Right medicine. Right route. Right time. Right dose. Right documentation. Right to refuse.

Eardrop administration: y Need paper towel y Cotton ball y Inform parents of cotton ball in ear. y Child- pull ear down and back. y Adult- pull ear up and back. Meds can be given: y IM, IV, Topically, Sub Q, PO, Rectally, vaginally, ECT. Parentally Injections: y IM, IV, IVP, IVPB, SubQ, Intradermally. Makes sure you have all of you supplies when you walk into the room. Always tell them what you are going to do. Seldom you massage, but some meds require you to massage. y Always use alcohol wipes. y Wipe the area in a circular motion leave the alcohol wipe at the end pointing towards the site. y Always use sterile technique for any procedure that is going to pierce the skin. y Follow the manufacture s directions. y Check expiration dates. y Always ask meds taken/ allergies/ and medical history. y Always check clarity of solutions y Check if the pill looks normal y The internet is not a reliable source. y Cortizone- interferes with the body system so the body will not be able to sense infection because it masks infections. y Always acquire street meds they may use. y Open up pills directly at bedside. y Never leave pills for the patient come back. y Always make sure you read and check the medication Check the medication 3 times before you give it

Three checks before you give the meds two on the pt y Check the id band y Ask them what their name is date of birth ect. y Then check the MAR. PO medication administration: y Make sure the pt can swallow. y Always have fresh water to give. y Never leave meds unattended. y Always identify yourself. y If pt refuses meds find out why? Giving eye meds: y Have a tissue y Give in the cantus y Put inner to outer. y Have them lean back and in the direction it will be given. y If ointment, inform the pt that vision may be blurry. Applying Topicals: y Apply a thin layer y Remove old medicine y Rotate sites. y Mark name, time, and date on patches. Never give more than 3-5cc if you get more, question the dose. y Always chart the site y Chart how the pt tolerated Enteric coated is to bypass the stomach to get absorbed in the small intestines. Injection sites: Interdermal: y Normal sites- forearm, upper back, upper chest. y 25-27 gauge needle. y 1 ml or less syringe. y Bevel up y Slowly inject solution y Blot blood with gauze y Draw circle around the site of injection. y Check site 15 minutes after.

Intramuscular Injection sites:

Ventrogluteal site: o Side lying position. o Place hand on hip (right on left or left on right). o Place palm on Greater Trochanter. o Index finger on anterior superior iliac spine. o Middle finger on Iliac crest. o Injection site in the middle of the triangle between index and middle finger. Deltoid: o 4 fingers across the deltoid muscle. o Top finger across the acromion process o Injection site 3 fingers below the acromian process. Vastus Lateralis: o Have patient lie supine or sitting. o One hand at the Greater Trochanter and the other above the knee. o Form a rectangle o Injection site is in the middle. Dorsogluteal site: (Last resort.) o Prone position ONLY. o Locate Greater Trochanter and iliac anterior superior spine. o Draw a line between. o Injection site is one inch above imaginary line. IM injections: o 1-3 ml syringe. o 21-23 gauge o 1 inch in length o Hold skin taunt o Aspirate for 5-10 secs. o If you pull back blood take out and reprepare the medication. o Slowly insert medication. o Gently massage. Z-track Method: o 1-3 ml syringe, 21-23 needle gauge, 1 inch o Wait 10 sec after medication is entered before you pull out. o Do not massage site! Subcutaneous Sites: o Thigh, abdomen, upper arm, upper gluteral. o 25 gauge 5/8 needle inch.

o Pinch 2 inchs-90 degree angle. 1inch- 45 degree angle. o Do not massage site.
