 There are four states of Behaviors.
 Rushing( when you exceed the pace while doing any task)

 Frustration (Relationship inside & Outside the


organization.malfunctioning,inedquate tools, conflicting obj,Presscure)

 Fatigue (To tired Physically or Mentally)

 Complacency (“Self-satisfaction especially when


accompanied by unawareness of actual dangers or deficiencies.”)

engineer.

Business plan to all concern
Deptt, How many have gone
through it.
Negative Behaviors.
 We still Think that only HSE personnel have
the Responsibility to note Hazards.
 We will perform safe work only when HE is
there.
 Only HE has the responsibility to highlight our
mistakes.
 HE has the responsibility to fill the relevant
document.
NEAR MISSES:
 UNSAFE BEHAVIORS DON’T ALWAYS RESULT IN
ACCIDENTS
 NEAR MISSES ARE GOLDEN OPPORTUNITIES TO
PREVENT FUTURE ISSUES
 DON’T IGNORE UNSAFE BEHAVIOR
 WE DO A LOT OF UNSAFE THINGS & GET AWAY
WITH IT.. DOES THAT MAKE IT OK?
 EVENTUALLY IT WILL CATCH UP TO US

80-85% of all incidents are the result of unsafe acts;

15-20% of all incidents are the result of unsafe


conditions.

Question: If true, why are most efforts geared


towards unsafe conditions?
Answer: It’s easier to deal with unsafe conditions
than unsafe acts.
