I.

INTRODUCTION

Family is a set of relations, especially parents and children.

As we all

expect the most important aspect which we could consider foremost is Health of each member of a family. Thus, we cannot achieve this health without concrete knowledge or education on how one or each one of the members would take care of himself in so far as to avoid unpleasant health condition.

Family perceptions about health and related behaviors are central to the work of FHNs including issues relating to family health promotion, self-care and family adaptation to illness or disability. In order to

support families effectively an ecological perspective was adopted which allows FHN's to take account of the inter-relatedness of individual, family and community. Families have very different resources at their disposal and use these in different ways, whether in social, economic or health terms.

It is for this reason that the researcher decided to conduct an intensive interview intended for a particular family in a barangay or in an urban community. In this way, the researcher believes that through proper and

actual/hands- on- teachings for this unknowledgeable people may somehow be

of great help in order for them to realize the essence of taking care of Gods creatures or our bodies.

II. RATIONALE
The study aimed to educate a particular family in Brgy 591 in taking preventive measures against unpleasant condition. Also, the study aimed to provide information on appropriate interventions on present health deficits of the family.

III. STATEMENT OF OBJECTIVES General Objective:


To determine the nursing students' level of cognition in the determination and prioritization of problems that exists in the community assessed.

Specific Objective:
To establish a nursing care plan which would be beneficial to the family, as well as to the community Discuss and explain the etiology of prioritized problem, its causes and effects and ways on how to avoid or remove it. Gain knowledge and skills Related to the prioritized problems

Formulate a clear and specified plan of action to give solutions to the existing health problems identified within the community Formulate a clear and specified plan of action to give solutions to the existing health problems identified within the community Evaluate the effectiveness of he action and the whole program done in the community given

IV. METHODOLOGY AND TOOLS USED Methodology


The researcher used random sampling method in choosing the family to be interviewed from the chosen urban community, Brgy 591 Sta. Mesa Family. We also used actual data gathering, data collation, data presentation and data analysis

Tools Used in Data Gathering


An initial database guide from Community Health Nursing book by: Maglaya was used.

V. SCOPE AND LIMITATION OF THE STUDY


This study was conducted in a Urban community, Barangay 591 Sta. Mesa Manila. Random Sampling procedure was used where in a particular family was interviewed. 3|Page

Furthermore, this study is limited to only one family. The study strivesto find the most prioritized family health problems, and do the most appropriate intervention. The family was interviewed using an initial database guide from the Community Health Nursing book By: Maglaya.

REMARKS Since most of her offspring is on other countries. The space is enough for 10 people Each of the mini House have a bedroom that can accommodate each of them Some jars have stagnant water on them. none Has safe cooking facilities and tidy food storage. Located in urban area. Uses cellular phone and telephone as the only communication gadget. Public Utility Vehicles are readily available for transportation purposes.

D.Health Status of each family member


According to Ms. Soriano she have a hypertension that she inherit on her family since they have a history for having Hypertension.

Sometimes the kids would have cough and cold

*Health Lifestyle Practices:

--Foods are served more than 3 times a day. Children hate to eat vegetables. Maintain good sanitation. Always have time for recreational activities like trading stories

*ADEQUACY OF: a.) Rest and Sleep Makes sure that the 2 children sleeps at least 8hours at night time. Adult members of the family sleep at least 6 hours a day and b.) Exercise/Activities rest in between work time. Everyday chores became their everyday activities in which everybody participates . Plays outdoor during weekends. Walks to c.) d.) Use of Protective Measures Relaxation and other Stress school every weekdays. Every member of the family takes vitamins for protective measures. Chat with one another everytime. Watch television together with other members of the family. Seldom go to malls.

-Threat of cross infection from a lice infestation case as a HEALTH THREAT -One of the family member has a lice infestation -since they live together other family members sleep together on one room, and thus, may cause cross infection.

Inability to recognize the presence of the condition or problem due to denial about its existence or severity as a result of fear of consequences of diagnosis of the problem, specifically social stigma.

-Presence of breeding sites of vectors of disease as a HEALTH THREAT -many jars are left open and filled with stagnant water that can serve as breeding ground for mosquitoes

Inability to provide a home environment conducive to health maintenance and personal development due to lack of skill in carrying out measures to improve home environment.
