IV. FUNCTIONAL HEALTH PATTERNS Before hospitalization The patient had no history of hospitalization.

The patient maintains her personal hygiene by taking a bath every day, brushing her teeth twice a day. The patient was taking some food supplements like vitamins and energy drink. The patient eats all kinds of food. She eats 3x a day, 1-2 cups of rice per meal. She has a good appetite and likes to eat rice, vegetables, fish, fruits and sometimes fatty, sweet and salty food. The typical daily fluid intakes are coffee, soft drinks and water. She drinks 8-10 glasses of water a day. The Pt. defecates at least once a day or sometimes once every 2 days. Her stool is soft in consistency and light-brown in color. The patient urinates frequently (56 times a day), amber in color without any odor. The patients form of exercise is jogging. She also considers walking as her form of exercise. She is the one who cook food sometimes. She sleeps 4-5 hours a day, irregular sleep due. She usually sleeps at 2 am and wakes up at 6 am. She also takes her nap every afternoon. She watches television while she is resting. During hospitalization This is the first time that the patient was hospitalized. She does not maintain her personal hygiene care due to restriction. She is in complete bed rest without bathroom privileges. She also hopes that she will get fast recovery. The patient eats 3 times a day and still in a good appetite. She eats fruits and vegetables. She drinks 6-8 glass of water a day.

She has no difficulty in defecating. She frequently voids of about 6 to 7x a day, amber in color without any odor.

She just stays at her bed. She is in CBR w/o BRP.

Her sleep hour has increased to 5-6 hours a day. She sleeps every time she wants.

F. Cognitive Perceptual pattern She can do whatever is instructed to her. she can also hear whisper voice. She uses eyeglasses. G. Self-perception, Self-concept pattern She views herself as a kind, industrious, patient and peace loving person. She expresses worry about his condition and concern for his family but states no difficulty in dealing with his condition.

H. Role-Relationship Pattern Her family is intact and close with each other. Conflicts arise between them but they manage to resolve them easily. She has a good relationship with her husband including her in laws, friends, neighbors and relatives. I. Sexual-Reproductive Pattern She lives with her husband. Her first menstrual period was when she 13 y.o., regular menstruation and sometimes feels dysmenorrhea. J. Coping Stress/ Tolerance Pattern According to her, she seeks her husbands advice whenever she has problems. She is very anxious of his condition. She copes with his condition by taking enough rest and sleep and by following his doctors orders. K. Value-Belief Pattern She is a Roman Catholic. She sometimes attends mass religiously and she use to pray before going to sleep. She doesnt believe in any superstitions but he respects these beliefs. She perceives God as the center of their lives and the source of strength whenever she is not in the state of good health.
