HIPOPARATIROIDISMO

DEFINICIN
Entidad clnica donde hay una produccin insuficiente de PTH que no es capaz de mantener los niveles de calcio del lquido extracelular dentro de los rangos normales.

Cuando las concentraciones sricas de PTH no es capaz de ejercer su funcin a nivel de los rganos efectores para mantener el calcio dentro de los niveles normales.

FISIOPATOLOGA

La PTH estimula la sntesis de vitamina D por el rin; la vitamina D aumenta la absorcin de calcio en el intestino, lo que ayuda a aumentar el nivel de calcio en sangre.

Las causas se dividen en tres grupos:

Se originan en las propias glndulas paratiroides, hay una disminucin o ausencia de PTH en sangre y niveles de calcio constantemente bajos (hipocalcemia) y niveles constantemente altos de fsforo (hiperfosfatemia).

Hay una falta de PTH e hipocalcemia debido a un fallo de las glndulas paratiroides provocado por bajos niveles de magnesio en sangre (hipomagnesemia).

La PTH se fabrica y se libera a sangre pero esta PTH no es eficaz porque los rganos donde tiene que actuar (hueso y rin) no responden a la hormona, por lo que se produce una baja del calcio en sangre.

Los signos y sntomas agudos de la hipocalcemia se presentan con relacin a los estados de mayor demanda de calcio. Tetania latente o clnica Papiledema. Crisis convulsivas.

SIGNOS Y SNTOMAS
Los signos y sntomas del hipoparatiroidismo de cualquier etiologa se manifiesta por irritabilidad neuromuscular latente o evidente. Se expresa como tetania latente (signo de Chvostek y Trosseau positivos) o se manifiesta con parestesias y convulsiones.

TRATAMIENTO
Hipocalcemia aguda (>7,5 mg/dl) o signos de tetania: Los pacientes con tetania deben recibir calcio por va

intravenosa como Cloruro de calcio (272 mg de calcio/10 ml) Gluconato de calcio (90 mg de calcio/10 ml) Gluceptato de calcio (90 mg de calcio/10 ml)
Puede administrarse por VI lenta en dosis de 400 a 1 000 mg/24 h hasta que la

terapia por V.O haya surtido efecto. El calcio irrita las venas, y es mejor administrarlo en una vena de gran calibre o a travs de un catter venoso central.

TRATAMIENTO
Hipocalcemia crnica: El objetivo es conservar al paciente libre de sntomas, y

mantener un [Ca2+] srico de aproximadamente 8.5 a 9.0 mg/dl.

Las piedras angulares del tratamiento son calcio y una forma de

vitamina D.
El calcio por va oral puede administrarse en una dosis de 1.5 a

3 g de calcio elemental o ms al da.

Seudohipoparatiroidismo
Comprende un grupo heterogneo de enfermedades

que cursan con hipocalcemia, hiperfosfatemia resistencia a la PTH y con concentracin elevada de esta hormona. Son defectos de la protena ligante, estimulante: guanina nucletido. Se distinguen varias formas clnicas del cuadro.

Diagnstico.
El diagnstico de hipoparatiroidismo se realiza en

base a la deteccin de un calcio en sangre bajo. Los valores de fsforo y de PTH sern diferentes segn la causa del hipoparatiroidismo.
