ницистом. В результате многолетней практики в области одновизитной эндодонтии автор
выработал собственные принципы современного эндолечения. Впервые для русскоязыч-
ных стран представлен полноценный протокол лечения периодонтитов за одно посеще-
ние. Описаны только те профессиональные приемы и методы лечения, которые опробо-
ваны автором лично на практике. Даны ответы на вопросы, которые задавались автору
во время его мастер-классов за 12 лет.

Важные мысли
x Любой эндодонтически леченный зуб со здоровым пародон-
том и адекватной реставрацией имеет огромные преимуще-
ства (биологические, функциональные и эстетические) по
сравнению с имплантатами.
x Основные преимущества одновизитной эндодонтии:
1) уменьшается вероятность инфицирования каналов меж-
ду посещениями;
2) меньше обострений и осложнений;
3) повышает доходность клиники.
x Полноценная хемомеханическая обработка системы кана-
лов в течение первого посещения уменьшает риск после-
операционных осложнений и дискомфорта.
x Среднее расстояние от физиологической верхушки (апи-
кальное сужение) до кончика корня — 0,9 мм.
x Расстояние от вершин бугров до крыши пульпарной камеры
составляет в среднем 6 мм, от дна полости до бифуркации —
3 мм. Средняя высота пульпарной полости — 1,5–2,0 мм.
x Дополнительные каналы в области бифуркации наблюдают-
ся у 21% моляров. Они соединяют полость зуба с периодон-
том.
x Для лечения апикальных деструкций одновизитная эндо-
донтия является современной и научно обоснованной аль-
тернативой лечению каналов за несколько посещений. Успех
лечения периодонтитов в одно посещение составляет ~78%.
С применением гидроокиси кальция апикальные дефекты
заживают примерно в 75% случаев (рис. 1.18, а–г).

Важные мысли
x 60% эндодонтических патологий протекают бессимптомно.
В частности, около 40% пациентов не испытывают боли при
пульпите.
x Самым достоверным симптомом при пульпите является
спонтанная боль.
x Рентгеновский снимок — это двумерный образ трехмерного
объекта.
x Если наблюдается сочетанное эндопародонтальное пораже-
ние, рекомендуется сначала проводить эндодонтическое ле-
чение.
x Прогноз лечения комбинированной эндопародонтальной
патологии в большей степени зависит от состояния паро-
донта.
x Вертикальный перелом корня характеризуется тем, что кли-
нически выявляются:
1) локальный зубодесневой карман, достигающий апекса
корня;
2) свищ, в основном ведущий к середине корня;
3) боль или дискомфорт при нагрузке;
4) признаки эндодонтического лечения зуба.
x До того как открыть доступ к устьям каналов, необходи-
мо полностью удалить кариозный дентин, старые пломбы,
штифты и дентикли. При этом применяется кариес-детек-
тор и ультразвук.
x Эндодонтический доступ считается полноценным, если при
определенном положении зеркала видны все устья каналов.

Важные мысли
x Для получения предсказуемых результатов необходимо
применение протокола лечения, предполагающего соблю-
дение определенной последовательности этапов во время
эндодонтического лечения.
x При любом диагнозе суть эндодонтического лечения зубов
заключается в тщательном удалении живых и некротизиро-
ванных тканей, микроорганизмов и их токсинов из канала
с последующей трехмерной обтурацией всей системы.
x Начальное инструментальное исследование и обеспечение
проходимости канала проводят только ручными инстру-
ментами.
x Всегда пульпарная камера находится в центре коронки зуба,
а канал — в центре корня. Канал всегда повторяет форму
корня.
x Кальцификация — это дегенеративный процесс, который
начинается вблизи мест повреждения или раздражения
пульпы (т.е. в пульпарной камере) и прогрессирует в сторо-
ну апекса.
x Для начального прохождения устьевой и средней тре-
ти узкого проблемного канала надо проводить только
step-back-расширение!
x В инфицированных корневых каналах ориентиром для пре-
парирования может быть только анатомическая верхушка.
x Точность апекслокаторов в районе физиологической вер-
хушки равна 50% в случае, если их точность у анатомиче-
ской верхушки — до 96%.
x Если придерживаться мифического для практики понятия
«физиологической верхушки» и все процедуры закончить
на этом уровне, то будет большая вероятность неполноцен-
ной обработки канала у апекса.

