Auricular Treatment Guide

IntroductIon
Your Pilot Diode Laser is equipped with a unique attachment which will aid in the therapeutic treatment of disease and infection of the external auditory meatus (EAM) or ear canal. The auricular treatment attachment allows the clinician to administer photo therapy deep into the ear canal in order to irradiate and treat the entire infected area.

The auricular treatment attachment is a 6.5 cm (2.5 inch) sturdy glass tube with a highly efficient reflector which redirects the laser from a frontal projection outwards towards the surface of the ear canal thereby treating the infected EAM while protecting the tympanic membrane.

The auricular treatment attachment is easy to attach and easy to use following these instructions. 1. 2. 3. 4. 5. 6. Strip the end of the fiber to 10 mm using the measurements on the fiber striping tool. Cleave the end of the fiber to assure a proper cleave. Insert the fiber through the black handpiece extending about 2 inches. Insert the fiber into the pink end of the auricular attachment pushing the fiber though to the visible end of the tip. Attach the auricular attachment to the end of the black handpiece. The end of the fiber should be 2 mm (1/8 inch) from the reflector at the end of the attachment. Adjust fiber to correct length and tighten the cap on the end of the handpiece to secure the fiber.

The glass tube of the auricular attachment will contact the ear canal of the patient. This is not a concern. The only precautions are to adjust the laser for minimal frontal projection to avoid lasing the tympanic membrane and also to keep the power setting to below 2 watts to avoid heating the tube and reflector. Using any flat surface, the clinician can see the projection pattern and can adjust the pattern by moving the fiber forward or backward as needed to optimize the treatment. Using a colored straw is an easy way to simulate the ear canal and demonstrate the method of treating the ear.

The complete ear treatment will involve a series of 5 or 6 treatments. The first treatment will include a complete ear canal evaluation, bacterial culture swap, cleaning, and administration of appropriate antibiotics and medications. 1. Examine, clean, and flush ear. 2. Using a setting of 2.0 on pulse mode, insert the Pilot Auricular Treatment Tip into the patients ear and activate the laser. 3. With slow, steady motion, retract the laser taking 5 8 seconds to exit the ear. 4. Repeat 3 4 times. 5. Repeat treatment in 48 hours and 2 3 times for the first week and again the following week. 6. Evaluate the condition of the patient and determine continued or follow up treatment.

treAtment Frequency
Like any other method of treatment, a single treatment is seldom adequate. Repeated treatments over a period of 1 2 weeks is generally recommended. Treatments may range for multiple treatments per day to every couple of days. Over-treating in a single treatment does not compensate for multiple treatments. This does not work with antibiotics and it does not work with laser therapy.
