Assessment Phase

The family health process involves conducting an initial assessment to determine the presence of any health problem.

Shows whether there are deviations in the health condition of individual members

Ability and willingness of the family to promote wellness among its members

Shows the nature and the extent of the familys performance of specific tasks to meet the physical, social and emotional needs of its members

Family Environment

Defines the condition in the home and environment that interfere with the promotion and/or maintenance of the family members health and recovery from illness

Individual health assessment

Awareness on community resources and organizations

Wellness Condition

Stated as POTENTIAL or READINESS

A clinical or nursing judgment about a client in transition from a specific level of wellness or capability to a higher one

Ex. Potential for effective breastfeeding

Health threats

Conditions that are conducive to disease, accident or failure to realize ones health potential

Ex. Medical History of recurrent UTI

Health Deficits

Instances of failure in health maintenance (disease, disability, developmental lag)

Ex. Head of the family was diagnosed for Emphysema

Stress Points/ Foreseeable Crisis

Anticipated periods of unusual demand on the individual or family in terms of adjustment or family resources

Ex. Unplanned pregnancy

Second Level Assessment

Second-level assessment data include those that specify or describe the familys realities, perceptions about and attitudes related to the assumption or performance of family health tasks on each health condition or problem identified during the first-level assessment
