23 April 2012 Dear Year 5 Student, 2012 Year 5 Term 3 Common Test (23 May, 25 to 29 June 2012) Information

for Candidates: 1. Please find attached: i) Common Test Schedule, ii) Instructions To Candidates. 2. Morning and afternoon written papers commence at 0800 hrs and 1300 hrs respectively. 3. Lessons will be cancelled from on 23 May and 25 June to 29 June 2012. 2 July (Mon) is Youth Day off in lieu. Normal lessons resume on 3 July (Tue). 4. On 23 May and from 25 June to 29 June 2012, morning assembly would be cancelled. Students are to report directly to the examination venues at least 15 minutes before the start of the examination. 5. Students who will be absent from any paper, due to medical reasons, must inform the Students Affairs Centre (Tel: 6319 3120) before the commencement of the examination. 6. Examination Numbers and Seating Plan All students will be issued a 4-digit examination number, in the format of (XXYY) where XX is the class number. This number is unique to each student. Students are to remember their examination numbers. Note that YY may not be the students register number. For students offering Physics, Chemistry and Biology, their OMR numbers will be sixdigit numbers where the first 2 digits are 13, followed by their 4-digit examination numbers. Students are to write and shade their OMR numbers on the optical mark sheets for Multiple-Choice Questions (MCQ). The detailed seating plan will be displayed on the notice boards near the parade square and outside the examination venues one day before the examination. The softcopy of the seating plan can also be downloaded from the Stamford website one day before the examination. Students are to locate the examination venues using their examination numbers, and are to report to the examination venues at least 15 minutes before the start of the examination. Students are to refer to the seating plan displayed outside the examination venues and to locate the column number and row number which correspond to their assigned seats. The columns are numbered A, B, C, . For easy reference, each table in MPH and ISH has a sticker with the column number and row number indicated.

7. Examination Rules and Regulations Students are to be attired in their full school uniforms (with school badges) for all examinations. Writing paper will be provided for all examinations. With the exception of calculators and graphing calculators, all other electronic devices capable of storing information are not allowed in the examination venues. Mobile phones are strictly prohibited in the examination venues and bags are not to be left within or outside the examination venues. All students are to keep their mobile phones, bags and valuables in their lockers before the examination. All unused paper must be returned to the invigilators at the end of the papers. Students should not remove any writing paper from the examination venues. Students are to remain quietly seated while the examination scripts are being collected.

Refer to Instructions To Candidates for more Examination Rules and Regulations. Any infringement of these rules will result in the issuance of warning slips.

8. Excuse from Common Test Students who are unable to sit for any paper in the Term 3 Common Test, due to involvement in inter-school competitions, are to seek official approval from HOD(PE/CCA), through their CCA teacher mentors, by 9 May 2012 (Wed). Students who are unable to sit for any paper in the Term 3 Common Test, due to other reasons, are to fill in a request form, available from the Student Affairs Centre, and get it endorsed by their parents/guardian, before returning it to the Student Affairs Centre by 9 May 2012 (Wed). Absentees without valid reasons or medical certificates will be annotated with an Abs, for absence.

9. Refer to your subject tutors or/and department notice boards for the topics to be tested and the examination format.

INSTRUCTIONS TO CANDIDATES
a) Candidates are to be attired in their full school uniforms (with school badges) for all examinations. The wearing of college T-shirts and polo-T is prohibited even on Wednesday.

b) Mobile phones are strictly prohibited in the examination venues and bags are not to be left
within or outside the examination venues. All students are to keep their mobile phones, bags and valuables in their lockers before the examination. c) All candidates are to report to the examination venues at least 15 minutes before the start of the examination. d) Candidates who are late will not be given extra time. e) Candidates are required to place their student passes or NRIC on their desks throughout the examination. f) Use of Graphing Calculators for Examinations Candidates are required to reset and clear the memory in their graphic calculators before the start of the examinations. A physical check on the candidates graphic calculators will be conducted by the invigilators after the resetting is completed. It will be deemed as a dishonest act if any candidate fails to adhere to the instructions on the use of graphic calculator. A candidate is liable to have his or her result of the paper cancelled, if he or she is found to have committed an act of dishonesty.

g) For papers of specific subjects that allow the bringing in of texts, candidates are to ensure the following : (i) Only original texts (original published editions) should be taken into the examination venues. Nothing else, including critical works or study notes of any kind, should be taken into the examination venues. (ii) Only underlining, highlighting or the use of vertical lines in the margins are permitted. Nothing else should be written in the texts. Separate pieces of paper such as post-its and tape flags are not allowed. (iii) Any kind of folding or flagging of pages in texts (e.g. use of post-its, tape flags or paper clips) is also not permitted. h) Candidates must inform the invigilators immediately if : (i) they are issued a question paper for a subject they are not offering, OR (ii) they are issued a question paper that does not contain the number of pages or questions specified on the cover page, OR (iii) they are issued a question paper not scheduled to take place at that time, OR (iv) they are not issued supplementary materials such as graph paper, map or drawing paper stated in the question paper. i) j) Candidates are required to write their names and classes on every sheet and cover page of their answer scripts. The duration of each paper includes the time for reading the questions, studying the maps, etc. No extra time will be given at the end of the examination for the writing of names.

k) Candidates are to use writing paper issued daily by the school. Candidates should not, at any time, fold their writing paper. Candidates should not divide the page into columns. l) Candidates who offer Physics, Chemistry or Biology may be issued optical mark sheets (green in colour) if Multiple Choice Questions (MCQ) are tested. Candidates are to fill in their names, class names, and OMR numbers. Candidates are to check their OMR numbers before the examination.

m) Candidates should note that legible hand-writing and correct spelling are taken into account in the awarding of marks. In addition, candidates are advised to read the questions carefully. No marks are awarded for information not asked for in the questions. n) At the end of a paper, candidates should stop writing immediately when told to do so by the invigilators. If they use writing paper, they should arrange their answers in the numerical sequence of the questions, with the smallest number on top. o) Candidates should remain quietly seated at their assigned seats while their answer scripts are being collected. p) No candidates may leave the examination venues before the scheduled time unless permitted by the invigilators. q) All unused paper must be returned to the invigilators at the end of the examinations. Unwanted used paper must be thrown into the bins provided for this purpose. r) A candidate may be expelled from the examination venues, and have his/her papers cancelled if he / she: attempts to obtain unfair assistance or is detected for dishonesty; copies from another candidate or allows his/her answers to be copied by another candidate; possesses books, notes or memoranda or any other reference materials not authorized for use in the examination; possesses electronic devices (e.g. pocket PC, PDA), communication devices (e.g. pagers, mobile phones) and computerized aids (e.g. computerized wrist-watches) capable of storing and displaying visual and verbal information prohibited in the examination venues; writes offensive or obscene materials in answer scripts; removes from the examination venues any used or unused writing paper. s) A candidate who is absent from one or more papers without the support of medical certificates is deemed to have failed in the papers concerned. Medical certificates must be submitted to the respective subject tutors as soon as the candidate reports back to the school.
