RATIONALE
EXPECTED OUTCOME After series of nursing intervention, the clients sense of social isolation will be reduced.
Objective data:
After series of nursing intervention, the clients sense of social isolation will be reduced.
perception of unacceptable social behavior as evidenced by expressed feeling of aloneness and rejection.
Spend time talking with patient during and between care activities.
Patient may experience physical isolation as a result of current medical treatment and some degree of social isolation secondary to the diagnosis of AIDS. Reduces patients sense of physical isolation and provides positive social contact. Explanation may help client understand reasons for procedures and provide feeling of inclusions in
Encourage open visitation, telephone contacts, and social activities w/in tolerated level. Be alert to verbal/nonver bal cues, statement of despair, sense of aloneness and suicidal thoughts.
When these cues are acknowledge by the caregiver, patient is usually willing to talk about thoughts of suicide and sense of isolation and hopelessness.
