• Date of Admission: 5TH MARCHvia OPD
• RESIDENT of Wazirabad
Presenting Complaint

• Unable to bear weight on both legs--- 3 months


• Generalized body pain --- 3months
History of Presenting Complaint

• My patient was in usual state of health four months back


when he suddenly became unable to bear weight on both
extremities. He experienced pain on standing that was in
pelvic area,groin area and it was diffuse,continuous,
radiating to both extremities and not relieved by any
medication.he consulted orthopaedic surgeon in his area
and was diagnosed with multiple fractures.
• According to patient 10 years back when he was going to
school he suddenly felt pain and swelling in his right
thigh.it was sudden in onset without any history of
trauma or fall.he had difficulty in walking after that and
was never able to walk without support and
independently since then.
• Normal birth history

• Milestones achieved normal

• No medical or surgical history

• Contact history of TB is positive


General physical examination

• A young male with short stature lying in bed with wel


oriented in time, place and person having following vitals
• PR 97/min BP 110/60 mmHg Temp 99 F RR 14/min

• Pallor +ve jaundice –ve lymphnodes not palpable


Local Examination

• Inspection:both lower extremities are in a posture of int


rotated and generalized swelling from thigh regions till
foot on both sides
• Palpation: deep palpation is painful at thigh,groin and
knees
• Movements: no joint active movements at hip,knee and
ankle
• Power:
muscle power of both lower limbs
• Dorsiflexors 3/5 Plantar flexors 3/5
• Knee flexors 2/5 knee extensors 2/5
• Tone:
Normal
• Reflexes:
normal
