(Risk) - (the assessed chance of the potential occurring)

We must report all near-misses, as accidents are always repeats of near-misses
& other accidents.

The difference between a near-miss & death = (maybe only 1/2”)

All accidents are preventable


An accident is an unplanned event which has resulted in bodily/property damage.

Punishment and Disciplinary action should be taken when,

a. Blatant disregard for safety rules and procedures


b. Un-professional conduct on the behalf of the supervisor.

- reward good safety behavior
- decrease disadvantages of safe behavior
- explain potential of unsafe acts
- close down unsafe acts

Not How and accident happened, but, Why an accident happened.

An accident investigation is only the Means to an end.

Causes of an Accident

1. Direct cause
2. Root/contributory cause, (had an effect)

Accident Investigation Process
1. Preserve the scene
2. Interview witness individually
3. Don’t allow witness to talk to each other (if poss.)
4. Take pictures and notes (write everything down)

All conclusions must have a recommendation following
Never end with conclusions, always end with recommendations
