Case Manager - The Case Manager provides direct individual and family casework services with an
emphasis on meeting the psycho-social needs of clients. This includes the assessment of service needs, service planning and service delivery for assigned clients within UCAN policies, procedures and practices. A Bachelors degree in Social Work or, BS/BA in Human Service field is required. Either a Licensed Child Welfare Services Employee or license eligible, 0-2 years of experience working in human services, child welfare, or crisis intervention. Car and valid Illinois drivers license required.

Community Support Specialist -The TLP Community Support Specialist works in conjunction with
the rest of the TLP treatment team to provide direct individual and group services to adolescent teens and teen parents and their children who reside in a transitional living setting. GED or H.S. Diploma required with 3-5 years of experience working with children and adolescents. Associates Degree preferred with at least 1 year experience working with children and adolescents.

Data Integration Analyst -The Integration Analyst reviews UCANs data and management systems as
well as the process infrastructure to recommend options for improving process efficiency. The integration analyst is responsible for data integration and design standards for the Chicago Allies data initiatives in order to meet the Chicago Peace Hub objectives. The analyst also advises department heads on system integration methodologies to make operating activities more efficient. Bachelors or Masters degree in Computer Science, Management of Information Systems or related degree with 5+ years in software development, three to five years of supervisory experience, 5+ years of experience with the following technologies: ASP, HTML, JavaScript, VBScript, ADO. Microsoft .NET: ASP, VB, C# (2+ years), ADO.NET, Microsoft SQL Server 2000 and higher Database including PL / SQL (Stored Procedures, Triggers, etc.), Crystal Reports v9.0 and higher and Microsoft Internet Information Servers.

Director of Residential Training/Staff Development - Masters in Social Work, Counseling or


Psychology with LCPC or LCSW certification required. 3-5 years of experience in Residential treatment, with a mixture of experience in direct care, case management, therapy and as a supervisor. 3 years of previous supervisory experience preferred, experience conducting training preferred proficiency in using computer software (MSWord, PowerPoint, Publisher, Curriculum Design, and Data-entry). Automobile and valid Illinois drivers license required.

Foster Care Case Manager -Provides direct service for children in traditional as specialized foster care.
Develops and implements service plans to achieve permanency for children. Works with biological parents to help them achieve service plan objectives, supports foster caregivers as they provide care for children. Represents UCAN in Juvenile Court proceedings, clinical staffings, child and youth investment teams, and other areas as needed to plan and coordinate services. Supervises parent child and sibling visitation, provides transportation for visits as needed. Bachelors degree required; valid Driver's license and Car.

Future Leaders Now Manager - Future Leaders Now Manager manages all aspects of the Future
Leaders Now Initiative with a focus on coordination, consultation and technical support. As a member of the ULEAD team, the Future Leaders Now Manager also organizes and participates in ULEAD and UCAN events and initiatives as needed. Bachelors Degree in Human Services or related Field required. Five years youth development experience. Supervisory experience preferred. Alumni of UCAN run or similar youth programs preferred.

Psychotherapist - The Psychotherapist provides individual, group and family psychotherapy services to
adolescent clients in the Transitional Living Program. This requires proving home-based services and working within a treatment based team setting. A Masters Degree in Social Work or MA/MS in Human Services/Psychology/Counseling is required. 0-2 years of experience providing therapy in a clinical or related setting, two years of experience in child and adolescent individual and group therapy LPC or LSW a plus LCPC, LCSW or equivalent for Psychotherapist II. Car and Valid IL Drivers license required; experience and/or certification in Domestic Violence.

Quality Improvement Coordinator - The Quality Improvement Coordinator coordinates numerous


program evaluation and quality assurance functions within the Department; including monitoring programs for compliance toward quality standards, implementing tools used for program and service evaluation, and organizing data collection needs. Bachelors degree required; Masters degree in Social Work or related field preferred. 2- 3 years of work experience in social service field, working knowledge COA and DCFS standards preferred, working knowledge of Child & Family Profile preferred.

Residential Case Manager - The Residential Case Manager coordinates individual and group casework
services to clients in the residential treatment program by carrying a caseload of moderate complexity that is representative of the cases found in the program. A Bachelors degree in Social Work or BS/BA in Human Service field is required, 0-2 years of experience working in human services, child welfare, or crisis intervention. Valid IL Drivers License and Car required.

Supervising House Parent - The House Parent lives on-site and provides on-site 24 hour, to nonparenting and parenting adolescents who reside in UCANs Transitional Living Program. The House Parent is responsible for the management of TLP facility and supervision of Community Support Specialist staff. House Parent is responsible for the safety and well-being of the teens within UCAN policies, procedures and practices. This is a direct service position that emphasizes on life skills, parenting, mentoring, and relationship building. This will includes working directly with staff and client to teach daily living skills, coping skills, ensuring the clients compliance with program rules and expectations as well as issuing/enforcing consequences within the TLP program guidelines. Associate Degree and or are actively and continuously pursuing a degree, 3-5 years of experience working with adolescents/parenting teens preferred. Car, valid Illinois drivers license and auto insurance required. Experience or ability to use computers and be flexible with scheduling needs and shifts.
