The contractor wrote the ministry requesting for an additional payment(Pg 29-31 is relevant). A site inspection is mandatory to be carried to ascertain the claim of the contractor. OBSERVATION. A. Gwanara Township. 1. The reticulation of additional Low Tension network based on the approved BEME had been done. 2. A letter of appreciation from the community which testifies to the above observation is on page 31. B. Ministry of Information (information division) 1. The Construction of the Ht network had been done. 2. The substation work is yet to be done as the material needed is available but yet to be installed ( no transformer on site). C. Yekini Aiyelore Area in Offa. 1. The contractor is yet to be mobilized on this site.

RECOMMENDATION The sum of N 368,705.82 is now recommended as an additional payment to the contractor based on the value of work done at site. The relevant certificate valuation is on page 32 for your approval

The Substation work is also completed and connected to the national grid.

A site inspection was carried to check if there could be any defect on the project. Observation. 1. Gaa Alakuko & Alatare( low Tension Network Extension) - There is no defect liability on the project 2. Gaa jubilee / Asipade (Installation Of Transformer & Reticulation of LT) - The Substation is still working perfectly. 3. Alasoro Isale( Extension of LT network) - There is no defect liability on the project. Recommendation There is no defect liability on this project and the sum of N 366,625.01 is now recommended as payment to the contractor as the retention fees due after six month.

Contractor: Comser Integrated Global Concept. Preamble. This is one of the phase II Electrification project. This project was connected to the national Grid since February, 2011. A site inspection was carried to check if there could be any defect on the project. Observation. - The Transformer is still properly functioning. - There is no defect liability on the project. Recommendation There is no defect liability on this project and the sum of N 511,765.84 is now recommended as payment to the contractor as the retention fees.

PREAMBLE Alfa funrae area is one of the recently awarded contracts on the installation of 3No. Un-installed 500KVA transformers in Offa L.G.A. The site take over was embarked upon. OBSERVATION. The following are the observation; (a) Additional work had been done on site, which differs from what was met at site during the survey exercise. However, earthing pit already done, existing HT poles dressed and uprisers cable was met at site. (b) The project is said to be financed through the world bank assisted project. (c) A sign post has just been put at site that display the details of the project.

RECOMMENDATION . This contractor is recommended for relocation to Level crossing area in offa and Kammanu Alayin in Offa L.G.A based on the attached BEME for both locations.

Contractor: sawji ventures PREAMBLE This project is basically on the replacement of burnt transformer at Idofin Igbana community at page 27-32 is the letter of completion and relevant test report submitted by the contractor. The need to inspect the site became mandatory to ascertain the claims of the contractor. OBSERVATION. The community is now enjoying electricity supply and the entire community (including the primary health centre and community chief palace) are now connected to the new installed transformer. RECOMMENDATION This project is now at 100% completion and the sum N1, 379, 531.25 is therefore recommended as payment to the contractor as per the c.v on page34.
