NOMINATION
PROCESS
1. The LGU-Claver PRAISE Committee will hold the nomination
for the following awards:

I. INDIVIDUAL CATEGORY (Monthly Awards)


- Best in Attendance Award
- Punctual Employee Award
The employee will receive the award based
on the attendance and daily time record
validated by HRMDO

NOMINATION PROCESS
1. The LGU-Claver PRAISE Committee will hold the nomination
for the following awards:

II. INDIVIDUAL CATEGORY (Semestral Award)


- Customer Service Award
Nomination for the Best Office Award is open on
the first Monday of the month of January (1st
semester) and on the first Monday of the month of
July (2nd semester) . Deadline of submission of
nominations is on March and October respectively.

2. Nominations may be from individuals or teams and
can be made by:
- Self-nomination (supported by the
nominee’s Supervisor);
- Peers
- Immediate supervisor
- Department Head
- Chairperson from concerned committees
- Human resource management officer

3. Nomination Forms may be secured from the
Municipal Human Resource Management and
Development Office (pls. see attached form A)

4. Nomination submitted beyond deadline will not be


accepted.

1. All nomination form will be submitted to HRMDO,
on or before the specified date per category.

2. LGU-Claver PRAISE Committee shall conduct


validation (documentary, interview of peer and
immediate superior/department head and on-site if
needed) to determine the truthfulness of the
nomination.

3. The PRAISE Committee shall adopt averaging
computation based on the provided criteria in
deciding for the recipient of specific awards. (Pls. see
attached form B)

4. The PRAISE Committee shall schedule the


interview for the nominees of the various award
categories on ____________.(Pls. see attached form
C1 and form C2)

5. LGU-Claver PRAISE Committee member who is a
nominee, or is the supervisor of the nominee or has
nominated a candidate for any award must declare
his/her conflict of interest and not participate in the
deliberation or voting only in relation to the award
category that is at issue.

6. The PRAISE Committee will evaluate the merits of


all award nominations against the relevant selection
criteria.

7. The PRAISE Committee adopts the weighted
scoring of 80% for the rating based on criteria and
20% rating based on the interview to come up with
the final scoring as basis in determining the awardee.
(Pls. see attached form D)
The PRAISE Committee endorses and recommends
via resolution to the Municipal Mayor for approval.

8. The Municipal Mayor shall approve the
recommendation from the PRAISE Committee after
his/her thorough finding that the same is in order and
in compliance with the parameters set by the Agency
PRAISE and this Rewards and Recognition Plan.

9. The decision of the Local Chief Executive is final


and irrevocable.
