Behavioral Intervention Plan Behavior Intervention Plan: Clyde refuses to complete class work assignments and homework. He appears to be lethargic from medications, and at times this behavior is due to non compliance. When he does complete work, he rushes through assignments and is often incorrect. He will refuse to make corrections. As the day progresses, he becomes distracting to others (tapping on the desk) and demonstrates attention seeking behaviors amongst peer groups. Baseline Data Observations: Clyde has been diagonosed as ADHD, inattentive type. He has diffuclty focusing for extended periods of time, without indications of medication management. Cylde functions on grade level, therefore is capable of completing assignments, with some modifications or assistance from teacher. Clyde is unable to complete assignments independently, as he is easily distracted, or intiates distractions to others. Invidualized Information about Clyde: Clyde enjoys physical fitness and is very socialable amongst peer group. Although Clyde is likable, due to his behaviors he has difficulty maintaing and respecting social boundaries. When medication manamgement needs are met, Clyde is able to maintain focus and attention during instructional time, and better refrain from distracting behaviors, often resulting in consequences. Clyde is receptive to positive praise from authority. Clyde will make self disparging remarks (I'm stupid, I can't do it ) when he is unable to complete a task daily. He makes excuses for his behavior, or inabilty to complete task, due to medication managaemnt needs not being met. Hypothesis statement based on functional behavioral assessment: Clyde will be able to complete task and increase accuracy with assignments daily while meeting medication management needs. Target Behavior: Increase completion of daily assignments by 90% . Reduce or eliminate distracting behaviors by 100%. Maintain consistent expectations and allow Clyde to complete a highly desirable task (extra computer time, playing with ipod) when assignments are completed.

Intervention Goal: To improve completion and acurracy of school assignments. To increase the number of assignments completed in alloted time. Reduce distracting behaviors and increase level of focus, by ensuring medication management needs are met daily. Intervention Plan: Clyde will be given 2 out 3 assignments to complete per subject daily. The assignments will be modified and the number of problems will be reduced to assist with meeting the outlined goal. Clyde will be given modified versions of the assignments (less problems, shorter reading passages or other variations in reading assignments). He will be given clear and concise directions and expectations (length of time for assignment), and will repeat the directions to ensure level of comprehension and understanding. Clyde willl be provided immediate feedback regarding task completion. Daily/ or as needed collaboration will be made with parent regarding medication mamangement. Intervention Data Collection Summary: Clyde will be monitored and observed by a behavior counselor to assist with meeting goals daily. Behavior counselor will address behaviors that impede the learning process for student. Clydes progress will be monitored and made visible to student through a behavior chart that will provide immediate feedback, as well as positive and negative reinforcement for student.
