Case 5: You are the QA manager. One of the user groups in your Organization has reported unacceptable
defect levels in module 1 of a new development project. The project Team Leader says that
a) the defects were due to insufficient skills of a fresh batch of trainees allocated as team members
b) since a release was already announced, meeting the schedules was the main objective
c) the user group made changes to standards half way through the development
What steps would you recommended the project follows to avoid repetition of the problem and why?

entry in the adjustment and cheque writing process seems to have problem. Error due to this is one percent.
Each of it has an impact of $1000.
a) Estimate the loss because of the problem.
b) How do we reduce the probability?
