Our

mission is to conduct a thorough investigation of the companys two crane accidents. Clear written procedures will be implemented and monitored. This will ensure a safe and profitable future for the company.

Implement

a thorough investigation of the accidents in full cooperation with OSHA. Make necessary adjustments to procedures or implement new procedures that will eliminate the root cause of the accidents. Present to OSHA, insurers, and customers proof that the root cause of the accidents has been eliminated.

customer, employee, and public relations areas to ensure that clear written training procedures will be evaluated, implemented, and monitored. Will refer any legal issues to the companys lawyer of choice. Will plan and manage project within the Team United unit.

United will treat each other with respect. Every member will give their full cooperation and have open lines of communication. The team will listen to all ideas and suggestions without interruption. All members are responsible for completing assigned tasks on time. Maintain a positive attitude toward reaching the team goals.

Investigation
We have hired an independent firm to investigate the incident for the root causes of the accidents. Findings

The crate was improperly erected. Bolts that holt the turret to the vertical base were not torqued to the proper specifications. Operator failed to do a daily check on equipment. Crane was over stressed lifting a load that was to heavy.

Time Line

Training

We have contacted a crane training company. CraneTech is going to visit out crane sites.

CraneTech is going to give instruction to all operators and all operators will be certified under NCCCO (National Commission for the Certification of Crane Operators) regardless if they already have their certificate. They will provide training to our operators and riggers. They will be visiting all of our active sites and provide site specific training. CraneTech is also creating a safety check list. This is a daily operator checklist to be competed daily. Also to be signed off by the site manager.

Training Budget
Initially,

Training is going to cost about 3 thousand dollars per site.


The checklist is going to cost about 500 dollars for its creation.

Executive Summary

Our company was able to incorporate investigation, training, public/customer relations, legal language, and employee relations into our six week solution. We believe that the implementation of our plan will be extremely effective and will eventually benefit our company. We were able to overcome our organizational barriers and we will continue to focus on the future of our implementation of our plan.
Our company has been able to maintain our operating principles throughout our problem solving journey and we plan to keep our operating principles one of our main priorities throughout our implementation of our plan.

Executive Summary (Cont.)

Our independent investigation firm reached their conclusion on the primary causes of our crane accidents. We immediately started planning and decided to have Cranetech visit our crane sites. Instruction will be given to all operators within our company from this point forward.
