BSCCPL/ Accident Investigation D) ACCIDENT/ OCCUPITATIONAL DISEASE DETAILES- STATE EXACTALY 1. DESCRIBE THE INJURY, PART OF BODY INVOLVED AND SPECIFIC LEFT OR RIGHT SIDE

Operator got First Aid Injury, near knee of the right leg, immediately taken him to hospital. He is ok and no any fracture or internal injury occurred, but could have resulted in major one.
2. WHAT HAPPEN TO CAUSE THE INJURY

At around 4:00 PM on 08/10/09 Mr. Pathak was loading the cargo in trailer by Hydra (MH 43: 2213) at Mumbra Warehouse, the Regular Operator Mr. Rakesh Pathak was absent on the day because of sickness and due to emergency work Mr. Pathak arranged Mr. Rajendra ( Operator) for work. First trailer loaded successfully. For loading Second Trailer Operator lifted 1 ton 150 Kg Cargo by web sling and Reversed the hydra to take position , The boom was extended and the load lifted was on height, He took fast turn and the load lifted goes out of his control, due to jerk to the boom Hydra Got Tilted. In this Incident Operator got First aid Injury to his Knee of the right leg , cargo packing of bottom side got Heavy Damaged, the engine oil and break oil of the Hydra is spilled on the ground due to tilted on one side and operators cabin Door Glass fully damaged .

4. EXPLAIN WHAT THE EMPLOYEE WAS DOING AND THE EFFORT INVOLVED The operator Mr. Rajendra was loading Cargo in the trailer by Hydra Crane 5. IDENTIFY THE SIZE, WEIGHT AND TYPE OF EQUIPMENTOR MATERIAL INVOLVED

6. WHAT CONDITION ATTRIBUTE TO ACCIDENT

Due to work pressure Mr. Pathak arranged untrained operator to operate the Hydra Crane, He was not having sufficient experience and Knowledge of the operating. He did not know that lifted load should be always kept as low as possible. Also cargo was not labeled/ marked with the slinging point, center of gravity, Type of the sling to be used and other handling instructions. The accident was held on the spot was uneven, so the load distribution on the Hydra Crane was not accurate.
7. WHAT STEPS HAS TO BE TAKEN TO PREVENT RECURRENCE

The lifting Tools and Tackles should be inspected daily before use and the statutory third Party Test should be carried out. The un authorized operation should be avoided ( untrained operators not allowed to operate any equipment unless approval from the equipment manger and the HSSE Officer) The method statement of any High Risk Operation (Loading, Unloading, Chemical Handling, Transporting ODC cargo, Chemical Warehousing Material Stacking Etc.) activity should be approved by the HOD and the HSSE Officer.

(Undertaking High Risk operation without getting approved from HOD and HSSE Officer will be deemed as Serious Violation ) Before Slinging the load/ Cargo following things should be calculated / Considered: The Weight of the Cargo Dimensions of the Cargo Center of Gravity of the Cargo The type and Number of Slings to be used Slinging Point Capacity of the crane and Load Chart. (Crane Checklist To be filled and submitted to the HSSE Dept.) Capacity of the sling Physical Inspection of the Slings, D Shackles, Working Environment, Wire Rope, Etc. Trained Operator (Driver), Signal Man. Competent and constant supervision Area Should be Barricaded Approved Method statement. The operator and the employees working in the vicinity should be provided with the PPEs (Personal Protective equipments) and the compliance should be strictly monitored by the warehouse supervisor of Operation Supervisor. 8. Responsibility to Implement D. (7): The responsibility to implement the recommended actions is lies with the HODs of all dept. those who undertakes crane or Equipment operation.
