o Assist in validation (DQC) of Monthly FHSIS Report for the month of May
2017
Remarks:
o DQC had uncovered the following sources of inaccurate reports: not all
services were recorded in the FHSIS recording tools; some Target Client List
is not updated; also some services are not recorded properly according
to the procedures for accomplishing the Target Client List; and recording
of some services did not follow definition and formula for estimating
service utilization.
o Assist RHM and PHN regularly in consolidation and validation of reports.
Thorough analysis of the report is important for a precise data entry

o Assist PHN/MHO in the initiation of anti-TB treatment (may list name of
patient(s))
o Reinforce health teaching on the duration of treatment, monitoring, DSSM
follow up, assessment of contacts especially children <5 y/o, possible
side/adverse effects of the treatment, DOTS strategy, and compliance to
treatment regimen
o Masterlisting and validation of data for TSEKAP reporting (May list name of
patients provided with TSEKAP services)
Remarks:
o Establishing trust and consistent rapport between public health
workers and patients is critical to gain full information and long term
cooperation during treatment.
o Observe proper standards of dealing with patients.

Activity:
o Interagency meeting with Local Disaster Risk Reduction and
Management Council (LDRRMC) in planning for Municipal Wide Disaster
and Response Drill
o 2nd Quarter Earthquake drill
o Ensure Functionality of Health Emergency Management System (HEMS)
Remarks:
o There were some aspects in the procedure that still need to be
improved.
o Organize meeting involving major stakeholders.
o Assist in formulating a unified plan that addresses the structural flow of
response in event of disasters