Завершающее промывание,
снятие смазанного слоя
Эндодонтические инструменты, независимо от того, стальные они
или NiTi, прямые или заранее согнутые, соприкасаются со стенкам
канала только в одном или двух направлениях [55]. Получается, что
главной целью инструментальной обработки основного канала явля-
ется открытие доступа и пространства для промывающих растворов
(ирригантов). Только ирриганты могут полноценно очищать сложную
систему корневых каналов и обеспечить дезинфекцию. Самым рас-
пространенным и эффективным средством для промывания считает-
ся 0,5–5,25% раствор гипохлорита натрия (NaOCl).
Желательно ирригацию канала проводить часто, после смены каж-
дого файла. Однако эффективное промывание начинается только по-
сле расширения каналов. В канале промывающий раствор достигает
глубины погружения эндодонтической канюли. Чтобы NaOCl до-
стиг апекса, надо параллельно использовать ручные файлы № 10, 15.
Baumgartner (1993) показал, что промывание каналов только раство-
ром NaOCl обеспечило чистоту гораздо больше, чем обработка только
инструментами. На самом деле корневые каналы могут быть очищены
и продезинфицированы с помощью ирригантов вместо ненужного пе-
рерасширения и уменьшения толщины дентина [15, 17, 23, 24].
На стенках канала во время его расширения и формирования об-
разуется смазанный слой, который состоит из органических и неор-
ганических компонентов. Это остатки пульпы, дентинные опилки,
бактерии и кровь. Смазанный слой может стать источником инфи-
цирования периодонта, он мешает дезинфекции дентинных трубочек
ирригантами, ухудшает прилегание корневой пломбы к стенкам кана-
ла, закрывает устья боковых каналов.

Важные мысли
x Современная эндодонтия должна быть трехмерной на всех
этапах лечения: начиная с диагностики и заканчивая герме-
тичной трехмерной обтурацией.
x Апикальная треть канала должна быть расширена только
после обработки устьевой и средней трети.
x После полноценной хемомеханической обработки, если уда-
ется идеально высушить каналы, можно уверенно обтури-
ровать их при любом диагнозе.
x Если полость зуба после лечения каналов осталась откры-
той в течение одного месяца или дольше, есть показание
к повторному лечению корневых каналов.
x Основными причинами вертикального перелома корня
являются чрезмерные усилия спредерами во время холод-
ной латеральной компакции и фиксация внутриканальных
штифтов.
x После эндолечения очень часто требуется восстановление
зубов реставрацией с бугорковым перекрытием или искус-
ственной коронкой.
x Обтурация эндодонта начинается у апекса корня и заканчи-
вается на эмали зуба. Она завершается надежной и герме-
тичной коронковой реставрацией.
x Поскольку никогда и никто не может быть уверенным
в стерильности канала после обработки инфицированного
зуба, целесообразно обтурировать канал до анатомической
верхушки, несмотря на то что при этом есть вероятность
выхода пломбировочных материалов за верхушку.

