The implementation of the 2016 Wellness Campus Program will be in


three parts, as follows:

Part 1: NUTRITION EDUCATION FOR PUBLIC HIGH SCHOOLS AND GRADE


SCHOOLS IN ALL PARTICIPATING REGIONS.
1. NPI will provide the Nutrition Modules, Teaching Guides
Flyers, Posters, and streamers:
a. Nutrition Module (Video) – Grades 4 to 6; Grades 7-10
b. Nutrition Module Guide for Teachers – Grades 1 to 3;
Grades 4 to 6, and Grades 7 to 10
c. Dancercise Music
d. Dance Instruction Video
e. Posters or streamers, and flyers

2. Integration of the Nutrition Modules in the MaPEH/PE curriculum subject to


DepEd rules and regulations.

3. Videos can be downloaded at www.nestle wellnesscampus.com.

Part 2: THE 2016 NESTLE CAMPUS DANCERCISE FOR ALL COVERED


PUBLIC SCHOOLS.

a. We have a new music and new dance moves:


1. Elementary – Champ Moves Dancercise
2. High School - Wellness Dancercise

b. Copy of Dancercise videos will be available at Nestle


Wellness Website.

c. The music video provided by NPI will be the required music


and dance during flag raising activities and warm-up
exercise during MAPEH classes.

Part 3: WELLNESS CAMPUS REGION OF THE YEAR CONTEST


The contest aims to get the best ideas, best practices and find
better ways or approaches to effectively educate our student
participants as we continue to implement the Program in the
schools in the years to come.

Eligibility:
1. Only the four participating regions are qualified to
join.
2. Only students from grade 1-6 for grade school and
Grade 7 to 10 for high school.
3. Participating schools must submit the duly signed
Parent Consent waiver

The contest will be in two parts namely:


I. Part 1 - Implementation – 50%
2.Part 2 - Case presentation – 50%

Part 1 : Implementation Criteria (50%)

(Based on the percentage of schools who have implemented the


activities versus the total number of schools in the region)

1. Submission of parent’s consent and waiver = 10%

2. Implementation of dancercise in flag raising ceremonies = 25%

3. Implementation of nutrition module in MAPEH = 40%

4. Submission of program portfolio = 25%

Qualifications of the Criteria:
1. Submission of parent Consent waiver:
a. delivery – during division cascades
b. retrieval will be from the division office ( schools are
encouraged to submit the duly signed form to the division
office).

2. Implementation of dancercise during flag raising ceremonies


a. Using the dancercise music provided by Nestle
b. Following the dancercise instructional video provided by Nestle

3. Implementation of nutrition module in MAPEH classes


a. Integration of the nutrition modules in MaPEH or TLE subjects

4. Submission of Program Portfolio:


a. 100% of schools in a participating region will submit a
program portfolio

PRIZE PACKAGES FOR - NCR
Validation Process:

Validation Process will be in two ways as follows:

1. 50% of Schools via Actual Visit (by GTI coordinators subject to Deped’s Approval)
 50% of the total school listed in both High School and Elementary
in a region will be visited to validate the implementation. Selection
of schools will be based on the student population.
• 50% of high school ( based on the total number of schools)
• 50% of grade school (based on the number of schools)

2. 50% Remaining will be via the Program Portfolio Submitted


 50% of the total population of schools in a region will be
validated through program portfolio submitted to the Division
office.

Validation will start 2 weeks after the scheduled cascade.

• A program portfolio should contain the following information:


a. How did you ensure the 100% and correct implementation
of the Program in your school?

b. How did you apply the learning from the modules? What
are your school’s best practices?

c. Implementation of Nutrition, Health and Wellness initiatives


in the region/schools to promote behavior change

d. If you were given an opportunity to further improve the


Nutrition, Health and Wellness of your region/school, what
programs/ activities would you implement?

• All program portfolio must be submitted to the Division Supervisor.

• Division Supervisor will make a power point report/presentation


and submit it to the Wellness Regional Team.

Part 2 : Case Presentation (50%)

• The regional office through the regional director or the duly assigned
representative will form a Wellness team consisting of 4 members
and a team leader (total of 5), which may be taken from among the
Division Supervisors, school representatives or representatives from
the regional office involved in the implementation of the program.

• Regional Wellness Team will collate and gather best


ideas and practices from the reports submitted by the
Division Supervisors.

• The Regional Wellness Team will then prepare a case presentation.

• Each Wellness Team from the four participating regions will be given a
maximum of 1 hour to present their case to the panel of judges. The
criteria for judging will be as follows:

1. Overall impact of the presentation 20%


2. Best Practice 20%
3. Program continuity plan 40%
4. Viability of the recommendation 20%
Total 100%

• The Regional Wellness Team will present the case at the


Nestle Center Makati City on a specified date which will be
on December 13, 2016.

• All expenses related to the travel to Nestle Center will be shouldered


by NPI through GTI.

12.The region that will get the highest average percentage score
will be declared as the “2016 Nestle Wellness Campus Best
Region of the Year”. The average percentage score for a region
will be derived by adding the total percentage earned in part 1
and the total percentage earned in part 2 divided by two.

Example:
Total percentage score in Part 1 = 90%
+
Total percentage score in Part 2 = 92%
Total 182%
Divided by 2
Average percentage score 91%

• The decision of the judges will be final and non-appealable.

• Judging and Awarding of winners will be conducted on the day


after the presentation of the DepEd Regional Wellness Teams from
the four regions.

• All expenses related to the travel in going to Nestle Center ( such as


meals, transportation and lodging) and in going back to the regions
will be shouldered by NPI.

• NPI has the absolute right to change the mechanics to ensure the
successful and orderly implementation of the Program.
