ROOT CAUSE ANALYSIS Tumbaga, Mary Jane
 There are four loading pumps connected in parallel through
the main pipeline.

FACT
 The line connects the diesel oil pump to the depot truck loading
gantries.
 Two of the four pumps have been labelled “non-operational”
since 2017.

 Automatic recirculation valves are installed to control the
discharge of the pump by opening the bypass pipeline.
 The line has been observed with disturbing noise during times
of varying delivery rate to loading gantry.

 U-clamps have been installed for temporary control of
