Rationale This assessment is being conducted based on specific provisions of Republic (RA) Act 7722 otherwise known as the Higher Education Act of 1994, specifically those in Section 8 Powers and Functions of the Commission a) monitor and evaluate the performance of programs and institutions of higher learning for appropriate incentives as well as imposition of sanctions such as, but not limited to, diminution of or withdrawal of subsidy, recommendation on the downgrading or withdrawal of accreditation , program termination or school closure; b) promulgate such rules and regulations and exercise such other powers and functions as may be necessary to carry out effectively the purpose and objectives of this Act; and c) perform such other functions as may be necessary for its effective operations and for the continued enhancement, growth or development of higher education. Objectives of this assessment: General a. Create an accurate picture of the Mathematics programs in the Philippines in light of CHED Memorandum Order No. 19 series 2007, Polices and Standards for the Bachelor of Science in Mathematics/Applied Mathematics and in view of rationalizing the mathematics programs in the country. Specific b. Collect and analyze data regarding administration, faculty, students, facilities, and other information regarding the operation of mathematics programs in higher education institutions in the country; c. Verify collected data through subsequent interviews with school administrators and faculty involved in operating the mathematics programs; and d. Recommend possible policy directions for mathematics based on the acquired results of this assessment. Instructions: 1. This instrument shall cover undergraduate/graduate mathematics grograms. 2. This assessment instrument shall be provided beforehand. 3. The unit/department should make available all the necessary supporting documents and facilities for verification of data entered into the attached form. 4. The instrument must be accomplished electronically. 5. Accomplished survey instrument (2 Hard Copies and an electronic copy in CD)should be forwarded to the CHED on or before March 15, 2012 via email through these addresses: ched.tpsm007@gmail.com, alineamags@gmail.com, or mawigabrillo@gmail.com via post to Dr. Sinforoso M. Birung, CESO III Director IV Office of Programs and Standards Commission on Higher Education 3rd Floor, Higher Education Development Complex C.P. Garcia Avenue, 1108 Diliman, Quezon City 6. The period covered by this assessment is 2001 to the present. Inquiries regarding the assessment may be directed to the CHED-TPSM Secretariat through the email addresses above or via telephone (02)441-1253/441-1228.
