Now look at question 1 and take notes on Brians work situation Pause 10 seconds GP Good Morning. Come on in Brian and tell me how I can help you Brian Ah hello Doctor look Ive come here because Ive got a real issue with um sleeping and I was hoping that youd be able to prescribe me with some sleeping tablets. GP Well just before we go straight into a prescription for tablets. I think it would be useful if you could just tell me a little bit more about your problem. Brian Ok well it relates to my work. I work at the BP oil refinery and I do a lot of shift work and because of that Ive got irregular sleeping hours and Im not sleeping well at all. GP And how long has this problem been going on? Brian Ive been working at the refinery about three months now. GP Umm Brian And I started night shift about six weeks ago and its just been building up since then. GP And tell me more about your work hours Brian Well.basically we do twelve hour shifts in the day time and thats followed by two twelve hour shifts in the night time. GP And is this problem sleeping after the night shift or is it no matter whether its day or night? Brian Ah actually I can sleep after the day shift but after the night shifts Im having a lot of trouble. I dont seem to be able to get more than 4 or 5 hours sleep after the night shifts GP Oh thats no good Brian Umm GP And how does this impact on your work? Brian Well it is a problem because you know my job is quite dangerous we need to monitor a lot of equipment and operate some machinery GP Umm Brian And Im finding that I need to be alert and when Im tired I have to resort to drinking a lot of coffee and lots of caffeinated drinks just to get through my shifts. GP Right well Brian would you mind if I just went through and talked about what things we can do to help you sleep better because I think its important that we do more than just prescriptive sleeping tablets, Brian Okay, I guess so PAUSE 15 seconds Now look at question 2. Take notes on the reasons against taking sleeping tablets Pause 10 seconds GP Generally speaking sleeping tablets are of short term use only. What happens is that if we start you off taking sleeping tablets regularly youre going to find over time you have to increase the dosage to get any real effect and there is a real risk that you will become dependent on them. Brian Oh I dont really care too much about that,.. its now that Im having trouble. Whats important to me is that I, you know, keep my jobyou know what happened to me on my last shift I actually fell asleep for 15 minutes and my supervisor caught me out there and it was due to not getting enough sleep so thats really why Ive come here today. GP Well well go on to look about what we can do to help you over this immediate problem but I would like you to realize that with sleeping tablets its not only the fact that you may become addicted to them, but that they can actually impair your memory and your ability for critical thinking. Brian Really? GP Now you were telling me youre working in quite a dangerous profession and its really important that you are alert, that your not suffering a hangover from sleeping tablets. Brian Yes I see what you are saying Pause 5 seconds Now take notes on the living environment Pause 5 seconds GP Now Brian, Id like to talk to you about what living environment is like - where you go to sleep normally, and what sort of things would make it difficult for you to get to sleep. Brian The noise in the daytime I suppose. There is a lot. You know we live in the suburbs and if its not one thing then its another for example a neighbour might start mowing the lawn in the day time. Weve got the birds that can wake me up. . a lot of crows at the moment. they seem to hang around the house on certain days, sometimes the phone rings, even the brightness of the room or hot weather, all the these things have been disturbing me. Pause 5 seconds Now take notes on the solutions given by the doctor Pause 5 seconds GP Well thats why I think that what we really have to do is to get you the best possible environment. Now just some solutions I would offer Think about overseas travelers, they use earplugs to get the noise down um you can get an eye mask that can help you sleep in a darkened state. Many people say it works for them. You could also have dark curtains - ah it depends but you could take the phone off the hook so that youre not being interrupted when you are trying to get to sleep. Some people find if they play soft music that helps them but as you have said this is a permanent job so you need to get a permanent solution. Brian Yes I do GP We need an environment where you are comfortable and its s conducive to a good sleep. Brian OK I havent actually tried the earplugs yet so that might help. PAUSE 15 seconds Now look at question 3. Take notes on further solutions given by the doctor Pause 10 seconds Brian Well doctor apart from that are there any other things I can do? GP Yes there really are a number Brian. Perhaps the most important is to get into a regular routine and establish really good habits prior to going to sleep. Brian What do you mean? GP Well for example its not a good idea to have a heavy meal and then go straight to bed. Brian

GP Its also important to avoid caffeinated drinks - you were telling me you are having a lot of coffee and thats going to stimulate your nervous system and when you go to bed you are going to be awake and alert Also alcohol, just be aware of the impact that can have and then of course - you have probably heard about - it but there are a number of really simple but very effective relaxation techniques. Brian Like what Doctor, can you explain? GP It can be just as simple as learning how to breathe slowly in through your nose and out through your mouth. There is a very good relaxation centre in the Valley that you might be able to consider, there is also herbal tea and I dont know if you are a music lover but many people find that relaxation..you know relaxing calming music played softly in the background helps them considerably. Brian OK Yeah well I could definitely try that. PAUSE 10 seconds Now look at question 4. Take notes on future management Pause 10 seconds Brian Doctor I am still in a bit of a difficult situation at the moment. Ive just done two day shifts and Im about to go on to night shift and I really do need to get a good nights sleep I mean even now coming to you today I had a bit of trouble concentrating and I do feel tired GP Right, well I think this is a case where we can help you in the short term by using some sleeping medication and Im going to prescribe some Normison for you but what I want you to do is look at this always as a short term solution and now tonight before you go to bed perhaps half an hour before youre ready to go to sleep take your Normison then. You dont want to wait four or five hours and then think so Im not sleeping Ill take it and then only give yourself a short period of sleep because then youll end up in that hangover effect Make your decision that you will try to get a good nights sleep and half an hour before your going to bed youll do the other things we discussed and then take your tablet. Now what Id like you to do is to try to avoid taking more than three tablets in any one week. Brian OK GP Say to yourself right Ive had a good nights sleep and Ill see if I go to sleep naturally the next night Brian Fine, OK I can do that GP Right - well what Ill do is, Ill give you the prescription for the Normison . Now Id like you to come back after two weeks and tell me how your plan is progressing. Brian OK Doctor GP And Ive also got a brochure here for the relaxation centre near you. Id like you to contact them and see if you can get some idea of some techniques that you can learn to use that will help you relax more. Brian OK thank you Doctor, I really appreciate you helping out with those pills GP Right well Good Luck with that Brian
