Is it a learning acquisition lesson, learning fluency lesson, learning maintenance lesson, or learning generalization lesson?) This will be a learning maintenance lesson. The students will call on previous knowledge to complete this lesson. We will learn all the basics of childhood obesity and they will put this you tube video together by using this previous knowledge.

When will this lesson be completed? (Will it be a one-day lesson or a multiple day lesson?) This will be a multiple day lesson Where should this lesson be presented to ensure maximum student access and participation? (computer lab, classroom, science lab) AND what materials will be needed? This lesson will be presented in the computer lab. The students will have prior knowledge, but will need to locate facts on obesity. Also creating a you tube video requires internet access and computer use.

Why are you planning to teach this lesson? Why must this information/skill be presented to the students? I am planning to teach this lesson to reinforce prior knowledge on childhood obesity. I want to make sure the students comprehend previous information. The presentation they give will be a great way for me to evaluate their knowledge on the subject.

How does this lesson relate to the PA Academic Standards? This lesson relates to the PA Academic Standards because the students will gather and apply facts about health relating to personal habits, peers, and media. How does this lesson relate to the previous lesson? How does this lesson relate to future lessons? This lesson relates to previous lesson because it calls on previous knowledge acquired during the glogster and the stop childhood obesity video. This lesson relates to future lessons because will we use the information from the you tube video to break down what the students eat and how they exercise to make a program of what they need to do to maintain a healthy lifestyle.

How will you determine if students have met the lesson objective? (Think assessment) I will determine if the students met the objectives by observation. The presentation with their you tube video will be a great assessment. I will be using a rubric I made to score them on the presentation as well as their content knowledge and application. The students will also be assessing each other using a premade rubric.

How should this lesson be presented to ensure maximum student access and participation? (lecture, whole group activities, small group activities, cooperative learning groups) This lesson will be presented in two ways. 1. Lecture and 2. Small group activities. I will begin with lecture so the students have an understanding of what they are to do. Together we will work through you tube and how to upload a video, and then we will break into groups of two for them to prepare and present their material. This will also benefit the student who is at a lower reading level because he will have a partner to help him through the lesson.

Analyze the relationship between health-related information and adolescent consumer choices. 10.2.D. Analyze and apply a decision making process to adolescent health issues.

Lesson Objective(s) (Stated in observable and measurable terms) After instruction on childhood obesity, students will locate and list at least 3 ways a person becomes obese while preparing for their you tube presentation 4 out of 4 times for 1 week. After instruction on childhood obesity, students will locate and list at least 3 ways a person can combat child hood obesity while preparing for their you tube presentation 4 out of 4 times for 1 week. Given a list of directions and tasks to complete students will complete the tasks accurately each day with 100% completion for the duration of this lesson.

Assessment Plan (What will be done to determine if lesson objectives have been met?) I will informally assess students each day as they are working by observation and notes, as well as have an exit ticket each day. The students will write down one thing they have learned, or one task they have completed for the day. I will also be assessing the entire project and presentation using a rubric I will create in rubistar.

Materials: Student will be utilizing computers for this assignment.

Inclusion Techniques for Students with Special Needs: For the student with a lower reading level I will assign him a partner who works well with other students and can help him comprehend what he is reading throughout the process.

Enrichment Techniques: I will have one student that this project will be particularly easy for. To further challenge this student he will have to find information and create a graph related to obesity in America or in his state. This graph will be uploaded to his computer and embedded in his video before he uploads it into you tube. He will be asked to explain the graph during his presentation.

Lesson Differentiation (What modifications/accommodations will be made to ensure that ALL students have access to and are able to participate in the lesson): The student on the lower reading level will have a fact sheet concerning childhood obesity. I will help the student make this fact sheet and I will read it to him and he will read it back to me and to himself. This way he is hearing it, speaking it, and reading it. He will keep this worksheet in his folder to refer to when doing the you tube presentation. I will also make a printout of the directions and read it aloud to him so the directions are clearly stated for the student, and he can refer back to them and ask me as needed.

Lesson Presentation
Introduction/Motivational Activities/Anticipatory Set: To introduce this lesson I will show my own video that I have created and uploaded to you tube. I will present it in the same manner the students will be presenting it in class. I will then tell the students they will have their own videos on you tube for everyone to see so make sure you make them good. I believe this will get the students excited and motivated about the project and hopefully about staying active and healthy.

Detailed Teaching Sequence: (Provide sufficient detail that would enable a substitute to effectively present this lesson. Bulleted statements are preferred) Day 1. Go over the directions, expectations, and rubrics for the upcoming lesson. Present the you tube video to the students and explain this is what they will be doing. Go to photostory.com so students can create an account and work through the site. Explain to them they will be uploading photos and facts into photo story to create a you tube video Begin to gather pictures and facts on child hood obesity. (Prevention and causes) Day 2. Upload all pictures and facts to photo story account Work through the process of designing the video with the students (transitions, text, Colors, music, etc.) Save the progress Day 3 Complete the photo story and upload it to you tube. (Before you they upload) Check the stories and go through them with the students and make sure they are ready to present. (Students will need to go to you tube and create an account to upload their story) Day 4 Present the photo stories in class. Make sure the students write down at least 1 question for the presenters. Collect the students questions and rubrics for the presentation.

Guided Practice/Independent Practice/Assessment Activities Students will assess each using a premade rubric I have given them. They will score, ask one question, and provide feedback to their peers on the rubric then turn it in to me. I will go over it and make sure they have done this. I will then return it to the presenter along with my rubric and their grade for the assignment and the lesson.

Closure: Students will write two paragraphs in their journals on what they learned, enjoyed, and did not enjoy about the project. I will also ask the student for feedback about the groups, the manner in which the lessons were presented, and what they would change or keep the same.
