infestadas por vectores del dengue

sistemas de servicios

INTRODUCCIÓN

 En el Perú los primeros casos de DC se


detectaron en 1990
 El vector A. aegypti fue detectato en Lima en
el 2000
AGENTE ETIOLOGICO

 Virus del dengue pertenece al genero de


Flavivirus
 Periodo de incubacion es de 3-14 dias
 Virus de 45 – 50 nm
 Es labil a 56 C por 10 min, luz ultravioleta,
detergentes, desinfectantes y formaldehido
AGENTE VECTORIAL

 El principal vector del dengue es el mosquito


Aedes aegypti
 El riesgo de infeccion aumenta en la estacion
lluviosa
 Se reproducen en aguas estancadas,
alcantarillas abiertas y otras depositos de
agua.
AGENTE VECTORIAL

 El Aedes aegypti fue erradicado en casi la


totalidad de las Américas en los años 50 y 60
 Peru la erradicacion certificada en 1958
 En octubre de 1984 el mosquito fue
detectado nuevamente por funcionarios del
MINSA
EPIDEMIOLOGIA

• Nuevas infeciones al año: 50 millones
• Muertes: 24,000 annually
• Personas en riesgo: 2.5-3 billion
• Hospitalizaciones: 500 000/year

 Antes de 1960 el DC y de 1981
 70 , dos epidemias azotaron islas caribeñas
de Jamaica, Puerto Rico, Antillas Menores, y
al país sudamericano de Venezuela
 70, estos dos serotipos se expandieron a
Colombia
 1977 el dengue 1 fue introducido en las
Américas

los países de América, manteniéndose
durante los últimos 20 años con brotes
cíclicos cada 3 a 5 años.

SITUACION DEL DENGE EN
SUBREGION
 primer trimestre del 2009, se describen
brotes en Guayana Francesa, Aruba,
Guyana, Bolivia, Paraguay y Argentina .
 Hasta la Semana Epidemiológica (SE) Nº 11
del 2009 se han reportado 113.758 casos de
dengue, incluyendo 2.052 casos de dengue
hemorrágico y 42 defunciones

SUBREGION ANDINA
 Esta subregión contribuyó al 19%
(n=819.466) de casos de dengue en las
Américas entre 2001 y 2007
 Los serotipos de DEN-1 2, 3 y 4 circulan en
esta subregión
 En lo que va del 2009, esta subregión ha
reportado 53% (n= 61.262)

EPIDEMIOLOGIA – PERÚ
 Perú la reemergencia del DC se inicia en
1990
 brotes importantes de dengue, fueron el
2001, en los distritos de Sullana y Pariñas
(Piura), Trujillo y El Porvenir (La Libertad) y
Jaén (Cajamarca)
 La población de 15 a 39 años fue la más
afectada en los últimos 10 años
 El riesgo de contraer la enfermedad se
triplicó entre los años 2003 y 2004
EPIDEMIOLOGIA – PERÚ
 2003 los 4 serotipos de VD circulaban en
Piura y Tumbes
 ser 1, 2 y 3 en Ucayali, San Martín, Loreto y
Huanuco
 13 de abril del 2005 la Oficina General de
Epidemiología tuvo referencia por la
Dirección de Salud Lima Norte, de la
presencia de 11 casos febriles con erupción
cutánea en Comas
EPIDEMIOLOGIA – PERÚ
 14/04 se realizó la investigación de campo con la
participación de la OGE y el INS, para determinar la
etiología de la enfermedad y tomar las medidas de
control y prevención necesarias.
 27/04, el MINSA en nota de prensa, confirma 118
casos de DC en Lima
 19/05 el INS presenta 161 casos por muestras
positivas producidas en Lima .
 27/05 el consolidado de, casos para la región de
Lima fueron de 791 casos de dengue
EPIDEMIOLOGIA – PERÚ
 30/05 luego de la vigilancia e investigación
epidemiológica, se concluye que existen
evidencias del cese de la actividad
epidémica del dengue en el distrito de
Comas

Treatment of Dengue Fever
& DHF I & II
 Fluids

 Antipyretics (avoid aspirin and non-
steroidal anti-inflammatory drugs)
 Monitor blood pressure, hematocrit,
platelet count, level of
consciousness
Treatment of DHF III & IV

All above treatment +


– In case of severe bleeding, give fresh whole
blood 20 ml/kg as a bolus
– Give platelet rich plasma transfusion
exceptionally when platelet counts are below
5,000–10,000/ mm3 .
– After blood transfusion, continue fluid therapy
at 10 ml/kg/h and reduce it stepwise to bring it
down to 3 ml/kg/h and maintain it for 24-48 hrs
Treatment of DHF III & IV
•1 unit of RD(Random Donor) Plt. (50ml) per 10 Kg body
weight.---- expected to increas the Plt. Count 5000-
10000/uL. (If No splenomegaly, Fever or DIC)
•Alloimmunized (who have received multiple transfusions and
thus sensitized) may have little or no increase in the count.
•They can be best served by SDAP(Single Donor Apheresis
Platelets) as 1 SDAP unit(150ml)=6 RD units
Evaluation of Refractoriness of RD units
Post transfusion count – Pre transfusion count X BSA
CCI= Number of Platelets transfused X 10 11
Appropriate if
-CCI is 10X10 9 /ml in 1 hr post transfusion sample and/or
-CCI is 7.5X10 9/ml in 18-24 hr post transfusion sample.
Treatment of Dengue Fever

Papaya Juice vs. Dengue ?


Raw papaya leaves, 2 pcs just cleaned and
pound and squeeze with filter cloth. You will only
get one tablespoon per leaf. So two tablespoon per
serving once a day.
Do not boil or cook or rinse with hot water, it will
loose its strength. Only the leafy part and no stem
or sap.
It is very bitter and you have to swallow it like
Won Low Kat. But it works.
