DISCHARGE PLANNING
DEFINITION
Kozier (2004) defined discharge planning as a
process of preparing the patient to leave on
service unit to another unit in or out of a public
health agent.
OBJECTIVES

 * The main objective is to help clients and families achieve optimal levels of
health
 * The purpose of planning the return of patients is
 to improve the understanding of patients and families about health problems,
possible complications and restrictions imposed on patients in hospitals.
 "Develop the ability to care for patients and families to meet patient needs and
provide a safe environment for patients.
 Ensure that the referrals needed for further care are made appropriately.

PRINCIPLES

• some of the principles put forward by The Royal Marsden Hospital


(2004), namely:
• Discharge planning procedures must be carried out consistently
with high quality in all patients.
• The need to provide care (care giver) must also be assessed.
• The patient must be discharged to a safe and adequate
environment.
• Information about the preparation of repatriation must be
informed between the health team and the patient / care giver
TYPE OF RECOVERY :

• Conditional discharge.
• Absolute discharge (absolute or permanent return)
• Judocal discharge (forced return)
