Those who have BP >140/90 on both readings
shall be referred to Barangay Health Station. ENROLLMENT All patients with Chronic Lifestyle Related Non-communicable disease shall be encouraged to enroll in appropriate heath club in their respective health centers.
Registered members shall be issued with an ID and patient
booklet.
The club member shall be informed of scheduled activities
from which incentives can be provided if attended (i.e. ZUMBA) ESTABLISHMENT OF OTHER HEALTH CLUBS Each health club shall elect their own officers and they are encouraged to meet at least once a month and election every yea. CORE ACTIVITIES -Lifestyle improvement activities such as changes of diet, increased physical activities, smoking cessation and reduction of alcohol intake. -Health education -Periodic BP and Blood Sugar assessment Replenishment of free medications -Stress Management MEMBERSHIP AND ID -Each club shall be given a unique chronological Club Membership Number

-Peer group engagement in health education, regular activities
like Zumba and sustained medication to ensure continuity of care leading to better health outcomes.
-Social activities like get-togethers to further raise the
understanding of health messages (ex: meals served should be diabetic-friendly or hypertensive friendly)
-Own fun raising activities to get free or discounted laboratory
tests such as cholesterol and ECG, among others. -Involvement of family members to encourage participation especially health promotion activities.
