ROOT CAUSE ANALYSIS Tumbaga, Mary Jane
There are four loading pumps connected in parallel through
the main pipeline.
FACT
The line connects the diesel oil pump to the depot truck loading
gantries.
Two of the four pumps have been labelled “non-operational”
since 2017.

Automatic recirculation valves are installed to control the
discharge of the pump by opening the bypass pipeline.
The line has been observed with disturbing noise during times
of varying delivery rate to loading gantry.
