Single User Access Request for personalized UNIX user


General Form Information Version: 4.1 How to handle this form? 1. This form is intended for user access management only. Only ONE user per form! 2. Fill in all the information in this form, store the filled form locally and then send the form to your responsible/hiring line manager to ask your line manager for approval. 3. The line manager must forward your request to the Global Service Desk (gsd@vodafone.com) with the approval statement. To avoid any problem with the checking of the relationship between user and approver e.g. in case of external employees or during change processes the following statement is appropriate: I am the responsible line manager of "name of affected user" and I approve the attached access request" 4. The GSD verify the request and collect the required approvals. Afterwards the request is forwarded to the responsible department for implementation. The GSD will open a Remedy ticket for your request. You will receive a notification with the corresponding INC ID. This INC ID will enable you to track the progress. Who must use the form? Below the actions are described: Create access = Select this option in case you didn't have this user access so far. Modify access = Select this option in case you already own this user access/account and you want to change the configuration for this access/account. This is Revoke access = Select this option in case your existing user access/account should be disabled and revoked from the system(s)/application(s). If you have requested "Modify access" or "Revoke access" please provide your existing UNIX username. The main group also known as group ID describes the UNIX group where the user will be assigned to. If known provide the information regarding the required main group/group ID. Otherwise provide a reference user. In case you need additional Secondary Groups, you have to specify these within this field. If known provide the information regarding the required main group/group ID. Based on the "valid from" date the maximum validation period can be set to 90 days into the future. It is possible to select a validation date lower than the max. validation date. Note: This information is only used as a refernce information. Name a reference user which should be used to duplicate the account. But please keep in mind that the access level is set to a minimum. Approvals OpCo requests must follow OpCo specific approval process/policy before being sent to GSD. Only agreed contacts (SPOC, local helpdesk) are allowed to 1 forward OpCo requests to GSD. 2 3 VF Group requests must be sent by the line manage/hiring line manager of the affected user directly to the Global Service Desk (SOX requirement).
