Licensing of Lifting Operations


Under RoK Rules A State License for Exploitation of Lifting Equipment Facilities is REQUIRED for entry Into the Site. This is Required of ALL Contractors and SubContractors who Use and Direct Cranes.

Certificates shall be in the format as required by RoK rules They shall be Up to date. They shall have the appropriate GGT/MI stamps and photos, Including Company data. This is for all persons previously identified. They shall have also had the Agip KCO site safety induction.

For Safe Crane Operations, this is one of the Most Important Positions in your staff. He is the person who is field responsible for the safe operation of all you field cranes. You may need more than 1 person to fill this role. All lift plans and lifting permit to work orders are his responsibility.

Written Procedures
I.A.W. Schedule D of all Agip KCO contracts you shall have written procedures for Lifting Operations. Ref.1.7.3 Also , RoK Section7.4.30, 31 and 7.5.15 refer to procedures and Special lifting Operational requirements. Lift Plans are Required for ALL Multiple Crane Lifts within the Project. No Exceptions

I.A.W. Schedule D and RoK Rules Section 7.5.6 . Written procedures for Crane Operations Shall be followed as WRITTEN. This includes Lift Plans Developed for Unusual Circumstances If They Can NOT be followed, they Need to Be re-Written to proceed.

Equipment Condition

Examination of the cranes components and safety features for compliance and condition is considered COMPREHENSIVE. The number items are far too numerous to cover in a presentation such as this, but are the very heart of crane safety. We at Agip KCO HSE require a high Degree of Quality in this Evaluation Process

Equipment on site that DOES NOT meet the Minimum Criteria as Outlined in the RoK Rules will stop working and PROPER Repairs Will be Made or Their Permits will be removed and they will be Asked to Leave the Site. Equipment entering the Site will be Denied Entry until Proper Repairs are Made. Our Continuous Auditing of Crane Operations Will Insure the High Degree of Safety is Maintained Throughout the Site.

Rigging Equipment and Logs


All Rigging Equipment used on this Agip KCO Project are subject to RoK Rules and Regulations Sections 7.3.28, 7.5.14, Appendix 10 and Appendix 13 Logs or Records of purchase, inspection, testing and certification shall be kept and appropriate audits will confirm compliance or disclose non-compliance with the RoK Rules. Shall be I.A.W. current Internationally accepted practice unless it is determined to create a safety hazard, and then referred to the original maker for advise. All other, nonstandard terminations will be evaluated for safety performance.

Training Evaluation Process

RoK Rules for the evaluation and training needs of selected Crane Operating and Rigging personnel are outlined in sections 7.4.2 and 7.4.25. Formats for documentation are in Appendix 14 and 16

Audits and Inspections

Detailed Inspections are the Responsibility of the Crane Owner, I.A.W. RoK Rules sections 7.3 and 7.4 and are to be performed on a regular Basis. Daily Inspections are to be performed by the Crane Operator I.A.W. RoK section 7.4.31 and recorded in the Log book (see Appendix 17) Rigging Equipment is to be inspected before use by qualified Rigging Personnel. All Records shall be I.A.W. RoK Rules Including Rigging Equipment.

Audits and Inspections continued


Agip KCO HSE routinely Audits Contractors for Compliance with the fore mentioned RoK Rules. The Results of these Audits are Summarized and Recommendations are sent to Agip KCO project management for Actions as Needed to Ensure that Safety in the Work Environment is Kept at a High Level.

Action Items
Under the RoK Rules section 8.0, various actions are listed for Violation of the Rules as Stated. In addition Agip KCO management has a Responsibility to uphold these Rules and the Rules outlined in Schedule D of all Project Contracts. Actions for Correction of Rules Violations Shall be made as timely as Possible.

Incidents

Any Incidents involving Cranes and Rigging MUST be Reported to Agip KCO Immediately. RoK Rules of Section 9.0 are the Responsibility of the Crane Owner, and are part of the Agip KCO Commitment to the Safe Use of Cranes on This Project Please Report any and All Incidents or Near Miss Incidents to Agip KCO HSE
