Sparta, MI 49345 Phone & Email: 616-887-4207, lorilflorida@yahoo.com BUYER: Name: __________________________________________ Address: ______________________________________________________________ Phone & Email:_________________________________________________________ PURCHASE PRICE The Purchase Price of the Puppy is $1,000.00 HEALTH GUARANTEE Breeder warrants the dog to be in good health and free from parasites as of the time of sale. Further, Breeder warrants that both the sire and dams hips have been X-rayed and evaluated by the Orthopedic Foundation for Animals (OFA) in the United States, and have been found to be of the grade Fair or better and are free of canine hip dysplasia. In the event of emergence of any genetic disorder within one year of purchase, which is sufficiently serious in the opinion of two qualified veterinarians, one of whom is selected by Breeder, to warrant euthanasia of the Puppy, Breeder agrees to: Replace the Puppy with one of equivalent quality from the first litter bred by the breeder or, at the breeders option, Refund the original purchase price less the non-refundable deposit Breeder strongly encourages Buyer to have the Puppy seen by a veterinarian of Buyers choice within 72 hours of picking up the Puppy. The puppy may be returned to the Breeder

for any reason within the 72 hours following purchase. Provided that the Puppy has not been damaged in any way, Buyer will then be entitled to a refund of the original purchase price less the non-refundable deposit. REGISTRATION Breeder certifies that the Puppy is eligible for registration under the Regulations of the American Kennel Club as a purebred Airedale Terrier and will provide the proper forms for such registration. Breeder reserves the right and Buyer agrees to affix the kennel name of LEGACY in the AKC registered name. If the Puppy is purchased as a pet, limited registration will be provided prior to neutering. RIGHT OF RECOVERY OR PLACEMENT Should the Buyer find themselves unexpectedly in a circumstance where death or other loss prevents them from being able to properly care for the Puppy, Buyer agrees that we will be contacted first and given first rights to the Puppy. If the dog is to be sold, Seller will be given right of first refusal. Buyer agrees that the Puppy will NOT under any circumstances be placed in a dog pound or rescue center.
