Theories of Aging Nurses and other health care providers should review theoretical perspectives describing the aging

process and older adults. An understanding of theories of aging can provide direction for assessment, planning, interventions, and anticipatory guidance. A few social theories of aging described, as follows: Activity theory : the key to successful aging is the maintenance of optimal levels of activity from the middle of years of life. This theory led to establishment of many activity centers. Disengagement theory : as people age, their needs changes from active involvement to withdrawal for contemplation about the meaning of life and implending death. In addition to the withdrawal of the individual from society, with drawal of society from the aged occurs. This theory caused great controversy. Socioenvironmental theory : this theory responded to the limitations of disengagement and activity theoriees. As a holistic theory, it integrates older adults and their social and environmental contexts. The theory has the following basic components. o Normative component : involves behaviorexpectations shared by older adults in social situations. o Individual component : based on the amount of behavioral flexibilty that older adults have in terms of resources (i.e., health and money). o Personal component: older adults consider the meanings that norms have for themselves and their interactions with others. Loss of Major Life roles theory : this theory challenged the disengagement theory by claiming that disengagement was forced by denying older adults societal roles (e.g., mandatory retirement from the workforce). This led to the establishment of goverment programs to provide volunteer and work opportunities. Continuity theory : the unique personality and lifelong behavioral characteristic and habits of an individual continue into old age. This theory is an attempt to balance the extremes of the activity and disengagement theories. Social disruptive events theory : if life is severely disrupted in several ways in a brief period of time (e.g., by retirement or loss of a spouse), social withdrawal becomes an appropriate response. This response should reverse with time; however, if disengagement becomes established, it becomes difficult to reverse. Reconstruction theory : negative labeling of older adults by society and self-labeling after negative life events led to a view of older adults as incompetent and helpless. Age stratification theory : this theory assumes that societies are inevitably stratified by age and class. The relative inequality of older adults at any given time or situation depends on their typical life course experiences and is due mostly to the physical and mental changes that take place and the history of the time (e.g., wars) through which the cohort lived. Modernization theory : loss of social status among older adults is a universal experience in all cultures in which modernization processes, which mostly affect the young and often involve new technology, are occuring.

Wear and tear theory : this theory focused on degenerative aspects of aging and the various multiple rates of degeneration. It is based on the life process essentially run out. Autoimmune theory : this theory is based on the premise that as age increase, mutation in cell divisions increase and the body responds to these mutation as foreign matter. In an attempt to neutralize the mutations, antibodies are produced, resulting in an autoimmune responses are intended to be adaptive, overtime these response result in the body destroying itself.

Teori Aging (Penuaan) Perawat dan penyedia perawatan kesehatan lainnya harus meninjay perspektif teoritis yang menggambarkan proses penuaan dari lansia. Pemahaman tentang teori aging ini dapat memberikan arahan untuk pengkajian, perencanaan, intervensi, dan pencegahan. Beberapa teori sosial tentang penuaan dijelaskan sebagai berikut. Teori aktivitas : kunci untuk penuaan yang sukses adalah dengan memelihara tingkat optimal dari aktivitas sejak usia paruh baya. Teori ini menyebabkan pembentukan pusat aktivitas. Teori disengagement : seperti usia orang, kebutuhan mereka berubah dari keterlibatan yang aktif menjadi lebih banyak merenung tentang makna kehidupan dan kematian yang akan datang. Selain penarikan individu dari masyarakat, terjadi pula penarikan masyarakat dari individu dengan lanjut usia. Teori socioenvironmental : teorii ini menanggapi keterbatasan pelepasan dan teori aktivitas. Sebagai teori yang holistik, teori ini mengintegrasikan lansia dengan konteks sosial dan lingkungannya. Teori ini memiliki komponen dasar sebagai berikut. o Komponen normatif : melibatkan ekspektasi perilaku lansia dalam situasi sosial. o Komponen individu : berdasarkan jumlah fleksibilitas perilaku bahwa lansia memiliki keterbatasan dalam hal sumber daya (misalnya, kesehatan dan keuangan). o Komponen pribadi : lansia mempertimbangkan arti dari norma-norma yang mereka anut untuk diri mereka sendiri dan interaksi dengan orang lain. Teori Loss-of-major-life-roles : teori ini menentang teori disegagement dengan mengklaim bahwa disengagement terjadi akibat menolak lansia dari peran sosial (misalnya, pensiun wajib dari pekerjaan). Hal ini menyebabkan pembentukan program pemerintah untuk memberikan kesempatan bagi lansia sebagai sukarelawan dan bekerja. Teori kontinuitas : kepribadian yang unik dan karakteristik perilaku serta kebiasaan dari individu dimulai seumur hidup, dari lahir sampai usia tua. Teori ini merupakan upaya untuk menyeimbangkan teori aktivitas dan teori disengagement yang ekstrim. Teori social disruptive events : jika hidup sangat terganggu dengan beberapa cara dalam waktu singkat (misalnya karena pensiun atau kehilangan pasangan), penarikan

sosial menjadi respon yang tepat bagi lansia. Respons ini harus disesuaikan dengan waktu penarikan diri. Semakin lama lansia menarik dairi, akan semakin sulit untuk pulih. Teori rekonstruksi : pandangan negatif dari orang-orang dewasa di masyarakat dan pandangan negatif dari diri sendiri setelah lansia mengalami peristiwa hidup yang buruk dapat menyebabkan ketidakberdayaan pada lansia. Teori age-stratification : teori ini mengasumsikan bahwa masyarakat dikelompokkan berdasarkan usia dan kelas. Ketidaksetaraan terjadi pada lansia pada waktu dan situasi tertentu, tergantung pada pengalaman semasa hidup dan tergantung dengan perubahan fisik dan mental yang terjadi, serta tergantung pada tempat dimana kelompok tersebut tinggal. Teori modernisasi : terjadi kehilangan status sosial pada lansia merupakan hal yang pada umumnya terjadi di semua budaya dimana proses modernisasi terjadi, yang sebagian besar dipengaruhi teknologi baru dan yang muda yang sering dilibatkan. Teori wear and tear : teori ini difokuskan pada aspek degenatif dari penuaan dan berbagai macam angka degenasi. Hal ini didasarkan pada habisnya proses kehidupan. Teori autoimun : teori ini didasarkan pada premis bahwa sebagai lansia, mutasi pada sel meningkat dan tubuh merespon sebagai mutasi benda asing. Dalam upaya untuk menteralisir mutasi, antibodi diproduksi, sehingga menghasilkan respon autoimun yang adaptif, akibatnya respon ini menghancurkan bagian dirinya sendiri.
