Chief Complaints :
Difficulty in chewing on left lower jaw
 According to the statement of the patient, his lower left first
molar was endodontically treated at conservative dentistry &
endodontic department in BSMMU three months back. After
completion of RCT his tooth was gradually broken down. So
he was referred to department of prosthodontics in BSMMU
for better management.
Clinical findings:
Extra oral:
Face form : Tapering symmetrical face
Face profile : Convex
Lip : Competent
TMJ : Normal
Clinical findings:
Intra oral:
Oral hygiene: Average
Mucosa : Normal
Restoration : Right lower 1st & 2nd molar and
left lower 1st molar.
Arch form: U shaped.
Saliva: Normal.
Clinical findings:

Plaque index : Score 1.

calculus index: Score 1.

Occlusion:
Incisor relation: Class 1
Canine Relation: Class 1
Molar relation: Class 1
Lingually placed left lower second premolar.
Dental history:

RCT on left lower first molar followed by miracle mix

restoration three months back .
Old restoration on right lower first & second molar.
Investigation:

X –ray: Periapical view on left first molar.

Diagnosis:
Endodontically treated left lower 1st molar tooth with
broken down crown.
Treatment plan:
Dowel core & crown following surgical crown
lengthening of mandibular left first molar.
Procedure: Endodontically treated left mandibular
first molar tooth with broken down crown
Crown lengthening and periopack
dressing
Gingival Margin after crown

Removal of restoration
Removal of GP by peeso reamer & widening of
canal
