Nasogastric Tube Feeding Nasogastric feeding tube is a medical device used to provide nutrition to patients who cannot obtain

nutrition by swallowing. Purpose: to restore or maintain nutritional status to administer medication Equipements: -disposable feeding bag -asepto syringe -stethoscope -prescribed formula -clean gloves PROCEDURE: 1.Asses client's need for gastric tube feedings; impaired swallowing;decreased level of consciousness, head or neck surgery, facial trauma, surgeries of upper alimentary canal 2.evaluate client's nutritional status. Obtain baseline weight and laborator values. Assess client for fluid volume excess or deficit, electrolyte abnormalities, and metabolic abnormalities such as hyperglycemia. 3.verify physicians order for formula, rate, route, and frequency. Laboratory data bedside equipements, such as finger-stick blood glucose measurement, are also ordedr by the physicians 4.explain procedure to the client 5 prform hand hygiene 6.auscultate fir bowel sounds before feeding 7.prepare feeding container to administer formla a.check expiration date o forula and integrity of the container b.have tube feeding at room temperature c.connect tubing to container as needed or prepare ready to hang container d.shake formula container well, and fill container with formula. Open stopcock on tubing and fill with formula to remove air. 8.for intermittent feeding, have syringe ready and be sure formula ias at room temperature 9. place client on semi-fowlers positio, or elevate head of bed 30 degress. 10. Verify tube palcement: consider together the results from ph testing and aspirate's apperance. 11. Check for gastric residual a.draw 30ml of air with syringe. Connect to end of feeding tube. Flush tube with air. b.pull back evenly to aspirate gastric contnent c. return aspirated contents to stomach unless the volume exceeds 100ml 12.flush tubings with 30ml of water 13.initiate feeding 14. advance tube feedig gradually 15. following intermittent infusion or at end of continuous infusion, flush nasogastric tubing with 30ml of water 16. when tube feeding iare not being administered, cap or clamp the proximal end of the feeding tube 17. remove gloves or perform hand hygiene 18. do after care and document all relevant information.
