Requirements for Food Establishment:
Bacteriological Test, Vermin Control of Compliance, Health Card, Chest
Xray, Urinalysis, latest 1X1 picture, Seminar for Food Handler
Requirement for Non-Food Establishment:
Receipt of Payment
Submit the requirements to the Sanitary Inspector on Duty
Sanitary Inspector prepares the Sanitary Permit

Forms Needed to Apply:

• Application Form
• Barangay Business Clearance with Official Receipt; 2 copies; the original copy must submit to
the Permits and Licensing Office
• Tax Declaration of Lot and Building if owned; 2 copies
• Updated Tax Receipt where the Business is placed; 2 copies
• If the lot or building is not owned or
If rented; Submit the updated and notarised Lease Contract
If free with consent of the owner of the Lot or Building to use: Submit a notarised
authorisation from the Owner stating the consent of free of use; 2 copies and a valid ID
with signature of the Owner.
• Building Permit or Occupancy Permit; 2 copies
• DTI Registration or SEC, Articles and by Laws – photocopy
• Sketch of the Location of the Business
• Occupational Permit or Master List of Employees (If 5 above) with Community Tax Certificate
each employee
• Authorisation Letter with Valid ID and signature of the owner of the business if the owner is
not the one who will process the application of the permit.
