Sub: Health Insurance Appeal Letter Dear Sir, I am Mr. Sandeep Mishra making an appeal with your health insurance company. I have got admitted to Leelavati Hospital in Bandra, because I am suffering from a viral fever. Due to which I had to undergo for 7 days observation and been hospitalized. The entire cost of my recovery at hospital is 15000 Rs which includes hospital bill and medicine expenses. I have taken a special health care insurance policy from your insurance company for which my policy number is HI-875021. I am sending you all necessary documents needed by you. Please prefer my case in a very special manner because the amount has to be paid to the hospital authority after my discharge which is on 20th December, 2012. Yours Sincerely, Mr. Sandeep Mishra

Dear Sandeep, This is to inform you that your claim for medical insurance, Policy No HI-875021is being approved by our company as per your documents were found proper and all premiums are paid by you on time. As per norms of the company, we are liable to pay your claim. We are sending you the amount of Rs 15000 by cheque through your Insurance advisor on 20th December on the day of your discharge .