Диагностика
75% апикальных периодонтитов протекают бессимптомно
(Siqueria, 2001) [59]. Поэтому для стоматолога крайне важно точно
диагностировать эндодонтическую патологию и зуба с симптомати-
кой, и зуба бессимптомного. Нельзя ставить диагноз, опираясь только
на рентгенограмму, на которой видно очаговое просветление в об-
ласти верхушки. Для подтверждения клинического диагноза всегда
надо выполнить температурные тесты пульпы и ЭОД. Всегда надо
помнить, что рентгеновский снимок — это двумерный образ трех-
мерного объекта.
Примерно 34% апикальных периодонтитов премоляров и моляров
верхней челюсти остаются не выявленными с помощью рентгеноди-
агностики [52]. Эту патологию можно легко диагностировать с помо-
щью дентальной конусно-лучевой компьютерной томографии (КТ).
На КТ можно найти очаги периапикального воспаления, утолщение
слизистой гайморовой пазухи, которое сопровождает хронические
периодонтиты, найти каналы, переломы и резорбции корня. На КТ
можно пораньше судить о восстановлении периапикальной кости,
точно планировать резекцию верхушки, узнать параметры кости, от-
четливо показать пациентам клиническую ситуацию в челюсти. На
сегодня актуально применение дентальной томографии для диагно-
стики и лечения болезней периапикальных тканей [52] (рис. 6.7, а, б;
6.8, а–д).
Периапикальный очаг воспаления имеет четыре характерных при-
знака:
1) на рентгене костный дефект (затемнение) остается на апексе,
независимо от угла рентгеновских лучей;
2) костный дефект имеет форму капли;
3) всегда присутствует причинный зуб с некротизированной пуль-
пой [13, 62];
4) кортикальная пластинка лунки зуба отсутствует у апекса.

Важные мысли
x В долгосрочный период наблюдения повторное эндодонти-
ческое лечение обеспечивает лучший прогноз, чем апикаль-
ная хирургия.
x Количество бактерий за пределами канала существенно уве-
личивается только при остром или обострившемся хрони-
ческом периодонтитах.
x Микробы в периодонте встречаются в 6% случаев, в составе
биопленки.
x В апикальной трети количество дентинных канальцев
в 3 раза меньше, чем в коронковой и средней трети канала.
x Возникают острые клинические симптомы (боль, отек
и т.д.) тогда, когда бактерии выходят в периапикальные тка-
ни в большом количестве.
x Нет никакой связи между размерами рентгенологического
дефекта и присутствием бактерий в нем.
x Независимо от того, гранулема это или киста, большой или
маленький очаг деструкции, этиология эндодонтических
патологий одна, и требуется одинаковое лечение.
x Процент успешного лечения инфицированных каналов за
одно посещение такой же, как за несколько.
x Появление и интенсивность послеоперационных болей не
зависит от выхода инструментов за верхушку.
x Около 75% зубов с апикальным периодонтитом не имеют
физиологического сужения по причине воспалительной
апикальной резорбции.
x С точки зрения современной эндодонтии ориентиром для
препарирования инфицированных каналов может быть
только анатомическая верхушка.
x Основной причиной послеоперационной боли и обостре-
ний хронических процессов является выталкивание инфи-
цированных масс за верхушку.

Важные мысли
x Апикальная треть канала должна быть расширена только
после обработки устьевой и средней трети.
x Следует расширять апикальное отверстие только во время
лечения хронических периодонтитов, когда есть расширен-
ная периодонтальная щель и периапикальный костный де-
фект.
x Есть всего два противопоказания для пломбирования кана-
лов в одно посещение: стойкая экссудация из канала и не-
хватка времени.
x После полноценной хемомеханической обработки, если уда-
ется идеально высушить каналы, можно уверенно обтури-
ровать их при любом диагнозе.
x Есть два вида реакции организма на инородное тело: экзо-
генная (на целлюлозу, тальк, гуттаперчу, силер, соли каль-
ция) и эндогенная (на кристаллы холестерина).
x В 85% случаев зубы, удаленные после эндолечения, были без
искусственной коронки.
x Хирургическое лечение околоверхушечных проблем долж-
но проводиться по возможности только после консерватив-
ного эндодонтического лечения.
x Любая пустота в системе каналов до постоянной обтура-
ции — это инкубатор для размножения микробов.
x Основной причиной обострения хронических процессов
