    HEALTH PROBLEM Poor Sanitary Condition that results to lack of personal hygiene. Data: Sanitary Condition specifically personal hygiene Involving 5-Families = 100% PROGRAM FACTORS IMPLICATION OBJECTIVES OF CARE After 5 days of community nursing intervention: 1. 5- 100% of the families will acquire and discuss the information about the benefits of having good sanitary condition mainly having good personal hygiene. TARGET POPULAT ION 5 families PERSONS INVOLVED EVALUATION

    Sanitary Condition regarding Good Personal Hygiene Health Education:  Benefits of Good Personal Hygiene  Ways to have Good sanitary condition with the aid of keeping good personal hygiene.

    Inability of the family to have good sanitary condition due to: 1. Lack of Knowledge about its benefits and ways to do it.

    1. Due to lack of knowledge, the community may not be able to recognize the benefits of good sanitary condition which may lead to lack of good personal hygiene.

     Students of CMCC Section II-1 Group 4  Clinical instructor ( Mrs. Jocelyn Amlog RN, MAN)  President of the community  CHN

    1. The families started discussing the benefits of good sanitary condition mainly having good personal hygiene and plan to implement it.
