Incidents that Happened • At approximately 1420hours on the 6th May, 2009 during Preparation of High-potential

At approximately 1420hours on the 6th May, 2009 during Preparation of High-potential testing on power cable with tag No. P-2603, 3CX350kcmil at GTG ‘C’, CNL QA/QC pointed out the possibility of an Incident from the cable which was not adequately protected on both ends of the cable.

Lessons to be Learned  Ensure that all cables and connectors are properly identified and corrected.

Ensure that all cables and connectors are properly identified and corrected.

Ensure personnel's are not present on work site and are restricted during test.

Ensure adequate house keeping on site. WRPs’ should ensure they provide adequate supervisory coverage in all their work areas. Ensure HIPOT accessories, cables and equipments are inspected regularly.

Test operator must be alert due to presence of dangerous voltage
